The documents should be scanned clearly and attached in a single file in the following order.

1. Principal Forwarding Letter on school letterhead.

2. Parent / Candidate Request Letter.

3. Completely and correctly filled in and signed proforma.

4. Attested copy of Previous School Transfer certificate admission in middle classes.

in case of (If the T.C. is in another language, the translated version in English should be submitted).

5. Attested copy of Admission Form.

6. Attested copy of complete page of Admission & Withdrawal Register where the candidate details are
recorded.

7. Attested copy of Birth Certificate. (If the Birth Certificate is in another language, the translated version
in English should be submitted).

8. Copy of Certificates of Class-X/Class-XII (if any) issued by the CBSE/OTHER BOARD.

9. Copy of page where the Candidates particulars were given in the list of Candidates of Class-X/XII (as
applicable)

10. Copy of Public document, i.e Aadhar Card/Pan Card/Passport, etc.

The above-mentioned documents in single pdf file [Compressed] within 12 MB, should be sent to this
office immediately through E-harkara portal for processing. Encl: DOB correction Proforma

issued at Cochin. (Copy of my Passport is attached)

My Date of Birth is mistakenly recorded as 28.5.1991 in my Secondary School

Examination Mark list issued by the Central Board of Secondary Education. (Copy of my
Secondary School Examination Statement is attached).

I have appeared for my Secondary School Examination 2006 through Navodaya

Vidyalaya, Malampuzha, Palghat, Kerala and my Roll No. is 4141820. An affidavit is attached
herewith stating that my correct my date of birth is on 26.11.1990.
 So I humbly request your Goodself to correct my date of birth as 26.11.1990 in my
Secondary school Examination Statement 2006. I expect your kind considerations and speedy
remedy.
