PRELIMINARY INCIDENT REPORT

On dated: 20/03/2024 Shift-D has taken charge from shift B at about 08:10 hrs as per routine
checks carried out by shift staff and noted following parameters.
Unit#1 Turbine Front Operator visited DC charger room and found battery charger in service, and
noted DC 240 volts and MOT level 15.1cm, Turbine Generator Operator found no any indication
on BTG panel and checked testing alarm, Generator Sealing System was in service noted H2
pressure 2.1 kg/cm2, As per Cooling Tower Operator Fire Fighting Tank level 7.0 Meter.

Unit#2 Generator Attendant visited DC charger room and found battery charger in service, and
noted DC 230 volts.

At about 08:20 hrs Turbine Front Operator Unit No:1&2 reported that smoke is coming from cable
bus ducts / cable trays and from DC Battery Banks Room of unit 1, immediately informed to local
fire section and shift staff mobilized for tracing actual fire location and extinguish, under signed
rushed to the site along with J.E, TG operator and TF Operator continued efforts to trace the smoke/
fire/ fault location.
Whereas smoke found from the Unit#1 battery bank room and found DC battery charger was still
in operation to which Senior Shift Engineer tried to trip/cut off charger from its mimic control
panel in order to isolate the effected DC battery bank but could not succeed. Keys of battery bank
rooms and Battery Charger panel are under custody of Electrical Section.
Meanwhile, at about 08:25-08:30 hrs, informed to Fire-In charge, EE-I, RE(O), ARE(O) and others
related persons.

At about 08:26 hrs Turbine Basement Operator reported that DC Power fail indication appeared
on H2 seal oil system local panel whereas H2 AC Sealing Oil pump was in service. Whereas no
any indication / alarm appeared on BTG Desk.

At 08:30 hrs Small firefighting Pump put in operation.

At about 08:35 hrs Chief Engineer (T.D), RE(M)/RE(O)/ARE(O)/ (JPCL Management) arrived at
site.

At 08:40 hrs Electrical Section opened the door lock of DC Battery Bank Room at that time heavy
dense smoke and high intense fire was observed from battery bank cells. Electrical Section cutoff
DC system Charger from battery banks of Unit#1.

As the smoke spread quickly and covered whole CCR/Relay Rooms and Surrounding areas. After
isolation of DC Battery Bank from battery charger, Firefighting staff extinguished fire with water
on blasted battery bank cells.

At about 08:42 to 08:47 hrs, after failure of DC supply, Turbine basement operator was directed
to escape H2 gas from Generator to avoid any incident.

At 08:48 hrs Generator sealing system cut off.

At about 08:50 hrs DC System of Unit #2 was also isolated from Unit#2 Battery Charger to avoid
any problem at unit 2.

At 08:53 hrs Unit #2 supply from Uni t No: 3 through breaker No: 230 opened.
The Fire smoke further flared up and expanded into all cable bus ducts, and from cable trays of
Unit#1&2 and nothing is visible due to dense black smoke and It was very difficult to stay at CCR,
so shift staff left CCR 1&2.

At 10:07 hrs Starting Transformer (T4) cut-off from 500KV grid station. After that Fire Fighters
spray waters on cable ducts, CCR,5 & 10 Meter cable trays.

At 10:07 hrs Small FFP stopped.

At 10:10 Emergency Diesel Engine Fire Fighting P ump put in to operation.

In addition to shift and maintenance staff, internal F.F team along with F.F vehicles and crew from
NTDC, PAK ARMY PETARO, Bolhari Kotri, Air base, Hyderabad local Govt: Municipal
Corporation were available for help and support to extinguish fire.

Internal Wapda Fortified Dispensary Medical Team along with In-charge medical officer and
Rescue 1122 team also available, no any causality/ Injuries occurred.

At 11:10 hrs fire was extinguished and further cooling process started.

At 14:15 hrs cooling process is in progress to minimize temperature of affected areas.

At 14:15 Fire Fighting Tank Level noted 6.0 Meter.
