2024 FUEL INDICATION ACCURACY
The reporter raised a concern of the accuracy of the totalizer fuel indication when the amount dropped by 0.4 tonnes during 10
minutes period. There were similar techlog entries in previous sectors of the same issue.
Flight Safety update: Engineering performed a FQPU test which was satisfactory.

2024 GO-AROUND - INADEQUATE SEPARATION
During ILS approach to runway 16, ATC instructed to go-around due to inadequate separation with the preceding aircraft.
Flight safety update: The QAR data indicate that a standard go-around was flown from 1,700 feet AAL.

BIRDSTRIKE - NO DAMAGE
Passing approximately 300ft on the approach one small bird hit the lower right hand corner of CM2's number 1 window.
Flight Safety Update: an engineering check completed and no damage found.

BLOCKED TOILET SYSTEM – HYGIENE CONCERNS
Three toilets were inoperative due to a blockage in the drain system and the intention was to return to DXB for rectification.
However, the toilets were contaminated with waste, producing a strong sewage smell that would have consequences related to
hygiene and passenger and crew comfort during the flight. MCC suggested to drain the toilets manually but the VIE cleaning staff
refused. As the cabin was nearly full and an ATC slot was creating pressure to avoid a delay, a solution was reached whereby the
engineer used blankets and tape to, as best as possible, hermetically seal the toilets closed. The toilets were monitored closely
during the flight.
Flight Safety update: A difficult situation handled with resilience by the crew, especially the engineer. Taking ownership of this
problem when others looked the other way prevented potential brand damage and preserved hygiene standards during the meal
service.

CABIN LOG ENTRY NOT SIGNED OFF
After landing in Dubai , the engineer advised that a cabin log entry was not signed off by the outstation engineer.
Flight Safety update: Before departure, the flight crew signed the last page of the cabin log but missed an item on the previous
page.

GROUND HANDLING SERVICE STANDARD - LOADPLAN NOT ADHERED TO
After departure, the crew received an ACARS message there was a ULD loaded in a different position than what was in the load
plan leading to a difference of MACZFW 32.9 vs 33.0 and MACTOW 34.1 vs 34.2.
Flight Safety update: The ramp team faced difficulties in loading a ULD at position 36L due to ULD base damage. The Turnaround
Coordinator decided to move the ULD without advising the CLC Coordinator before departure. Both the CLC Coordinator and
Turnaround Coordinated have been debriefed. The Turnaround Coordinator has been removed form EK operation until the
refresher training has been completed.

OFP DARD STRATEGY INFORMATION
The briefing package for this flight included a list of allocated airports for enroute diversion strategies, with two airports that raised
concerns (ORER&OIIE). According to company NOTAMs, diversions into ORER and other airports in Iraq were only allowed during
the daytime, while this flight was scheduled to operate at night. Additionally, NOTAMs for OIIE stated that Emirates aircraft should
not consider refuelling at this airport. Hence, the crew questioned the availability and suitability of these airports for dispatch.
Flight Safety update: FOS confirms that both airports are available for emergency use 24/7, and the restrictions are only for
scheduled operations. A new Co-Notam will be updated to reflect this information.

June 2023 B777 DXB - VIE Approach


UNSTABLE APPROACH - FLAPS - CONTINUED
During the ILS approach to runway 34, ATC issued a go-around from 900 feet AAL due to a blocked runway. The flight crew
requested immediate vectors for an ILS approach to runway 29 as the flight had committed to destination. During the second
approach to runway 29, a late approach configuration resulted in the landing flaps not being set by 1,000 feet AAL.
Flight Safety update: The QAR data indicate that a standard go-around was flown from 880 feet AAL. On the second approach to
runway 29, landing flaps 30 were selected at 1,131 feet AAL and set at 883 feet AAL.

May 2023 B777 VIE - DXB Parked pre flight


AIRCRAFT OVERFUELLED - 1.7 TONNES
While parked at the bay, fuel figures were communicated with the engineer. During refueling, the crew was completing the briefing
for departure and noticed the fuel was still going up while already reaching the requested final fuel. The amount of over fueling
was 1.7 tonnes.
Flight Safety update: Manager Regional Engineering investigation concluded the below:-
- The engineer, who was handling the flight was under the refueling station and supervised the refueling process.
- Before reaching the final required fuel amount the gauges started to fluctuate up and down causing the refueling valve not to
close in according to the selected amount of fuel.
- Immediately he stopped the refueling, and after that, he notices that was 1.7 tonne of additional.

