Subject: Written Warning, Notice to Explain, Notice of Hearing

Ms. De Vera:

It has come to my attention that last February 18, 2024 you withheld and took money from the store’s
daily sales cash drawer in the amount of sixty (60) pesos for personal use. This was also not the rst
time that this happened and you were already given a verbal warning that this behavior is a clear
violation of the store’s employee rules and regulations handbook and is grounds for immediate dismissal
or employee termination. (Section G # 6 of the Get Greens Rules and Regulations Handbook) This was
again repeated on February 20, 2024 and was reported that the amount of one hundred (100) pesos was
again taken from the store cash drawer without any authorization and was used for personal purposes.
Other than those mentioned, I have noticed on several occasions where you do not wear the store’s
personal apron despite having been advised to do so which is the only required dress code of the role of
a store barista. (Section E #2)

These alleged acts of dishonesty are considered as:

1. Serious misconduct
2. Willful disobedience
3. Gross and habitual neglect of duties
4. Willful breach of trust.

Which are grounds for dismissal for employment Article 282 of the Labor Code of the Philippines.

Please submit your written explanation within the prescribed period of FIVE (5) days upon receipt of this
Notice.

Failure to respond with a written explanation within the prescribed period shall be deemed as a waiver
of your opportunity to be heard and constrain us to decide on the matter based on the available
information and evidence on hand.

Also, you are directed to appear for a hearing on March 2, 2024 at 10:00 am at the store. This will give
you additional opportunity to verbally explain your side and present your witness and other evidence
related to the said disciplinary matter.
