The below are mandatory document requirement for issuing of gate pass.

----------------------SERVICE ENGINEER TEAM-------------------------

Please ensure to reach at Reception gate at morning 8.30 AM to 9.00 AM or 2.00 PM to 2.30 PM
to complete the document verification, Gate pass process & Safety training in time. Company will
do the safety training daily on working days in two slot i.e. 9.30 to 10.30 am and 4 to 5 pm.
If you reach late then Safety training will complete in the next slot. (Safety training is compulsory
to complete the gate pass process).

Safety requirement:
1) Helmet - KARAM; LESLICO; UDYOGI; ACME - CHAMPION
2) Goggles - UNICARE; 3M; KARAM; VENUS; UDYOGI (Visitor Goggles Type NOT ALLOWED)
3) Hand Gloves - KARAM; LESLICO; LAKELAND; FROUNTAIR
4) Shoes – KARAM; ACME; UDYOGI; TIGER; LIBERTY
5) Reflection Jacket – KARAM ; UDYOGI ; 3M
6) All switchboards should be good condition without any damage, as our safety team will
check during gate entry.
7) All portable tools and tackle should be good condition, as our safety team will check during
gate entry.
8) Vendor should deploy safety Supervisor as per CSM policy and need to be interviewed with our
safety personal after selection only ,they need to enter in to site for work execution. otherwise
they need to bring new supervisor in case of not selected. Safety supervisor deployment as per
user requirement and as per criticality of work.

Gate Pass Issue requirement:

1. Adhar Card with full date of birth (Date/Month/Year).
2. ESI E-Pehchan card (TIC form) and Date of birth should match as in Adhar card.
3. 2 Passport size Photos.
4. If the employee not covered under ESI, he should be covered under individual insurance
covering 13 lakhs (1 lakh for hospitalization & 12 lakhs for fatal accident).
5. Request letter from Company letterhead that all are testing and service engineers.
6. Returnable gate pass for tools and tackles on Company letterhead.
7. Safety Induction to be completed.
8. Remaining terms and conditions will remain as per PO. (PO copy should bring to time office
for verification).
9. For issuing Kronos HID Card (Kronos Gate Pass/Attendance Card) it takes 15 minutes for
registering new contractor/vendor. In addition, it takes 15 minutes to generate HID Card
for each individual (should key contract workers personal information, scan and upload
documents and take photo).

------------------------SERVICE WORKMEN TEAM----------------------------

Safety requirement:

Gate Pass Issue requirement:
1. After issuing of P.O by Purchase Department, the vendor should immediately approach
Time Office with a request letter for issuing of Form-V. Basing on that Time Office Issues
Form-V / Form-VIA for obtaining labour licence.

2. After getting the labour licence from Labour Department / Mee Sava, the vendor should
submit the original labour licence along with Welfare Fund Copy to Time Office.

3. In case if their manpower is from other states, interstate migrant licence to be submitted in
Form VI.

4. The vendor should produce the following documents for obtaining gate pass to their
workers:
a. Aadhar Card with full date of birth (Date/Month/Year).
b. ESI - TIC form (Temporary Identity Certificate). The date of birth should match as in
Adhar card.
c. 2 Passport size Photos.
d. If the employee not covered under ESI, he should be covered under individual insurance
covering 13 lakhs (1 lakh for hospitalization & 12 lakhs for fatal accident).
e. Request letter from vendor Company on their letterhead that their employees are
testing / service engineers / workmen duly signed by concern user department.
f. g. Safety Induction to be completed Time Office will sign the gate pass.
g. Returnable gate pass for tools and tackles on Company letterhead.
h. Other terms and conditions will remain as per PO.
i. If contract worker is a driver, he should possess valid driving licence and the details of
licence should be tallied with Aadhar Card.
j. If the contract workmen/service engineers/testing e engineers/SME’s/Contractors/
Company Officials who are visiting the mills are aged above 58 / 60 years - ?? What should we
do!!