2022 ATC CLEARED SID CHART NOT FOUND IN LIDO - ALPHABETICAL ORDER
The flight was cleared to depart via SID ASPIB 2C RUNWAY 29. The flight crew could not find the chart in the LIDO mPilot APP. ATC
was informed and re-cleared the flight on a different runway and SID.
Flight Safety update: The SID is named as NOISE ABATEMENT ASPIB2C RNAV (ATC) and it is not listed under the letter A in the SID
menu alphabetical order. It is necessary to scrow down the SID menu list to find the chart as it is listed with the noise abatement
SIDs.

October B777 VIE - DXB Taxi-out


2022 LATE TO INITIATE A TURN - REVISED TAXI CLERANCE
During taxi out to runway 34 for departure, the PF was late to initiate the turn when cleared to line and wait runway 34. A revised
taxi clearance was received and the runway was entered via A2.
Flight Safety update: There was no conflict with other traffic during event. A picture of FR24 is attached in the workflow.

During cruise in Ankara airspace, ATC responded and provided clearances to the incorrect flights. Emirates flights challenged ATC in
all the instances.
Flight Safety update: The report has been forwarded to the callsign similarity working group for information. Callsigns will be
monitored and listed for review and possible change.

LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in the Ankara and Istanbul FIRs. A time shift occurred. Approaching position EVGEG, EGPWS
TERRAIN PULL UP activated for two seconds at FL340. The GPS position showed the location over Crimea.
Flight Safety update: The QAR data indicates that in the Ankara FIR the GPS position instantaneously shifted 180NM to a position
over the Crimean Peninsular at low or negative GPS altitudes, and periods of being accurate. About two minutes later, ANP spiked
to 20NM and the aircraft rolled to the left to a maximum of six degrees. 34 seconds later, EGPWS PULL UP activated for five
seconds at FL340. 43 minutes later, the GPS position and altitude were accurate and ANP was 0.03 consistently. External electronic
jamming is the most probable cause of the reported GPS signal loss. FCN 2023-050 GPS Signal Interference has been published to

2023 LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING
GPS jamming was experienced in the Tehran, Ankara and Sofia FIRs. Approaching position EVGEG the clock shifted almost five
minutes (from 1544z to 1549z). After 37 minutes, near to position BALBI, the GPS jamming ceased and the clock returned to the
correct time.
Flight Safety update: External electronic jamming is the most probable cause of the reported GPS signal loss. FCN 2023-050 GPS

LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in the Sofia FIR and a time shift was evident. EICAS TERRAIN POS displayed. EGPWS TERRAIN PULL
UP PULL UP activated at FL340 and was assessed to be spurious.
Flight Safety update: The QAR data indicate that the GPS position jumped by 193NM over three seconds and five seconds later the
ANP peaked to 20NM. The GPS position moved to be over Sevastopol at a negative GPS altitude. The aircraft rolled to the left to a
maximum of five degrees, as ANP continued to spike to 20NM several times. 36 seconds after the map shift, EGPWS PULL UP
activated for five seconds at FL340, with the aircraft rolling to the left again, to a maximum of seven degrees. Several more spikes of
large ANP and small changes in roll and heading occurred over the next two minutes. Then the GPS altitude recovered to accurate
values and the GPS position drifted back to the actual position over the following 20 minutes. External electronic jamming is the
most probable cause of the reported GPS signal loss. FCN 2023-050 GPS Signal Interference has been published to provide generic

November A380 DXB - AMS Cruise


2023 LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in the Ankara FIR at position NINVA at FL380. EGPWS TERRAIN AHEAD, PULL UP activated.
Inconsistent terrain information was displayed on ND. The warning was cancelled as it was assessed to be spurious. The following
ECAM messages were displayed: GPWS 1+2 FAULT, TCAS 1+2 FAULT, SURV PRED W/S 1+2 FAULT, PWS 1+2 FAULT, GPS 1+2 FAULT
and GLS 1+2 FAULT.
Flight Safety update: The QAR data indicate that EGPWS TERRAIN, TERRAIN, PULL UP activated at FL380, for 69 seconds. The GPS
position was erroneous, showing the aircraft stationary over Syria at 1,200 feet for the period of the warning. External electronic
jamming is the most probable cause of the reported GPS signal loss. FCN 2023-050 GPS Signal Interference has been published to
provide generic information about GPS signal interference, its effects and Emirates’ experience and countermeasures. For aircraft
related information, refer to the respective type specific documentation. As indicated previously by AIB, in these events, the AESS
considers that GPS altitude to be valid, which leads to a spurious monitoring of the radio altimeters, which it then rejects as invalid,
as they do not match the GPS altitude. This results in the rejection of all three radio altitude readings by the AESS. Finally, as the
TCAS, GPWS and PWS functions of AESS use radio altitude input and these are rejected, then these three functions are lost, as
reported.

LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in the Ankara and Sofia FIRs. Various unspecified EICAS messages were displayed. RAD NAV INHIBIT
was switched OFF. A COMM message was received stating that a time shift had occurred. The clock was confirmed to be not normal
with approximately five minutes of the correct time. ANP increased to 20NM and then back to a normal value for few seconds on
multiple occasions. EGPWS TERAIN, PULL UP activated at FL340 for two seconds in the Ankara FIR, over the Black Sea and
considered to be spurious. The QRH GPS Signal Interference was referenced and MMRs needed to be reset. Shortly after the GPS
functionality recovered.
Flight Safety update: The QAR data indicate that EGPWS PULL UP activated twice, once for two seconds, and then 39 seconds later
for another two seconds. The GPS position and altitude was erroneous and showed the aircraft stationary over Sevastopol at a
negative GPS altitude. External electronic jamming is the most probable cause of the reported GPS signal loss. FCN 2023-050 GPS

B777 DXB - AMS Cruise


LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in the Ankara FIR. At FL340 after crossing position NEGEM into the Sofia FIR, a COMM message
displayed regarding a change in aircraft clock time by 5 minutes. A few moments later, EGPWS TERRAIN warning and PULL UP
activated for few seconds. Autopilot and Auto thrust remained engaged and no action was taken. All aircraft systems returned to
normal including the clock after exiting the GPS signal interference area.
Flight Safety update: The QAR data indicate that EGPWS PULL UP activated for five seconds at FL340 over the Black Sea on the
border of the Ankara and Sofia FIRs. The GPS position and altitude was incorrect, showing the aircraft stationary over Sevastopol
with a negative GPS altitude. A time change was not recorded. External electronic jamming is the most probable cause of the
reported GPS signal loss. FCN 2023-050 GPS Signal Interference has been published to provide generic information about GPS

2023 ATC SERVICE STANDARD - CALLSIGN CONFUSION BY ATC
While in the Baghdad FIR on 123.0 MHz, ATC called the wrong callsign UAE87 six times before calling UAE84N. The controller
apologised after realising the error.
Flight Safety update: The confusion might have been due to UAE87 in close proximity to UAE84N.

EICAS AUTOTHROTTLE DISCONNECT
Approaching top of descent, an EICAS AUTOTHROTTLE DISC activated together with an EICAS THRUST ASYM COMP. At
approximately 5,000 feet QNH, the AUTOTHROTTLE DISC self-cleared and operated normally for the remainder of the flight. A
technical log entry was made.
Flight Safety update: Engineering confirmed that the aircraft lost the thrust asymmetry compensation (TAC) function. Hence, the
autothrottle disconnected. As a precaution, engineering applied MEL 27-02-08. The MEL was cleared after 12 sectors on the
8/10/23 as the fault did not reoccur.

September B777 DXB - AMS Cruise


2023 MEDICAL-CABIN CREW ILLNESS-STOOD DOWN FROM DUTY-GROUND MEDICAL SUPPORT CONTACTED
During cruise a cabin crew member fell ill. Ground Medical Support were contacted and as per their advice medication was
administered. The cabin crew member was stood down from duty.

WEIGHT AND BALANCE
During cruise, new load sheet version was received by acars stating 14pcs/98kg of cargo were found in the ramp after departure.
Flight Safety update: Zafire misuse during loading reconciliation (not following SOP). EKAS Safety have highlighted this concern and
have issued a Safety Alert to the turnaround team.

