DEED OF DONATION OF A PERSONAL PROPERTY

KNOW ALL MEN BY THESE PRESENTS

This DEED OF DONATION is made and executed this 20 th day of MARCH at Balungao,
Pangasinan by and between:

EDUARDO P. APALLA, Filipino citizen, of legal age and resident of Brgy. San Joaquin, Balungao,
Pangsinan and hereinafter referred to as DONOR;

-and-

BRYAN A. UMAYAM, Filipino citizen, of legal age and a resident of Brgy. San Joaquin, Balungao,
Pangsinan referred to as DONEE.

W I T N E S S E T H:

WHEREAS, the DONOR is the registered owner of a sports utility vehicle wagon and more
particularly described as follows:

MODEL 2018

FOR AND IN CONSIDERATION of the generosity and appreciation of the DONOR unto and
for the DONEE, the DONOR, by these presents, voluntarily GIVES, TRANSFERS, and CONVEYS,
and has GIVEN, TRANSFERRED and CONVEYED by way of DONATION, unto the DONEE the
above-described motor vehicle, free from liens and encumbrances.

The DONOR has reserved for himself other properties in full ownership sufficient enough to
support him according to his economic status in life. This donation is not made in fraud of creditors since
he has more properties left to answer for any indebtedness or obligations contracted by him.

ACCEPTANCE BY THE DONEE:

The DONEE hereby accepts this Donation and expresses his heartfelt gratitude for the kindness
and generosity of the DONOR.
 IN WITNESS WHEREOF, we have hereunto affixed our signatures on the date and place

who have satisfactorily proven to me their identity through their identifying documents written
below their names and signatures, that they are the same persons who executed and voluntarily
signed the foregoing Deed of Donation which they acknowledged before me as their free and
voluntary act and deed. Further, persons acting in representative capacity declare that he/she has
been given authority to sign in said capacity.
The foregoing instrument relates to a Deed of Donation consisting of three (2) pages
including this page of acknowledgment signed by the parties and their instrumental witnesses on
all pages thereof, in my presence, and sealed with my Notarial Seal.
