Sequence of Activities
1. Visual Inspection
a) Inspect all components of the Disabled Toilet Alarm system for any visible damage or defects.
b) Ensure that the control panel, pull cord, reset button, indicator light, and audible alarm sounder are
all in place and free from damage.
2. Functional Testing
a) Activate the pull cord to simulate an emergency alarm activation.
b) Verify that the alarm indicator light illuminates immediately upon activation.
c) Use the reset button to reset the alarm and ensure that the indicator light returns to its normal state.
3. Electrical Testing
a) Use a multimeter to test the continuity of the pull cord circuit.
b) Test the voltage output of the control panel to ensure it is within the specified range.
c) Verify that all electrical connections are secure and free from damage.
5. Integration Testing
a) If the Disabled Toilet Alarm system is integrated with other building systems (e.g., fire alarm
system), verify that integration functions properly.
b) Test the interaction between the Disabled Toilet Alarm system and other relevant systems to ensure
seamless operation.
Commissioning Procedure:
1. Ensure that all the pre-commissioning checks are carried out successfully.
2. Check the communication between the zone controller & the electronic module is healthy.
3. Check the power supply for the zone controller is sufficient.
4. Check & ensure that the software is of the latest version.
5. Check all lamps are working properly & functioning as per the sequence.
6. Check for proper functioning of pull cords, indicator lamps & buzzer.
1. Documentation
a) Prepare a detailed report documenting the results of all tests conducted during the testing phase.
b) Include any observations, recommendations, or corrective actions taken during the testing process.
2. Training
a) Provide training to relevant personnel on the operation and maintenance of the Disabled Toilet
Alarm system.
b) Ensure that personnel understand emergency procedures and know how to respond to Disabled
Toilet Alarm activations.
3. Sign-off:
a) Obtain sign-off from the project manager or relevant authority confirming that the Disabled Toilet
Alarm system has been tested and commissioned successfully.
b) Ensure that all documentation is completed and filed according to project requirements.
Safety Precautions
Ensure that power to the Disabled Toilet Alarm system is switched off before conducting any
electrical tests or maintenance activities.
Use appropriate personal protective equipment (PPE), such as gloves and safety glasses, when
working with electrical equipment.
Follow all safety procedures and guidelines outlined in relevant regulations and standards.
Completion
Upon completion of testing and commissioning, ensure that the Disabled Toilet Alarm system is operational
and ready for use. Any deficiencies or issues identified during testing should be addressed promptly to
ensure the safety and functionality of the system.
Tripping Hazards:
Potential tripping hazards due to cables, equipment, or debris in the testing area.
Fall Hazards:
Risk of falls from ladders or elevated platforms when accessing components of the alarm system.
Chemical Hazards
Exposure to hazardous chemicals or materials used in cleaning or maintenance activities.
Environmental Hazards
Exposure to extreme temperatures, humidity, or other environmental factors during outdoor testing
activities.
Risk Assessment:
Likelihood
a) Evaluate the likelihood of each identified risk occurring during testing and commissioning activities.
b) Consider factors such as frequency of exposure, duration of exposure, and probability of occurrence.
Severity:
a) Assess the potential severity of each risk in terms of its impact on personnel, equipment, and the
environment.
b) Consider factors such as potential for injury, property damage, or environmental contamination.
Risk Rating:
a) Assign a risk rating to each identified risk based on the combination of likelihood and severity
assessments.
b) Prioritize risks based on their risk rating to determine the level of attention and resources required
for mitigation.
Hazard Analysis:
Electrical Hazards:
a) Implement lockout/tagout procedures to de-energize the Disabled Toilet Alarm system before
conducting electrical testing.
b) Provide personnel with appropriate training on electrical safety practices and procedures.
Tripping Hazards:
a) Keep testing areas clean and free from clutter to minimize tripping hazards.
b) Use cable covers or ramps to cover exposed cables and prevent tripping accidents.
Fall Hazards:
a) Ensure that ladders and elevated platforms are stable and secure before use.
b) Use fall protection equipment, such as harnesses and safety lines, when working at heights.
Chemical Hazards:
a) Provide personnel with appropriate personal protective equipment (PPE), such as gloves and
goggles, when handling hazardous chemicals.
b) Store chemicals in designated areas with proper containment measures to prevent spills and leaks.
Environmental Hazards:
a) Monitor weather conditions and adjust testing activities accordingly to minimize exposure to
extreme environmental factors.
b) Provide shelter or shade for personnel working outdoors during testing activities.
Mitigation Measures:
Training and Education:
a) Provide comprehensive training to personnel on the identification and mitigation of risks associated
with testing and commissioning activities.
b) Ensure that all personnel are familiar with emergency procedures and know how to respond to
hazardous situations.
Safety Procedures:
a) Develop and implement safety procedures and protocols specific to testing and commissioning
activities, including lockout/tagout procedures, fall protection measures, and emergency response
plans.
b) Conduct regular safety inspections and audits to ensure compliance with safety procedures and
identify areas for improvement.
