PARENT’S CONSENT

We, the undersigned, do hereby permit our son / daughter

to stay in Colegio de San Juan de Letran Calamba campus from

6:00 pm to 8:30 pm 6:00 pm to 10:00 pm

on March 21, 2024, Thursday, to watch the movie / two movies to be shown during the
Knight Sky Cinema. The movies to be shown are Elemental (Rated PG) and Barbie (Rated
PG-13).

We likewise hereby waive any claim to the Colegio and its any authorized representative for
any incident beyond human control that may occur during and after the event. Provided,
however, that those concerns have undertaken utmost care, attention, and participation.

Further, I will also be in Letran Calamba to accompany my son/daughter during the

Arriba Knight Sky Cinema.

Further, I will not be in Letran Calamba to accompany my son/daughter during the

Arriba Knight Sky Cinema.
