at Cagayan De Oro City on March 10, 2022.

We, parent/guardian, are aware that we will shoulder the transportation, food and accommodation
as indicated in the budgetary requirements.

Further, regarding the conduct of the said school activity, the parent/guardian/ward will waive any
legal rights; and will not hold responsible the College Administration represented by the Chaperon for any
untoward incident that may happen due to students' deviance from the itinerary, misconduct, and
carelessness.
