    Dear Mediassist team,

    I am very grateful that you had processed my Mediclaim related to my mothers’ epilepsy
    and fracture related treatment.

    In continuation to that claim, my mother had follow-up treatments related to fracture


    and epilepsy in Ahmedabad wherein we had consulted Dr. Keyur Machhavada for
    epilepsy related treatment. Further we had consulted Dr. Mukesh Shah and Dr. Kalpesh
    Mehta for fracture related treatment.

    Dr. Mukesh Shah had recommended for operation, so we took a second opinion with Dr.
    Kalpesh Mehta. He recommended an alternative treatment which did not required any
    operation. So, we continued with Dr. Kalpesh Mehta.

    I have put claims related to follow up treatments which were recommended in discharge
    summary of Cauvery hospital. You can please look into it. The original document is with
    your team only.
