AFFIDAVIT OF EXPLANATION
(Re: Issuance of Travel Document)

I.JANET NOBLE VELARDE, of legal age. Married. Filipino and presently

Residing at 239 Kaliraya St. Barangay Tatalon Quezon City, after having been duly sworn
to accordance with law, do hereby depose and say,

That I am the holder of Philippine Passport Number ZZ235170, issued at

the Philippine Consulate General, Guangzhou, China on 07 November
2012

That the said passport was used to travel to china to work in a hotel as a
From Office Attendant

That on 06 January 2012 while I was waiting for a Taxi in Dongguan,

Houjie, China my bag containing my Philippine Passport along with my
other personal belongings was snatched by two unknown perpetrators,

That in view thereof, I Sought help to the Philippine Consulate General,

Guangzhou, China and was issued a Travel Document Number 002185
on 17 February 2012, on which was stamped my arrival to the Philippines
on 07 March 2012;

That I am executing this affidavit in order to attest to the truth of the

foregoing fact and for all legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have here unto set my hand this 29th day of
October, 2012 at the Department of foregn Affairs Office of Consular Affairs,
Paranaque City, Philippines.
