AFFIDAVIT OF GUARDIANSHIP

I, JHEDSON ALARCON DIAZ, of legal age, Filipino, and a resident of 183 Mangga,
Candaba, Pampanga, after having been sworn in accordance with law, do hereby depose and say:

1. That I am the elder brother of JHAYSON ALARCON DIAZ ;

2. That my brother JHAYSON ALARCON DIAZ has a nine-year old daughter named
YESSICA LAINE AGUILAR DIAZ who was born on OCTOBER 5, 2013 ;

3. That my niece’s mother is LADY JHANE S. AGUILAR ;

4. That at the time of YESSICA LAINE’s birth, her parents were not married ;

5. That my brother JHAYSON and LADY JHANE are both currently working abroad as
Overseas Filipino Workers (OFW) ;

6. That as such, they have entrusted me Guardianship Rights over their minor daughter
YESSICA LAINE AGUILAR DIAZ ;

7. That I am the current actual and legal guardian of YESSICA LAINE AGUILAR DIAZ
while her parents are working overseas ;

8. That I am executing this affidavit to attest to the truthfulness of the above-mentioned facts ;

9. That I am also executing this affidavit for any legal purpose/s it may serve.
