HS INSPECTION REPORT
Pk 1 – Sacomar Iron Ore Terminal Date: 24.02.2024

1. INTRODUCTION

The purpose of this Safety Inspection Report is to describe the follow-up and monitoring of the work during
the period from 19th to 24th February 2024, recording the situations observed on Site, suggesting improvement
actions and/or requesting the correction of situations that do not comply with the established legal and
regulatory requirements by the Contractor that respects the safety conditions in the contracted Package 1.

2. ONGOING WORK

During visits to the Site according to the following activities:


• Temporary works:
• Worker accommodation – utility works
• Canteen – installation of roof trusses & block works
• Painting works
• Construction Works:
• Rail foundation for tower crane – installation of rebar & formworks for beams
• Rebar fabrication yard
• Assembly of Tower crane

3. VISIT RECORD

The Contractor must guarantee safety conditions on site for his workers, but it is also the responsibility of the workers to
make proper use of them when they have the means and equipment to carry out their work safely. The Contractor must
assess the state of knowledge of workers who inadvertently expose themselves to risk, when they have the necessary
means to carry out the work safely.
To ensure that events like this do not happen again and again, it is important to consolidate a safety culture from the
technicians right up to the top management.
The Contractor must carry out an awareness-raising session for all Project workers on the subject of working at heights.

Pic2 - Assembly of Tower Crane

When assembling the crane sections, the following must be checked:


• the positioning of the lifting equipment and its capacity
• the positioning of the workers involved in the activity.
When assembling the crane, workers must place their feet at points that prevent slipping, particularly in areas where
crossbars meet.
In this situation, workers are within the perimeter of the section to be placed, without being properly safeguarded against
any unforeseen movement of the suspended load and without being properly anchored.
The Contractor must carry out an awareness-raising session for all Project workers on the subject of working at heights.

Every ladder:
a) shall be securely fixed so that it cannot move from its top and bottom points of rest; or
b) if it cannot be secured at the top, should be securely fastened at the base; or
c) if fastening at the base is also impracticable, should have a man stationed at the foot to prevent slipping.
Regardless of the above comments, the Employer's Representative warns and reminds that the ladders shall not be
used as a working platform, but rather as a means of access between different levels.
The Contractor is requested to carry out a training/awareness-raising session for all workers involved in this activity.

Every ladder:

Pic5 and 6 - Tower Crane assembly

The contractor began assembling the tower crane without the safety conditions being met:
• No Critical Lifting Plan was drawn up.
• Delimitation of the radius of action area was scarce and with little distance.
• There was work on the old jetty and the passage zone for these workers was not defined, so they had to pass
within the crane's radius of action.
This activity led to the opening of non-conformity no. 1.002/24 H&S.

Pic7 and 8 - Facilities Constructions

The Contractor is requested to ensure that activities which are expected to affect the site's access routes are properly
planned and shared.
The alternative route signposting must be easily visible to all those involved, thus avoiding constraints or any type of
incident.

Pic9 and 10 - Scaffold (Canteen)

All work platforms must be fitted with footboards that completely fill the entire available space.
Climbing the scaffolding must be done from the inside of the scaffolding.
Scaffolding must be labelled accordingly. No scaffolding must be used without first having been checked and labelled.
