1 Introduction – Scope of Works


This Risk Assessment & Method Statement (RAMS), describes the works and associated risks, whilst carrying
out cofferdam construction works for the future Clarifier Area.
The scope of works outlined in this Risk Assessment & Method Statement, are as follows:
• Works area/ storage preparation;
• Delivery, offloading & stacking of sheet piles;
• Installation & removal of sheet piles (Subcontractor RAMS);
• Excavation and dewatering;
• Pipework install – 200mm Sludge Outlet and 200mm Inlet from Splitter Chamber.

3 Pre-commencement Requirements
3.1 General Pre-commencement Requirement
1. All management and operatives will attend a site induction, and will have completed the relevant training
programmes;
2. All operatives will be briefed on the contents of this risk assessment & method statement (RAMS) for the
works to be undertaken, and they will sign to confirm they fully understand its contents
3. Any permits required for the works will be issued, briefed, signed onto by the work team and strictly
adhered to during the works;
4. A Point of Work Risk Assessment (POWRA) shall be carried out at the place of work, immediately prior
to commencement of the works, with the operatives at the start of each shift, and they will all sign to
confirm they fully understand its contents;
5. Any additional hazards raised, will be noted on the Point of Work Risk Assessment, and control measures
put in place, prior to the works commencing;
6. If the conditions change significantly at the place of work, then the Point of Work Risk Assessment will
either be revised, or a new Point of Work Risk Assessment will be carried out;
7. A copy of the RAMS and a daily Point of Work Risk Assessment, will be retained at the place of work for
the following reasons;
• For reference by the work team;
• Briefing additional man power;
• Audit purposes.
8. Upon completion of the shift, the relevant daily Point of Work Risk Assessment and any permits will be
closed out, and will be passed to the Murphy Safety Adviser for filing;

9. All necessary consents will be applied for by the engineer, and once received the works shall commence;
10. This document will make reference to other RAMS if applicable, before the commencement of the works;
11. Documents to be issued prior to the commencement of the works, include but are not limited to:
a. AF1 / AF2;
b. All associated internal Murphy Permits for working (SHESQ);
c. Rescue Plan (SHESQ);
d. Emergency Procedures (SHESQ)

5 Methodology
5.1 General
1. All works will be carried out in accordance with the contents, of this method statement & risk assessment;
2. The works as detailed in this method statement, will be carried out under the supervision of the Murphy
construction manager / site foreman / site engineer;
3. Works area’s will be established and demarcated as deemed necessary by the MIL site management
team. Suitable timber skids will be cut as required.

5.2 Delivery and Offloading of Sheet piles.


Murphy will assess the weather conditions prior to Lifting Operations commencing.
1. Murphy Plant will supply the sheet piles to be used for the construction of the cofferdam around the new
Clarifier area.

2. Delivery lorries will arrive at the entrance to the wastewater treatment works and be directed to the
laydown areas as indicated in the sketch – Piling Works Layout Plan - included in the next Page.

3. Delivery trucks will be positioned at an off loading location, on the site as deemed necessary by the site
foreman. Trucks to be directed by a competent person;

4. A 60 t CRANE from ECCH will have been taken on to site and positioned prior to arrival. The crane will
be directed to the required position as indicated in the layout provided and be positioned on suitable hard
standing, and set up as the manufacturer’s recommendations and the lift plan requirements.

5. An exclusion zone will be implemented and can be maintained by a spotter during lifting operations. A lift
plan will be put in place to cover all lifting operations. A POWRA and a specific toolbox on Lift Operations
will be completed prior to this activity.

6. Prior notice will be given to the Operations team and no site activities are scheduled for this date.

7. During this activity, there will limited access to the Site and a dedicated observer on Site.

8. All straps will be removed from the load, each individual bundle will be slung as deemed suitable by the
slinger/signaller. Tag lines will be utilised as required to control the load when it is suspended in the air;

9. The load will be pinched to ensure the load is suitably balanced. The load will then be lifted and slued into
the required position and lowered to the ground;

10. The load will be positioned on timber skids and lifting equipment removed;

11. The steps detailed above will be repeated until all bundles have been off loaded;

12. Once all the bundles have been removed, the lorry will be turned and directed off site;

13. A R Duck will be used to move the sheet piles around site as required. Sheet piles will be separated using
an excavator/fork attachment. Once the bundles have been split, they will be suitably slung as required
by the slinger/signaller. Smaller bundles or single sheet piles, will be moved around site as required by
the R Duck and positioned on timber skids.

