I. Terms and Conditions of Employment

Hiring Rate : PhP 500 per day (training period) and PhP650 per day (after 1 week training period)

Status : 6 months probationary

Working Days / Week : 4 Shifting Schedule : Yes

II. Benefits

One time oral prophylaxis per year.

Vacation Leave (15 days per year)

Sick Leave (15 days per year)

III. Termination by Employee: At least one (1) month prior notice.

IV. Waiver

I agree to start working on the above mentioned date, with the understanding that my employment will still
be subject to the completion of my medical check-up within one (1) month from date hereof, and will be
subject further to the results of the same.

I hereby undertake not to share or divulge the company’s recipe procedures and company business
processes to other people, staff, colleagues and future employers during and from my separation or
termination from the company. For breach of this paragraph, you shall pay the Company liquidated
damages in an amount equivalent to twelve (12) months of your salary, or Three Hundred Thousand Pesos
(Php300,000.00), whichever is higher, in addition and without prejudice to other remedies and damages that
the Company may be entitled to under the law.
