AFFIDAVIT OF UNDERTAKING

I, MMM, of legal age, Filipino, a resident of Purok Apang, Casili, Mandaue City,
Cebu after having been duly sworn in accordance with law, do hereby depose and say
that:

1) I am one of the co-owner of a parcel of land known as Lot No. XXX situated in
Brgy. Canduman, Mandaue City ;

2) I am applying for the issuance of a Building Permit from the Office of the
Building Official on a portion of the abovementioned parcel of land;

3) I am planning to build a one-story residential building with store on the said

portion of the parcel of land;

4) In the event any issue on ownership may arise, I hereby undertake that the Office
of Building Official will have no pecuniary liability or whatsoever in connection
thereto;

5) That I am executing this affidavit in support of our application of a Building

Permit from the Office of the Building Official of Mandaue City and for such
other legal intents and purposes this affidavit may serve best.
