1. Purpose
This Procedure provides requirements for working at height in a safe manner
2. Application
This procedure applies to all personnel doing activities such as working or passing at surfaces above 6
feet and erecting / dismantling scaffolding, who are exposed to fall hazard in Site controlled facilities.
3. Potential Hazards
The potential hazard related to working at elevated locations:
 Height
 Slippery
 Unbalance
 Limited Space
 Weather: wind speed, lightening, etc
 Work tidiness
 Fatigue
 Chemical
 Etc

which may result the following consequence:


 Severe injuries or fatality due to falls
 Equipment damage or personnel injury due to structure collapse
 Etc.

4. Roles and Responsibilities


Site Line Management is the highest rank in site organization structures who is responsible for the
implementation of this Procedure.

Supervisor who is in charge of the respective task shall ensure that the execution of working at height is
performed in accordance with this Procedure.

Employees and contractors must follow this Procedure and comply with the requirements.
5. HSE Management Approach for Working at Height
Safe working at height shall be performed through the following steps:
1) Select the applicable methodology for Fall Protection System to be used for executing the work.
Prior to implementing this procedure, options to eliminate the need to work at height must have been
assessed. When eliminating is not possible, use option prevention or minimizing the fall impact.

2) Conduct Risk Assessments:

 Prepare JSA (to define safety requirement, personnel qualification, and necessary PPE). {See
section 6,7,8,9,10,11,12,13, Appendix B}

 Fill in the Working at Height Certificate (Appendix H) to be attached in Permit to Work.

3) Perform Worker Suitability Check prior to working at height. See Section 6.


4) Prepare Rescue Plan as required (Appendix D)
5) Proceed to Permit to Work
6) Prepare rescue facility as required as per Rescue Plan and prepare appropriate equipment for
working at height and equipment to minimize suspension trauma in the event of a fall refer to
Appendix D. Working at Height Rescue Plan.
7) Perform Work Activity
8) review the work at height plan for any improvement feedback gain from this experience and revise (If
any) the work at height plan for the next similar task.

6. General Requirement

All personnel working or have activity at height more than 6 ft (1.82m) or more from a solid surface or
ground, where a fall hazard exists must be equipped with fall protection system as follows:

 Fall Prevention System


 Permanently design handrail or approved temporary guardrail/handrail (work with in radius of
permanent design handrail or approved temporary guardrail/handrail and no holes may cause
fall off, fall arrest is not required). Or.

 Fall Limiter

 Full Body Harness with retractable winch, or


 Full Body Harness with adjusted length of Lanyard with or without Shock Absorber.

 Fall Arrest System


 Full Body Harness with retractable winch and shock absorber, or
 Full body harness with lanyard and shock absorber

The used of mesh screen, wooden or rope as fall prevention system is subject to risk assessment to
ensure fit for used.

Provision for hooking fall prevention system and fall arrest system:

1) Fall prevention system as fall limiter


Full body harness with retractable winch/full body harness with adjusted length of lanyard) shall be
hooked to any anchorage point (rated/non-rated, engineered/non- engineered) to prevent worker
from falling off.

2) Fall arrest system.


Full body harness with lanyard and shock absorber or without shock absorber shall be hooked to
anchorage (rated or engineered) point over the head or in any level as such to protect the
person from reaching the surface/ground (at minimum 1ft above ground) in the event of a
fall. See Appendix G – Fall Protection calculation for safe guideline to work at more than 6 feet
(1.82 meters).

See Appendix K Working at Height Working at Height Plan Process.

Where work at height could interact with persons working or passing below, suitable barriers shall be in
place to prevent injury from falling objects and walking into potential Line of Fire hazards.

Tools and equipment used while working at height must be properly secured from falling (i.e Toe Board
on Scaffolding, net, hand-tool leasing by rope, and/or hand-tool holder). The risk assessment should
define the proper securing tool from falling.

Work surfaces, including wall and deck openings on work platforms, must be properly protected to
prevent falls.

Where practical, a fixed or mobile work platform with handrails, mid-rails, and toe boards is used. A
competent person (Scaffolding Inspector or designated person) must approve these platforms.

Medical suitability check is required to be carried out for workers who will work on masts, towers and
other antenna bearing structures. The medical suitability guidance (Appendix I) shall be used as
reference by doctor/paramedic. Other than locations stated above, medical suitability check will be
conducted based on risk assessment result”

Example of potential hazards need to be identified:
 Physical hazard: heat stress, weather, temperature light, etc
 Psychosocial (human factor): tired/exhausted/fatigue, phobia, stress, etc

A buddy check must be carried out before performing the job.

