As per Article II, paragraph 1 (f) of MLC 2006, I, the undersigned, confirm my consent for Gretimybe Ltd,
Klaipeda, Lithuania, to hold, process and transfer for further Employment my personal information.

To review or to process my request for employment, my personal details may be provided to Ship Owners,
Flag State Documents Issuing Authorities and other Third Parties on the basis of a legitimate reason with
the purpose to seek and find a legal employment. Such information shall include identification documents,
professional qualifications / training courses, sea service, previous experience and recruitment details such
as interviews, test results and updates on such.

I acknowledge my legal right under the Data Protection Law to see any personal information on me that is
kept by Gretimybe Ltd.

I acknowledge my right to rectify such information on me if I can prove it is not correct.

I acknowledge my right to withdraw my consent to any further processing of my personal information by

Gretimybe Ltd.
