I, TERESITA S ESPARTERO, married, of legal age and with residence address at National Highway, Purok
Bagong Silang, Barangay Ambalgan, Sto. Nino, South Cotabato, having been duly sworn in accordance
with law, depose and say:

1. That I am a Certified Public Accountant, with PRC Certificate No. 0035704 and Board of
Accountancy Certificate of Accreditation No. 1249;
2. That my office address is at Stall TBB-3 Public Terminal, Sto. Nino, South Cotabato with
office telephone number 235-1052;
3. That included in my public practice of the accountancy profession is the TERESITA S
ESPARTERO TRAVEL & TOURS and the newly opened BANGKO NG BAYAN.

That, this affidavit is in compliance with the additional requirement of the South Cotabato State College
(SCSC) for the nomination of two (2) private sector representatives of the Board of Trustees.