7. Activities at Height
7.1 Passing by at Height
With Guar Rail
Each person who is standing by or passes through a gangway/bridge/platform at an elevated structure
6 feet (1.82 meters) or more above lower levels with guardrail/handrail systems, no full body harness
required. Minimum dimension (height of top rail and mid-rail) of guardrail shall meet scaffolding

standard. In adverse, if the Site Line Management or Supervisor consider that it is insufficient
or creates a greater hazard to execute the work, the fall protection system shall be applied
Without Guard Rail
Each person who standing by at or passes through a gangway/bridge/platform at an elevated structure
6 feet (1.82 meters) or more above lower levels without guardrail systems shall wear full body
harness hooked to engineered life-line (sling, pipe, rode, rope, et cetera) or engineered anchor point.
7.2 Erect / Dismantling Scaffolding
When someone is erecting or dismantling scaffolding, the person shall wear full body harness hooked
to fixed structure / engineered anchorage point / engineered calculated scaffolding tube / approved
life line or ladder safety refer to HSE Standards for Scaffolding and Ladder Safety Refer to
Appendix F1, F2, and G) for anchoring guideline.
A buddy check shall be carried out before erecting scaffolding.

In adverse, if the Site Line Management or Supervisor consider that it is insufficient or creates
a greater hazard to execute the work, the fall protection system shall be applied.
However, once part of worker’s body goes beyond the guard rail, full body harness shall be used and
hooked to any anchor point (i.e., rated or non-rated anchor point, engineered life- line (sling, pipe,
rode, rope, etc.) to prevent over the handrail. See Appendix F.1. and F.2. for reference when working
at height with scaffolding.

7.3 Working (Performing the Job) at Height


Climbing a ladder is not considered as working at height. Read more Ladder Safety, for ladder safety
practices. When working on a mid-ladder step without three point of contact (to prevent fall off), a fall
protection system must be applied.

7.3.1 In area with permanent/temporary guard rail on a fixed/permanent structure/platform.


When someone is working at height in area with permanent/temporary approved guard rail
systems on a fixed structure / platform available, no full body harness required. Permanent
guard rail system must comply with regulation & standard (i.e: Indonesia
Regulation/International Standard or subject to ris based assessment.

In adverse, if the Site Line Management or Supervisor consider that it is insufficient or


creates a greater hazard to execute the work, the fall protection system shall be applied.
However, once part of worker’s body goes beyond the guard rail, full body harness shall be
used and hooked to any anchor point (i.e., rated or non-rated anchor point, engineered life-
line (sling, pipe, rode, rope, etc.) to prevent over the handrail. See Appendix F.1. and F.2. for
reference when working at height with scaffolding

Fall protection devices shall be worn when meeting the following conditions:
 Risk based justification for using harness inside platform/scaffolding system.
 If some of body parts are exposed to fall hazard or over the side of the
platform/scaffolding system.

7.3.2 On Top of the Roof


Full body harness hooked to engineered anchor point / engineered lifeline is required.
7.3.3 On Top of Surface without Handrail (container, truck, open platform).

It is strongly recommended to find the method to eliminate working on top of surface without
handrail (i.e. using ladder, pre-installed tagline, etc.) otherwise:
 Full body harness with shock absorber must be hooked to engineered anchor point (If
applicable), or
 Full body harness with the use of lifeline, or
 Full body harness with retractable winch,
 The use of cantilever rolling ladder.

7.3.4 On a Fixed Ladder (height > 6m or 18 feet) with / without cage


 Full body harness with shock absorber must be hooked to engineered anchor point, or
 Full body harness with the use of man hoist riding, or
 Full body harness with retractable winch.

7.3.5 On a Temporary Approved Ladder attaching to a fixed structure


 Full body harness with shock absorber must be hooked to engineered anchor point, or
 Full body harness with the use of man hoist riding, or
 Full body harness with retractable winch

7.3.6 On a Temporary Approved Ladder attaching to a fixed structure.

 Full body harness with shock absorber must be hooked to engineered anchor point, or
 Full body harness with the use of man hoist riding, or
 Full body harness with retractable winch

7.4 Working at Height Using Special Equipment (e.g. cherry picker/aerial lift, gondola,
bosun chair/rope access, etc.).
Full body harness system attached to the anchor point as per manufacturer’s guidance.

