RE: PASSPORT APPLICATION FOR MR FREDRICK M MATOVU

Thank you for your passport application. Unfortunately you did not enclose all necessary
supporting documents.
WHAT YOU NEED TO DO:
Please provide at least two document(s) from the following list as proof of your address/residency
in Uganda (must not be photocopies). The document(s) should be dated within the last year or be
in their validity period
-National identity card or equivalent
-Valid visa or residence permit
-Tax record e.g. a letter from a tax authority
-Educational record e.g. school/college/university report
-Employment record e.g. official letter from your employer
-Letter sent to you from a central, regional or local Government Department
-Medical/health card
-Driving licence
-Voter's card
-Immigration documents
Where these are not available, consideration may also be given to bank statements, utility bills or
their equivalents. We can accept on-line bank statements and utility bills.

1) a selection of progressive family photographs which show you (Fredrick), from around the time
of your birth, up to the present date, with both of your parents, any siblings, and any other family
members. Also, provide some additional photographs taken, during the pregnancy and some of
your parents wedding celebration photographs with family and friends. Such photographs should
be clearly labelled on the reverse, stating the date and where they were taken, the full name of
each person on the photograph and their relationship to you.
2) Mother's (Ngobva Mukasa Teo SSERUWAGI) and (father's Ronald Mark Ngobya MUKASA)
Uganda/British passports at the time of Fredrick's conception which shows entry/exit stamps,
along with any other foreign passports that either parent has held. We require a copy of every
page.
3) You must send us a selection of documents which show both parents were together in the same
country at the time of Fredrick's conception, and that they continued to be in a family relationship.
The following is a list of acceptable documents: - flight ticket stubs/details - email confirmation
of flight or hotel bookings - evidence showing father being outside of the UK at the time of
conception. This may include documents such as bank statements or payslips showing annual
leave - landline or mobile phone bills showing regular calls to family overseas. Please note this list
is not exhaustive.
5) Mother's (Ngobva Mukasa Teo SSERUWAGI) antenatal records, including hospital appointment
cards, ultrasound scan images and reports, hospital birth/discharge records pertaining to the births
of Fredrick.
6) You (Fredrick) health and vaccination records, weight of your birth.
8) dates/names of any previous marriages which have taken place with either of your parents and
dates of divorce if applicable along with their divorce documents.
9) Mother's (Ngobva Mukasa Teo SSERUWAGI) Ugandan birth certificate. (original document,
photocopies not acceptable).
10) Father's (Ronald Mark Ngobya MUKASA) Naturalisation certificate issued by the Home Office
London. (original document, photocopies not acceptable).
You must include all your supporting documents with your application. You'll get 6 weeks to
provide any documents that are missing. Your application will be cancelled and you won't get a
refund if you don't provide them in time.
