FORM USED
IN EIM
At the end of the day, learners should be able to;

• classify the different types of forms in EIM

• identify necessary tools needed to borrow in
different scenarios
• Make use of borrower's form of EIM
• Value the importance of forms used in EIM
What is the purpose of Forms
Correctness of the entry of
different forms used in electrical
work is necessary to acquire the
RIGHT, COMPLETE, and
ACCURATE number of materials
and tools needed in a specific job,
Types of
Form
REQUISITION FORM
1. A request is a formal written application, usually
on a pre-printed form. An internal document is
used to inform stores or purchasing departments
about the items that are necessary or should be
ordered. The document specifies the required
quantity and the time frame for the delivery. It
may also contain the authorization to proceed
with the purchase if the required items are not
available.
Requisition Form
Inventory Form
Contains information ( Name numbers, sales
and purchase information, and cost of goods
sold account information) about items that
any business purchases for sale, tracks
inventory, and displays on a balance sheet.
Inventory Form
Job Order/Work Order Form
It is a written instruction to perform a work
according to specified requirements, within
a specified time frame and cost estimates. It
is an internal document extensively used by
projects-based manufacturing buildings,
fabrication, and businesses, and also used
for products and/or services.
Job Order/Work Order Form
Borrower’s Form
Is used for requesting tools
and equipment needed for a
particular Job.

Activity
Directions: Using borrowers
forms, write the electrical tools
that are needed on the questions
provided.
Questions
1. What tools do I need to borrow to continuity test
a cable and check for a continuous flow of
current?
2. I need to cut wires, however, I also need the
tools that are used for holding, bending, and
twisting. What tools I should use to borrow?
Questions
3. I need to loosen the screw inside the safety box.
However, I saw that the screw head is a cross-slot
head. What appropriate tools should I borrow?

4. I need to cut the metal conduits. What tools

should I borrow to cut the metal conduit?

5. I need to drive and pull some nails in my

workplace, what tools do I need to use?
Answer:
1. Multimeter
2. Lineman's plier/ Bull plier
3. Philips screwdriver
4. Hack saw
5. Claw Hammer

Ice Breaker
The players must obey all commands that begin with
the words “Simon says”. If Simon says, “Simon says
touch your nose” then all players must touch their
nose. However, if Simon says, “jump” without saying
“Simon says” first the players must not jump. If they do
jump, that player will remain standing and the other
will sit. And those players who are standing will write
the title of our lesson using their butt.

Measuring
Tools
Objectives:
At the end of the lesson, the student should
be able to.:

• Identify the measuring tools or instruments

• Make use of the various types of
instruments.
• to recognize the significance of various
types of measuring tools.
Measurement is the process of
determining the magnitude of a
physical quantity, such as length,
time, volume, temperature, etc.,
relative to a unit of measurement,
such as meter, second, liter, or
Fahrenheit.
Testing and measuring electrical
circuits and equipment is crucial.
Visible indications of electrical
quantities are achieved via instruments
that indicate magnitudes by either the
position of a pointer (analog) or a
decimal number (digital).
Measurement tools/Instruments
1. Test light is useful in determining if the
electrical power is present in a part of a
circuit.

2. Micrometer( Mike) was used to make

precise measurements. This can
measure to 0.0001 inch and measure
the outside/inside Diameter or depth of
an object.

3. Wire gauge- measure how large a wire

is, either in diameter or cross-sectional
area. It determines the amount of
electric current a wire can safely carry.
Measurement tools/Instruments
4. Ruler/ Foot rule is a stick
one foot long for measuring
length or distance

5. A pull-push rule is a
measuring tape that coils into
a compact case.

6. Ammeters are used to

measure electric current in
amperes. Amperes is a unit of
measurement for determining
the amount of electricity
moving through a circuit.
Measurement tools/Instruments
7. Voltmeter- an instrument that
measures the voltage or potential
difference between two points of
an electronic or electrical circuit.
Usually, the voltmeter is used
for Alternating Current (AC) circuits
or Direct Current (DC) circuits.

8. Clamp Ammeter- a clothespin-

shaped instrument that can be
clamped around a live wire to
measure the current it's carrying.
Measurement tools/Instruments
9. Volt-ohm meter
measures AC volts, DC
volts, and resistance
and can be used to
troubleshoot
controllers,
transformers, solenoids,
and field wiring as well
as verify AC and DC
voltage levels.
Directions: Identify the following electrical tools and
Quiz! instruments and give the function or uses. Write your
answer to your ½ sheet of paper. (2 points each)