Note:
Rope access/bosun chair are used when the use of scaffolding is impracticable and provide higher
risk (e.g. working at height on a tower, vessel hull, vent/flare stack, and large wall, and accessing to
drilling rig monkey board). This method is usually used to reach a difficult position while working at
height on a large surface (e.g. vessel hull) as such the use of scaffolding is impracticable and
provide higher risk.
7.5 Working Over/Near Water
Fall protection system shall be used or worn during working over/near water, i.e.: in deep water
condition, high waves, fast river flow, etc.
The following fall protection shall be applicable:
 Work vest style harness + lanyard with/without shock absorber, or
 Work vest style harness + retractable winch with/without shock absorber, or
 Life Work Vest + full body hardness and/or shock absorber with lanyard, or
 Life Work Vest + full body hardness and/or shock absorber with retractable winch
Work vest style harness, see Appendix J – Sample Pictures of Fall Arrest Equipment.
8. Emergency Response for Working at Height.
Any working at height activity shall provide rescue plan for the emergency situation.
 General rescue plan may be addressed in JSA when:
 normal access egress for rescuer is available and possible for manual stretcher maneuver
(e.g. availability of stairway, lift, gangway, bridge, ramp, permanent platform).
 Specific rescue plan
Additional specific evacuation is required when difficult situation/condition for rescue
activation exists, such as:

(e.g. limited access egress for rescue due to unavailability of normal access egress as
mentioned above
Condition that requires a descending device (e.g. rescue rope, pulley, etc.) as specific
rescue plan for work below but not limited to:

o Leading edge work (excluding scaffold work)


o Precast concrete erection work

o Roofing work
o Use of overhead crane hook for anchorage point

Any situation where anchorage points or common fall arrest solutions are not easily identified
or agreed during the pre-job planning meeting.

See Appendix D Working at Height Rescue Plan for specific rescue plan template.

9. Fall Arrest System

Fall Arrest System consists of:


 Full body harness with retractable winch and shock absorber if the height is equal to
5m or more from solid surface or ground (including tools tied with the person).
 OR full body harness with lanyard and shock absorber if the height is equal to 5m or more
from solid surface or ground (including tools tied with the person).

The fall arrest equipment and system includes these requirements:

 A responsible person has identified and provided sufficiently rated anchor point(s) for
continuous tie off, preferably above the worker’s head refer to Appendix D Anchorage Point
Rated Guideline.
 A full body harness with a D-ring attachment point is used.
 Synthetic lanyards with shock absorbers or fall limiter devices, all equipped with dual
action self-locking snap hooks at each connection, are used.
 Visual inspection of the fall arrest equipment and system is completed before each use,
and any equipment that is found to be damaged or has been activated is taken out of
service.

Note:

5m (15 ft) is taken from 18.5 ft – 6 ft = 12.5 ft (rounded up to 5 m) see figure in Appendix
G.

Fall limiter is required for less than 5m considering the total length of lanyard and
shock absorber which give total length of fall 18.5 ft (6m).

Double safety lanyard is required if worker needs to release a hook from anchorage point
to ensure worker‘s 100% tied off at all times (by attaching another hook before releasing
hook).
All fall protection equipment shall be used in accordance with the manufacturer's
specifications. Whenever work at heights is carried out and fall protection equipment is
being used, at least one person must be present nearby.

10. Lifeline

Lifeline shall be attached to personal fall-arrest system.


Lifeline shall be used in safe manner such as:
 All lifelines should be protected from abrasion caused by suspension over or movement
along a structural member.
 When installed horizontally, at a minimum, lifelines should be positioned above an
average-sized person’s waist level.
 When installed vertically, only one personal fall-arrest system shall be attached to a
single vertical lifeline.

 When installed as conjunction, only one personnel fall arrest system shall be attached to a
single conjunction lifeline.

11. Anchorage Point.

11.1 Rated Anchorage Point.


Rated Anchor Point to which personal fall arrest equipment is attached shall be capable of supporting
at least 5,000 pounds (f) / (22.2 kN or equal to 2.267 kg(f)) / 2500 Kg per worker attached. All
anchorage point rated must be clearly visible to understand the strength of the safe work load (SWL)
prior to the usage.
If there is no rated anchorage point available, then engineered anchor point is used.

11.2 Engineered Anchor Point


Engineered anchor point is an approved temporary anchor point. It is a part of complete personal
fall arrest system for a given load with a safety factor of at least two. See Appendix D for
Guideline of Engineered Anchored Rated Point and Appendix F.1 & F.2 for Guideline for
Scaffolding as Anchorage Point.

