Explanatory Notes

1. This form is to be completed and submitted to the Registrar of Singapore Ships when applying to register a ship and
immediately on every occasion the manager is changed.
2. The manager may be a company or an individual. An owner may appoint itself as manager.
3. The manager must be resident in Singapore and is responsible for the operations of the ship, in particular for all matters
related to the crew, safety and environmental protection. All communications relating to the ship will be directed to the
manager.
4. If the manager is a company, the name and status or title of the person in the company with the ultimate responsibility for
the ship must be given.
5. The confirmation must be made by the person appointed as manager.
6. Delete this statement if the owner of the ship is an individual.
