2 Introduction
The Accident Prevention Log is required by NIS/NOR Regulations on all Norwegian
Flag ships. WSM has decided to implement this Log across all its managed ships,
regardless of Flag as it provides a valuable tool for tracking the activities of Safety
Representatives.
The NIS/NOR Regulations state:
The safety representative shall keep an accident prevention log in which he shall
make entries concerning his work, such as instructions and requests. The same
applies to requests made to the master. The accident prevention log shall be
submitted to meetings of the working environment committee.
The accident prevention log shall be kept available on board for not less than three
years after the last entry was made.
The Senior Safety Representative will maintain the Accident Prevention Log
onboard. He will control this and distribute the duties to his safety representatives
and present it during safety committee / environmental committee meetings. When
the Senior Safety Representative is at the end of his tenure he shall pass it onto his
appointed successor. The name of the representative is to be entered into section 3.
Safety Representative duties are given in SSMM Ch. 6.5.4.4
Accident, near accidents and hazard observations are to be reported in DocMap/
DE45 and not entered into this Log.
WSM has additionally decided to include a review of PPE to enable ships to track
adequacy of PPE. This matches the PPE matrix contained within SSMM Ch.6.2.2
Copies of pages in sections 3, 4 & 5 made be made as required

If printed, this is an uncontrolled paper copy. Whenever this document is revised, a


printed copy of the new version shall replace the old if a hard copy is required. It is
the responsible of the reader of this document to ensure that this paper copy is
valid.
