purpose of the study is to know the extent of gadget usage in modular distance learning
and parental support and academic performance In this regard, we would like to ask
your permission to allow your child to be one of our respondents in the study. Please be

The survey will last about 15 to 20 minutes. All data that will be obtained shall held
confidential and anonymous. This survey is voluntary and will not subject your child to
any physical or emotional harm, and that you and your child has the right to refuse or
