TRANSPORT (2- Mobilization & Demobilization

WAY) 1 From Monark Yard to E & H CONSTRUCTION PROJECT – and back For the account of the LESSEE

NOTE:
1) AVAILABILITY: (Unit/s are subject to first come first served basis)
2) MINIMUM RENTAL: One (1) month rental or 200 hours, whichever comes first.
3) OPERATOR’s LODGING & TRANSPORTATION: Lessee shall provide the operator of the unit adequate lodging (Including light and
water) & Transportation for FREE at all times during the rental period. In any case that operator/s are required by the Local
Government or Barangay where the project is located to undergo several days of home/barracks quarantine, the Lessee will
shoulder the PhP 900.00/day standby rate of the operator/s. ‘’
4) PAYMENT TERMS: Required Purchase Order (PO) and Conformity to this proposal and 100% advance payment equivalent to
rental rate plus mobilization/demobilization charges prior to the delivery of the unit. (Advance payment shall also apply to

5) MOBILIZATION and DEMOBILIZATION Charge is for the Account of the Lessee. In the case where the Lessee assumes responsibility for the
physicalmob/demob of rental unit/s, a deposit equivalent to 100% of demob cost shall be required from the Lessee. Deposit will be refunded upon return of
unit
to Monark yard.
6) CONSUMABLES i.e. FUEL, OIL (for Preventive Maintenance), Ground engaging tools (G.E.T’s) tips & cutting edges Bit/trencher teeth (for trenchers):
Shall be provided by the LESSEE and is for Lessee’s account at all times. For equipment with tires, maintenance i.e.
vulcanizing
is for the account of the Lessee.
7) FOR NEAR SEA APPLICATION: LESSEE to provide daily fresh water supply, pressure washer, etc. for cleaning to avoid corrosion of
earthmoving units otherwise damages (i.e. corroded body of rental unit) shall be for the account of the LESSEE.

8) In case of Port-to-Port and/or inter-island delivery: Units will be covered by insurance while vessel is sailing provided that the LESSEE will provide the
necessary pertinent information/documents (Name of Barge, Shipping Line, Type of Barge, and Schedule of Shipment) for insurance coverage prior to
delivery schedule.
9) 3rd Party Certifications, Permits, Taxes, and Licenses etc.: All conceivable taxes, licenses, permits, fees, etc. imposed by the project, the government,
national or local, or by any of its departments, agencies or bureaus, in connection with the use, possession or operation of the UNIT, shall,
during its possession thereof, be for the Lessee’s sole and exclusive account.
10) Tires shall have an expected life as follows:
 Wheel Loader – 3,000 machine hours
 Motor grader – 2,200 machine hours
 Vibro. Soil Compactor – 3,000 machine hours
 ** Replacement cost due to premature failure shall be charged at cost to Lessee on a pro-rata basis.

11) Similar provision (in #10) applies to units with undercarriage; expected life of undercarriage is at 5000 hours.
12) All prices are already 12% VAT Inclusive.
13) PRICE VALIDITY: Thirty (30) days from the date of this quotation.
