It is observed that some of the Out-Patient medical bills submitted recently for reimbursement are not
genuine. Such act is not in line with our ‘Code of Conduct / Standing Orders’ and warrants strict
disciplinary action.

Claiming the fake bill is a sort of fraudulent activity that is done intentionally, strict disciplinary action
will be taken on employees, who misuse the medical benefit provided by our HMIL.

Genuine documents to be submitted for Out-patient & In-patient claims for reimbursement process.
As per Insurance norms, the below said documents (Original) are mandatory and only after
investigation / validation, claims will be processed.
1. Doctor consultation fee receipts required with Bill No, GSTIN and Bill date is mandatory. Bill
amount mentioned in Doctor’s prescription in not valid.
2. Doctor’s prescription for medicine bills (with detailed breakup) , investigation (with photo copy
reports) and Master health checkup is mandatory.
3. Online payment receipts duly signed and authorized seal required and online bills & receipts
through mail and mail print out is not valid.
4. Optical bills –Ophthalmologist Prescription is mandatory. Prescription from opticals (optometrist)
is not valid.
5. IP claims submission – Discharge Summary should be in hospital Letter-head (original) with sign
and seal / all investigation reports should be in original / medicines bills breakup / Advance
payment receipts to be submitted / hand written bills and discharge summary is not valid.
Note: Claims submitted without above said requirements for Out-patient & In-patient at medical
insurance helpdesk, shall not be considered for processing and settlement.

Please note that, the following Optical shops are blacklisted by our insurer and bills submitted from
these Opticals will not be processed.
