PHYSICAL EDUCATION STUDENT-PARENT CONTRACT

To help you successfully complete this course it is important that you familiarize yourself with the
following information.
RULES AND EXPECTATIONS

ATTIRE – All students are only allowed to wear the school uniform in order to participate and
receive a grade. No street clothes, tank tops, polo shirts, cut-off shirts/shorts, tights, stockings or
panty hose are permitted. Proper shoes are required and jewelry should not be worn. Wearing
anything other than the above specified will not be allowed.

ATTENDANCE/EFFORT/PARTICIPATION – A student’s grade reflects effort and participation


throughout the term. My goal is for every student to be successful and I understand that what may be
easy for some may not be for all, therefore effort and participation is of upmost importance. Every
student is expected to participate fully in daily activities. Non-compliant students will lose
participation points for the day. If on a given day, a student is unable to participate and has a doctor’s
note and/or parent-guardian note, the student may be excused for the day; after dressing out for the
class. Students who are absent fail to participate, therefore attendance is mandatory.
• If a student is sick and or absent, a note from the doctor or parent is required within 48.
Please submit a note through the school website within 48 hours of the class period missed so
that the absences can be excused. A “Z” will be placed as the grade. If the note is not excused
and or turned in within the allotted time, or the day missed made up, the “Z” will stay and
become a “Zero”.
All excused absences from class must be made up and non-participation due to injury or medical
excuse must be made up by written make-up (*Alternative Assignments). It is the obligation of the
student to meet with their teacher either: a) prior to the beginning of a medical excuse or b) the day
immediately following an absence from class to receive their assignments and due dates.
Safety- All students are expected to adhere to safety rules and procedures as well as activity/game
regulations. While incidental body contact is sometimes unavoidable during some physical education
activities, willful, flagrant, and/or overaggressive contact will not be tolerated.

Sportsmanship – sportsmanship must be displayed at all times. Inappropriate language will not be
tolerated at any time. Talk to people the way you want to be spoken to. Encourage others in a positive
way.

Injuries- students need to report all injuries to the teacher.

Grades- are cumulative and will be formulated based on:


Participation, effort, preparedness, attendance: 70%
Tests (written and performance assessments): 30%

Miscellaneous- Please be aware that Cell phones are not allowed at any time during P.E. Gum is
not allowed in the Gym or on the Field, at Any Time! Food is not to be eaten (exception medical
needs) Students will be warned at the beginning of class after which infractions will be upheld for
failure to adhere to rules.

Medical Conditions: If there is any health problem or medical condition that prevents a student’s
participation in Physical Education such as asthma, diabetes, heart condition, back problems,
epilepsy, etc. the teacher must be notified as well as Administrator. Please attach a doctor’s note to
this form and or submit it via email to me and to the main office so that it is documented and on file.
