I am Mr. S. V. Subbarayudu S/o Late S Nageswararao Residence Address Plot No, H.NO 1-23-444, near
seven temples, bhoodevinagar, secunderabad-500015

Holding the postal insurance policy no: R-AP-VP-EA-61832 name of the policy holder is s satyavathi aged
55 years. The nominee of the policy is Mr. S nageswararao aged 67 years and he is the husband of the
policy holder.

On May 2021 the policy holder demised due to covid-19, subsequently on 9-5-2021 the nominee of the
policy holder Mr. S. Nageswararao was also demised due to covid-19.The legal heirs of the policy holder
and nominee are mentioned below

Due to sudden demise of my parents (policy holder and nominee) and also due to some UN Avoidable