5.3 Cofferdam Construction


Ward Piling will be used to install & remove the sheet piles to the required depths. Subcontractor Method
Statement is included in Appendix D. The TWDC is also included in Appendix D In Revision P01 of this document,
we include the Preliminary Temporary Works Design provided by Ward Pilling.

1. Refer to Ward Piling Temporary Works Design for Pile type and design depth and orientation.
2. A specialist dewatering contractor will commence drilling once sheet piles are completed. 2 no. 250
mm wells will be drilled to a depth of 6.5 meters below ground level, lined with 150mm Boode Ecoline
PVC Casing and Screen (Screen to invert level of IL 37228) filled with 3-5mm washed pea gravel
surround.
3. An external generator and control panel will be provided to supply pumping requirements.
Please refer to sketches below for proposed pump arrangement and for outfall setup. Pumps sizes to
be confirmed once wells are completed and dewatering contractor will insert test pumps to confirm
flows. All flushing and testing of wells will be pumped back in within the perimeters of the cofferdam.
Water Sample will be taken to check turbidity levels of water.
4. Altrad to provide access point and handrails for edge protection – this detail will be forwarded for
review on the next revision of this RAMS and included in Appendix E.
5. Refer to Emergency Plan for the Cofferdam rescue procedure.
6. Excavation works (Phase 1) to commence with 16t R Duck. Excavation materials loaded directly onto
trucks. Phase 1 of excavation will achieve an invert level of I.L. 38095.
7. At this point, ground conditions will be assessed and once these have been determined to be
suitable, a 3t mini digger will be used to excavate to the cone area in the centre – I.L. 36928.
8. An oxygen monitor will be hung next to the excavator to measure the content of oxygen whilst
excavating within the cofferdam. The operator is required to work for 30 minutes then switch off the
machine for a period of 10 minutes to allow the area to fully naturally ventilate. Works may resume
for another 30 minutes followed by a 10 minute rets period. This is to be repeated for the entire
excavation time within the tank. If the oxygen monitor alarm sounds the operator is to leave the tank
immediately via the provided access stairs switching off the machinery in the process. Top man to be
present for the duration of the machine working period to raise the alarm if an emergency situation
arises.
9. A layer of blinding will be poured using a 24m concrete boom pump to form the base of precast
manhole diameter – internal measurement > 2.5 meters to facilitate Shay Murtagh cone section. Hi-
ab to be used to insert manhole into position.
10. 200 mm sludge outlet pipework and 200 mm inlet pipework from Splitter chamber will be installed, an
air test will be completed to conform integrity of the pipelines. A dumpy / laser level will be used to
ensure the correct level is obtained. Pipework will be set out by the site engineer in line with the

design drawings. Levels and falls will be marked on pins on site to provide a reference guide for the
setting out of the base.
11. Both lines to be backfilled with approved materials. Refer to Pipework RAMS. Both pipes to stop 200
mm from sheet piles and finished with blank plates and put a void fill at pipe end.
12. Area to be prepared for Shay Murtagh’s permanent base. Concrete blinding will be pumped using a
concrete pump and levelled off using a grade laser with levels set to suit the underside of the new
clarifier concrete base.
13. A flexible 25mm material to be installed on sheet piles prior to concrete base pour.
14. Backfilling of the clarifier will be carried out by a 1.5t mini excavator and 900mm compact roller.
Access will be provided by overfilling predetermined point between the sheet piles and the tank to
enable pedestrian/walking access on the southerly site and plant access on the Northern side.
Further loads of SR21 T0 material will be placed with the rubber duck where required and spread
with the excavator. An operative will remain top side to reverse in delivery trucks and bank the rubber
duck. Operatives will wear an 02 Monitor, should the alarm sound, all personnel must evacuate the
area. Machinery will be required to shut down every 30 minutes to allow the area to naturally
ventilate.
Once spread to a depth of 150mm each layer is compacted/rolled.

