PARENT’S PERMIT AND WAIVER FORM

This is to certify that I am allowing my son/daughter,________-

_____________, to take the National Achievement Test of Grade 12 at the Asian
Business Cabletow Cooperative Academy, Inc. (ABCCA) Building at Corrales
Avenue, Cagayan de Oro City on March 20-21, 2024 as his/her assigned schedule
pursuant to the mandates of the Department of Education.

I also allow him/her to travel from home to the school campus and vice versa
in attendance of the said test. It is understood that my son/daughter will obey the
policies and guidelines set by the school and the Department of Education for his/her
safety and welfare.

I understand that ABCCA shall exercise care and diligence in traveling from
home to the school to take the said exam on the campus and shall hold the institution
free from any liabilities that may arise thereafter.
