In partial fulfillment of the requirements in EDUC 412 Teaching Internship, at Mindoro State
University, I am writing to humbly request permission to conduct my proposed action research
entitled “Unveiling the potential of integrated Booklets: Exploration of their role in
improving Physics 9 academic achievement”, in your highly-acclaimed institution. This
study aims to investigate the effectiveness of integrated booklet in enhancing academic
achievement in Physics 9 which includes applications, conceptual summaries, and key ideas
together with learning activities to enhance teaching and learning process.

Moreover, I am hoping that your esteemed office will allow me to employ my proposed study,
utilizing a total of thirty (30) students from Grade 9-SSD, who will be the subject for series of
observations and will be tasked to complete a comprehensive set of pre-test and post-test
questionnaires.

The tests results will be pooled from the research project, and individual results will remain
anonymous and confidential. I assure you that neither the school nor the individual
participants will incur any costs associated with this study.

Your kind and favorable approval of this request will greatly help me in the completion of the
study.