6 Programme
These works are due to commence on the 01st November 2023, and will continue from thereon, as detailed in the
overall construction programme for the project.

7 Quality
All works will be carried out in accordance with Company Quality Procedures and the Employers Requirements.

8 Safety
All works will comply with the Safety Health & Welfare at Work Act 2005, and all other relevant safety legislation,
and in accordance with the Construction Stage Health & Safety Plan.
On a daily basis, the site foreman or a designated person, shall complete a Safe Plan of Action / Point of Work
Risk Assessment. This SPA / POW shall be briefed to all under their control.
Safety shall be promoted by induction and training. All site personnel including sub-contractors, will hold a Safe
Pass Card and Manual Handling, or their entry to site will be forbidden.
All works shall be carried out in a safe and satisfactory manner, using approved certified plant and equipment.
A qualified First Aider shall be present on site during the construction works.
All personnel will be required to wear PPE for the particular works which they undertake, and as a minimum Hard
Hats, Safety Gloves, Safety Glasses, Hi-Visible Jackets or Vests and Steel Toe Cap Boots will be worn.
Notices of the works will be issued to adjoining occupiers, informing them of the nature and timing of the works
outside their properties.
The site entrance, access road, works areas, signs, cones and barriers will be maintained in a clean and tidy
condition.

9 Environmental
Works shall be carried out strictly in accordance with the Construction Environmental Management Plan (CEMP).
Murphy Group will ensure that relevant permits, licenses’ or consents are in place, and their requirements briefed,
to those carrying out the activity before works commence.
Murphy supervision will monitor the noise, dust, vibration, & water associated with these activities. Should any
significant issues be observed, they will contact the Construction Manager and agree measures to be taken to
address them.
Daily visual checks of all site plant and vehicles shall be made, for leakage of fuel or oils. Leaks shall be rectified
immediately, or the item removed from site and replaced.
Fuels, oils, greases and hydraulic fluids shall be stored in bunded containers.
25 litre emergency spill kits shall be available at all times. All spills will be dealt with promptly, and reported to
Murphy site management.
Any waste generated, shall be managed in accordance with the Site Waste Management Plan.
All environmental incidents and non-conformances, shall be dealt with and reported in accordance with the
Construction Environmental Management Plan and the Murphy Procedure – Accidents and Incidents.

CAHIR COFFERDAM SCOPE AND DESIGN NOTES:

Design of cofferdam temporary sheet piling system to facilitate the installation of precast Shay
Murtagh concrete structure at Cahir WWTW.

Base of primary dig proposed at 3.50m below GL, with a localised sloping of dig within cofferdam
for installation of central sump.

Total excavation depth of 3.5m

From IGSL Borehole Log ref: 23137D, the subsoil are reasonable similar in characteristics and
vary in clay make up from silty/gravelly/sandy type clays. With 'N' values ranging from 21-50,
these clays are stiff.

It is proposed to install min 6.6m sheet piles with a 3.1m toe in and cantilevered only.

A 75mm blinding layer to be installed as capping to exposed ground immediately upon reaching
dig level.

1. This design is based on a “Pre-Driven” method of installation. The PU22 sheets are to be driven to full depth prior
to excavating to include a “toe-in” of 1.3m as shown in design. The sheet piles specified have been selected as a
suitable size and type to suit structural requirements. Further assessment of the soil strata may be required in
order to ascertain the drivability. It may be necessary to use a combination of hammer types and heavier piles to
achieve the full embedment depth as specified in this design. Please note that piling operations using impact or
vibratory hammers can generate large amounts of both noise and ground induced vibrations. It is essential that a
detailed risk assessment is carried out by the contractor in order to assess the effects of piling operations on site
personnel, the general public and adjacent structures / services (see also the residual risk assessment sheet).