When Scaffolding is used as anchorage point, then its 1st level which has been erected must be
checked to ensure that the scaffolding installation is durable and fit for purpose, before the erection
continues to the next level. Ensure that the scaffolding is tagged prior to work execution.

12. Fall Protection Device Care and Maintenance


Fall protection device specification, care and maintenance, are governed in the Personal
Protective Equipment Procedure.
13. Training
All personnel who are required to perform works at height shall be trained. The training requirements are
stipulated in HSE Training Procedure, where they are trained to recognize hazards and able to use
applicable fall protection equipment.

14. Continuous Improvement Process.


Continuous improvement process is to periodically ensure the reliability and sustainability the use of
anchorage point, temporary handrail/guardrail, mesh screen, etc as applicable. It has to be
inspected/checked at least annual or risk based scheme.

Appendix B – Definitions
Anchor Point A secure point of attachment for lifelines,
lanyards, or deceleration
devices. An anchor point must be capable of
supporting at least
5,000 pounds (3,600 pounds if engineered/
certified by a qualified person) per person and
must be independent of any anchor point being
used to support or suspend platforms

Competent Person An individual who is capable of identifying


existing or potential hazards in the surroundings
or work areas and has the authority to take prompt
corrective action to eliminate such hazards
Complex fall-arrest harness Strong harness approved as fall-arrest harness,
safety belt and sitting harness. The harness has a
high fastening point in the front and at the back
and has three fastening points at hip height.
Double safety lanyard A safety line with two lines fastened between the
body harness's highest fastening point (in the
front or at the back) and the construction. A
safety line with fall absorber will limit the
sudden jerk of a fall to a maximum of 6 kN.

Fall Arrest System A system is used to minimize risk or impacts to


workers which are potential falling off of 6 feet or
more from elevated working area.

Fall Limiter A fall limiter is a device as self-reminder system


to worker, it is to prevent fall off from height:

Fall Protection System A system is to protect workers from falling off


of overhead platforms, elevated work stations or
into holes in the floor and walls and minimize the
impact if it occurs

Full body harness Webbing/straps which are secured about an


employee's body in a manner that will distribute
the fall arrest forces over the thighs, pelvis, waist,
chest, and shoulders. Having means for
attaching it to other components of a personal fall
arrest system, preferably at the shoulders and/or
middle of the back.

Fixed ladder A ladder that is permanently attached to a


structure, building, or other equipment

Guardrail A tube incorporated in a structure to prevent the


fall of a person from a platform or access way. The
tube is to be 36" - 45" above the deck

Handrail Tube used on stairs before permanent handrails,


banisters, etc., are fitted to prevent the fall of a
person
Ladder Safety Device A support system limiting an employee's drop
or fall from the ledder

Lanyard A flexible line of rope or strap that has self-


locking snap hook connectors at each end for
connecting to body harnesses, deceleration
devices, and anchor points.

Life Line A flexible line used to connect components of a


personal fall-arrest system to an anchorage

Man Hoist Riding A mechanical device that is used to manual pull


in or let out or otherwise adjust the "tension" of a
rope or wire rope

Mid rail A tube incorporated in a structure midway between


the guardrail and deck.
Portable ladder A ladder that is intended to be moved from one
job to another; stepladders and extension ladders
are examples of portable ladders
Retractable Winch A mechanical device that is used to automatic
adjusts the "tension" of a rope or wire rope.

Single fall-arrest harness Harness designed to prevent a fall (not to hang in)
with a high fastening point in the front or at the
back.

Safety strap A strap used to stabilize personnel at the


work site. Used in addition to the safety lanyard.

Shock absorbing lanyard A specific type of safety lanyard used in


conjunction with safety harnesses as a part of a
fall arrest system to reduce the impact from
falling. The unique feature of a shock absorbing
lanyard is that it has a built-in woven inner core
that expands during the fall to ensure that the fall
arrest force is significantly reduced
Toe board (Kick Board A low protective barrier that will prevent the fall of
materials and equipment to lower levels, usually 4
inches or greater in height
Working at Hight Working at any location more than above 6
feet (182 cm) and above the ground or base
working level or next lower floor level of multi-
floored buildings. Examples include, but are not
limited to; scaffolds, roofs of buildings/tanks,
mobile elevated work platforms with articulating
arms (JLG’s), etc
