1. I am the School Head in Pasong Kawayan II West Elementary School during

this event;

2. I hereby declare that I consent the procurement of the following items for the
Principal’s Office on November 2020:
a. L-Type Sofa
b. Inverter Aircon
c. Executive Swivel Chair

3. I hereby affirm and confirm that the payment of the above-listed items was
partially paid using the School's MOOE Fund. The remaining payable amount
has been paid in full using School Fund earned through Cash Donations from
the Stakeholders and Teachers.

for the Principal’s Office on November 2020:

3. I hereby affirm and confirm that the payment of the above-listed items was
partially paid using the School's MOOE Fund. The remaining payable amount