2. The temporary works installation has been modelled providing adequate stability at each intermediate installation
stage. No beneficial soil phenomena have been considered to achieve stability during the installation process such
as removal of passive softening of the clay or any form of adhesion/interface friction coefficient.
4. This design includes allowance for the following surcharges. (Note: If the surcharge is not listed below it has not
been considered in this design and should therefore not be within the zone of influence):
a) A uniform surcharge of 10kN/m² at ground level to allow for general site traffic and excavators up to 30Tn
weight working near the excavation.
5. In the interests of design economy, overdig has not been allowed for in the design calculations. The contractor
must therefore ensure that the excavation process is controlled accordingly

6. As soon as the formation level is exposed, it is recommended that blinding concrete should be placed immediately
to prevent degradation of the base. It is recommended that measures are taken to prevent possible uplift due
to groundwater pressure.
7. The contractor should use a suitable “Bond break” medium such as visqueen, between the concrete and sheeting
to aid extraction of equipment.

8. It is the contractor’s responsibility to thoroughly check this temporary works design


submission to ensure that the design brief has been interpreted in accordance with the
contractor’s requirements.

9. It is the contractor’s responsibility to communicate the information contained within this


design to all relevant parties including the site principal designer(s) and temporary works
coordinators where appropriate. It is also the contractor’s responsibility to ensure that this
information is incorporated into site specific method statements and risk assessments.

10. Any significant residual risks remaining after the design process will be documented both on
the relevant page of this design and also on the scheme drawings. It is the contractor’s
responsibility to take steps to reduce these risks to an acceptable level.

11. A fee will be charged for any subsequent revisions to this full design that will require extensive
re-design work.

allowed prior to further excavation and 21 days prior to fully loading the waler by completing the excavation. The
excavation must not proceed beyond this level until the waler has reached 35N on a cube test result.

Residual Risk Schedule

The followings items have been identified as potential residual risks remaining after the design process risk
assessment. Based on the information that we have available, we have allocated each risk a rating number
depending on its potential to cause a problem. Those with a rating of 2, 3 and 4 are highlighted on the scheme
drawing. A further site specific risk assessment must be carried out on these items to assess their importance
and potential consequence and to determine a course of action or monitoring in order to mitigate the risk to
an acceptable level.

Notes on possible further mitigation action required at site level.

1. Overdig: In the interests of design economy, overdig has not been allowed for in the design calculations. Specific control
measures should be put in place to ensure that the dig is not allowed to progress beyond the indicated formation level unless
the designer is consulted. Alternatively the design can be redesigned to allow for an overdig allowance.
2. Surcharge: A uniform surcharge of 10kN/m² at ground level to allow for general site traffic and excavators up to 30Tn
weight working near the excavation. It is recommended any heavy plant is set back at least 1.0m from the excavation to
avoid excessive localised surcharges. If the plant exceeds the weight limit of 30Tn, or the total surcharge loading exceeds
the values stated on the design, the scheme needs to be reassessed. All spoil and excavated material should be moved to
outside the zone of influence to avoid additional surcharging of the excavation.
3. Lack of toe-in: Sheet toe in is critical to this design. A monitoring system should be put in place to verify that this has been
achieved. If for any reason the toe-in specified has not been achieved on a significant number of sheets or piles, the
designer must be consulted before the excavation is declared safe to enter.
4. Groundwater ingress and/or flooding: Significant groundwater ingress or flooding is possible in this excavation. The
contractor should include a means of dealing with this within his safety method statement.
5. Loss of fines: Contractor is to monitor the excavation for loss of fines due to the presence of granular material in the earth
strata and a high water table.
6. Unable to fully pre-drive sheets: The design and installation sequence has been prepared on the basis of the sheets being
pre-driven to depth prior to excavating. If this proves difficult to achieve in practice, the installation sequence will need
to be reassessed so as to maintain stability of the excavation at all times.
7. Vibration: This scheme requires that the sheets or piles are pre-driven using vibratory / impact hammers which will impart
vibrations into the ground. The effect of these vibrations needs to be assessed in relation to potential damage and/or
settlement of adjacent structures. PPV data is available for various hammer types to assist in making this assessment.
8. Hard nature of the ground: Contractor to monitor sheet installation due to the hard nature of the ground. Pre Auger
maybe require to help with installation.
9. Weight of equipment: Contractor to ensure that suitable lifting equipment is on site

All plant and materials are to be delivered into the site (and later removed) by
road transport, following the prescribed route in keeping with site access and
highway traffic constraints and under traffic marshal / vehicle banksman
instruction.

