AFFIDAVIT CUM UNDERTAKING

I, Vinay Kumar Jain s/o Shri Harish Chand Jain, r/o G-2, Gayatri Nagar, Jaipur do hereby solemnly
affirm and declare under:

1 That I hereby declare that I have received Rs. 1,10,000/- (Rupees One Lakh Ten Thousand
Only) as advance from Modest Infra Limited (“the Company”) vide Cheque No. ___________
drawn on _________________________ Bank.
2 That I authorize the company to deduct Rs. 15,000/- (Rupees Ten Thousand Only) per month
for coming 8 months and Rs. 5000/- (Rupees Five Thousand Only) for 9th Month, out of my
salary as refund for the advance payment.
3 That I have also supplied a Blank Cheque and the Company shall be free to use this cheque for
any due payment out of the advance paid amount.
4 That I hereby undertake to indemnify the company for any losses so sustained by the Company
in case of delay or non-payment/part-payment of the advance paid amount.
