Accident/Incident Reporting

1. Always report any accidents or near misses to your Site Supervisor and Employer.
2. Any injuries on site needing first aid or medical attention should be reported to your
Employer.
3. In the case of an emergency contact the site first aider and calling for an ambulance
if required.
4. Make sure you know where the nearest hospital is.
5. Do you know who the first aid qualified person or persons are on the job site?
6. If you witnessing an accident you should report what you saw to your Employer.
7. All accidents involving medical treatment should be recorded in the site accident log
book and have a full investigation conducted to determine the cause.

Remember: Always report any unsafe condition or unsafe acts, no matter how minor. It’s far
better to prevent accidents than it is to have to report one and go through the process of a
full accident/incident report and investigation.