Access and egress to and from the site will be strictly in accordance with site
policies issued by Murphys. Deliveries by road will be as directed in accordance
with site requirements.
T.M. will be the responsibility of murphys

All delivery vehicles entering, travelling in and exiting the site will have flashing
amber beacons.

Plant and transport travelling through the works will have a vehicle banksman
in attendance at all times.

Plant travelling into the site will have booms or masts folded away from existing
structures, site traffic / personnel and highways at all times.

The Movax/Excavator will be delivered by low-loader road transport in folded


and transport mode. The excavator shall be off-loaded in accordance with the
manufacturer’s operating instructions and under supervision of competent
vehicle banksmen at the works compound.

Road protection measures if required are to be provided by Murphys.

All plant and stores will be located such that no accidental spillage of hydraulic
or lubricating oils or fuel can contaminate the site. In the event of accidental
spills, a spill kit (absorbent pads and granules) will be kept at point of work.
SDS to be present & driptrays or bunds created to contain a spill

Demobilisation will essentially be the reverse of this.

(c) Handle, pitch and drive sheet piles

Work will be carried out within the exclusion zone as per attached layout
drawing _ Appendix A
Handling, pitching, driving and extracting (where required) of steel sheet piles
will be carried out using the Movax SG 75V vibratory hammer. The pile is picked
up by the Movax excavator and handled to the vertical position. The pile is
aligned and guided into position by the guidance of the banksman.
The pile pushed into the ground (or clutched in for subsequent piles) in its
correct attitude and alignment and driven a short way down.

The verticality is manually checked by the piling operative with a level and the
driving completed to level or refusal.

(d) Early refusal & obstructions:

Piles will be driven by the vibratory hammer until specified toe level or pile
refusal is reached. Levels will be monitored for record purposes.
I pile toe depths aren’t achieved after all piles are installed with the Movax
SG75V vibro hammer, the DH25 impact hammer will be fitted to the 35T
excavator and the piled impacted to the required depth.
Appendix C

(e) Pre- Extraction preparation Work


Prior to the removal/extraction of the sheet piles, the M/C ( Murphys )
will backfill the cofferdam

(f) Extracting of sheet piles:


We propose to use the 35T excavator and Movax to extract the sheet
piles and stockpiling
The method of extraction is precisely the reverse of the installation
without the need for checking for verticality.
The Movax shall vibrate the pile out of the ground until the pile is de-
clutched from the adjacent pile.
The Movax operator lowers the pile to the horizontal position and stacks
it in a bundle for reuse/stockpiling. Piles shall be stacked in bundles of 4
– 5T for loading onto artic trucks in the adjacent adjacent hardcore
working area.
Access to the stockpile/working areas will be the responsibility of the
M/C
The 35T & Movax shall travel around the perimeter of the cofferdam, for
extracting the piles. Clear access to be provided for this machine.

(g) Noise & vibration

All personell involved in the piling works shall be required to wear ear
protection at 85dB

If any noise or vibration monitoring is required, this will be carried out by


Murphys.

(h) Emergency procedures

Emergency procedures will be as per the Murphys’s Health and Safety Plan.

(b) All personnel will carry all necessary competence cards as required
for the work they are to undertake. SOLOS/CSR / CSCS / CPCS
All personnel will be Safe Pass or CSR qualified to enable them to work in
Republic of Ireland.

All personnel will have Manual Handling training.

All personnel will complete the on-line site induction prior to starting
work.

All personnel will carry and wear all appropriate PPE as required by the
site and appropriate to the work being carried out.
