A Smart Way to Pay Your Utility Bill
There is no way around it. You have to pay your utility bill each month.
Why not make it easy on yourself and let us direct debit your checking account?
It saves you time and money; and it’s easy!
Your bill will be mailed to you just as before, but it will include a comment line,
“DRAFT - DO NOT PAY.” We will debit your checking account 15 days after the
bill date. You have until the due date to review your utility bill and contact us if
you feel the charges are incorrect before we debit your account.

To sign up, just sign the authorization, complete the form, attach a voided check
or deposit slip and return it to us along with a check for the current month’s pay-
ment. You will be enrolled by the next billing cycle and you will never pay a late
penalty again.

AUTHORIZATION FOR ELECTRONIC FUND TRANSFER

I authorize Peru Utilities to debit my checking account and initiate adjustments for any trans-
actions credited or debited in error if necessary. This authority will remain in effect until Peru
Utilities is notified by me in writing to cancel it. Sufficient time will be allowed to afford Peru
Utilities and my bank a reasonable opportunity to act upon it.

If my account is debited and has insufficient funds or has been closed, I understand that I will
be charged an additional $25 and will be ineligible for direct debit for 12 months.
