THIS IS TO CERTIFY THAT CHARLES JOHN T. CUSTODIO, 25 years old, single, a bonafide
resident of Barangay Bayambang, Infanta, Pangasinan is the step-grandson of recently
deceased JESUS R. BACO JR. He is the only available immediate family member of the deceased
and as such was tasked by the other family members to process any burial assistance that may
be obtained to defray the balances incurred in the expenses on interment of his step-
grandfather.

THIS IS ALSO TO CERTIFY that the said person is personally known to me, because he is my
Barangay mate and as the Punong Barangay of Bayambang, Infanta , Pangasinan, I am aware of her
status within the barangay.
