I.

HAZARD IDENTIFICATION

1) SCENARIO:
Project to extend and upgrade one out of two runways at an international airport (Capacity
150,000 take off/landing per year)

The Project is divided by 3 Phases:
Phase 1:
Extend the width of the runway 17-35 from 45m to 60m, 200m to the south from cross sec-
tion with runway 10-28.
Phase 2:
Extend the width of the runway 17-35 from 45m to 60m, 200m to the north from cross sec-
tion with runway 10-28.

Phase 3:
Complete the extend, upgrade runway 17-35 in the center area 400m at cross section
between runway 17-35 & 10-28 (runway surface from asphalt to concrete). The width from
45m to 60m.

Project period: 4 months.


Continue using runway 10-28 while under construction:
Continue using runway 10-28 while runway 17-35 under upgrading (To maintain produc-
tion & safety).
Current Runway 10-28 length is 4100m. In phase 3, the length is reduced to 2600m from
runway 10 and cross section between runway 10-28 and runway G.

II. RISK ASSESSMENT


1) SCENARIO
A Boeing 747 from Singapore to Los Angeles through Taipei got into accident while tak-
ing off from CKS Taipei Airport at 23h18 LT.
a) The flight has been given instruction to take off from runway 05L
b) The runway 05R has been closed for repairing/Construct
c) The crew tried to take off from 05R runway
d) The airplane has impacted with concrete barriers, construction equipment on the
runway 05R while taking off.
e) The plane then fall off to the runway, breaking apart and fire while slide to the end
of runway, and continue impact with other construction equipment on the runway
05R.
f) Investigation report revealed crew mistake when tried to take off from runway 05R
(9092x150ft) instead of runway 05L (12008x200ft).
NOTAMS stated that: At the time of accident, runway 05R has been closed for repairing,
with many construction equipment on the runway surface.

 Perform Risk Assessment step:
List the type of operation/activity
a) Identify the generic hazard
b) Identify the specific hazards (1 Generic hazard may have multiple specific hazards)
c) Identify the related consequences for each specific hazards.
d) Assess the effectiveness of existing barriers/mitigations in place. Assess the risk
level by index.
e) Propose further actions for continuing mitigate the safety risk and assess the risk
level again.