August B777 DXB - AMS Cruise


2023 LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING
GPS jamming was experienced in the Ankara FIR earlier in the flight, which resolved itself. Upon arrival in AMS on the ILS approach
to runway 18C, when established on the localiser at 2,500 feet, EICAS NAV UNABLE RNP displayed and the navigation source
reverted to inertial. The FMS POS revealed that the GPS R position was missing. RAD NAV INHIBIT was selected to OFF.
Flight Safety update:
The PLF shows no active faults at the time of approach and landing, and only inactive faults, from the earlier jamming. The level of
ADS-B coverage around AMS, which was recorded as between 0-2% on the date of the report. The event on approach may have
been a latent effect from the earlier jamming, for which an MMR reset in cruise may have prevented. But it is also possible that
there was some transient jamming from a handheld device that affected the aircraft on approach. Since January 2015 there have
been reports of loss of GPS signal, and CoNOTAMs were promulgated for areas subject to such events. Preventative maintenance
actions have been put in place to reset MMRs after GPS jamming events, and EKIB-65 issued to 777 flight crew, followed by the GPS
Signal Interference NNC on 09/09/2021, which affect the 777 only. A Safety Issue has been raised for wider issues surrounding GPS
spoofing.

ATC SERVICE STANDARD – COMMUNICATION CONFUSION
Reaching position NINVA, ATC (Baghdad 132.875) instructed the flight to contact ANKARA on 127.925. The flight crew acknowledged
this instruction and contacted ANKARA. However, a short while later, they received an ACARS message requesting contact with
Baghdad on frequency 125.9. Despite several attempts, the flight crew could not establish communication with Baghdad on that
frequency. Subsequently, once the aircraft was already in communication with ATC (129.7), they were again instructed to contact
ANKARA on 127.925, the same frequency they were already in communication with.

DISRUPTIVE PASSENGER-RESTRAINT
During cruise a passenger moved from their seat in YC and sat in FC and refused to return to their original seat. It was confirmed
by the passenger's companion that the passenger had autism. Then the passenger went to the FC galley and tried to access the
cockpit by pushing the door and pressing the keypad. The captain was informed. A formal warning was issued and a decision was
made to restrain the passenger. Security services met the aircraft on arrival.

LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING
GPS jamming was experienced in the Ankara FIR and both GPS did not return to functionality until after engine shutdown.
Flight Safety update: Non-recovery of GPS on the A380 occurs in less than 1% of GPS jamming events recorded in the technical log.
External electronic jamming is the most probable cause of the reported GPS signal loss. Since January 2015 there have been
reports of loss of GPS signal, and CoNOTAMs were promulgated for areas subject to such events. Preventative maintenance actions
have been put in place to reset MMRs after GPS jamming events. A Safety Issue has been raised for wider issues surrounding GPS
spoofing.

May 2023 B777 DXB - AMS Cruise


LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in cruise. EGPWS PULL UP TERRAIN occurred at FL340. GPS R did not recover, and, in accordance
with MCC, the GPS SIGNAL INTERFERENCE NNC was completed with MMR reset.
Flight Safety update: The QAR and PLF data showed that the aircraft experienced at least two areas of GPS jamming, with the first
being over BGW airport at approximately 0230 UTC, and then the second being near to position SIVAS in the Ankara FIR. During the
later event, an EGPWS PULL UP warning displayed at FL340 for a period of six seconds at 0325 UTC. GPS R did not recover, and
when in the Budapest FIR at 0517UTC, the flight crew completed the GPS SIGNAL INTERFERENCE NNC, which resulted in MMR R
being reset in coordination with MCC. External electronic jamming is the most probable cause of the reported GPS signal loss. Since
January 2015 there have been reports of loss of GPS signal, and CoNOTAMs were promulgated for areas subject to such events.
Preventative maintenance actions have been put in place to reset MMRs after GPS jamming events, and EKIB-65 issued to 777 flight
crew, followed by the GPS Signal Interference NNC on 09/09/2021, which affect the 777 only. A Safety Issue has been raised for

GPS jamming was experienced in the Ankara FIR for a period of 45 minutes. The reporter did not specify any flight deck effects
observed.
Flight Safety update: External electronic jamming is the most probable cause of the reported GPS signal loss. Since January 2015
there have been reports of loss of GPS signal, and CoNOTAMs were promulgated for areas subject to such events. Preventative
maintenance actions have been put in place to reset MMRs after GPS jamming events, and EKIB-65 issued to 777 flight crew,

2022 MEDICAL-PASSENGER ILLNESS-DIVERSION
During cruise a passenger became ill. In consultation with Ground Medical Support and VPNC the flight diverted. The passenger
was met by medical services on arrival.
