Greetings!

I have the honor to inform your good office that MRS. HANNAH JOVANE E.
TIONGKO, a Social Science SHS Teacher II under permanent status, of this school will
be on MATERNITY LEAVE with pay effective March 11, 2024 to June 23, 2024.

In this regard may I request for the funding of a substitute teacher to handle her work
load while she is on leave effective March 11, 2024.

In line with this, may I recommend MR. ELMER DUANE R. TUGADE, a graduate of
Bachelor of Science in Hotel and Restaurant Management with earned teaching units
in Social Studies at ABE International Business College, Urdaneta City and a resident
of Brgy. San Blas, Villasis, Pangasinan. He belongs to the pool of Registry of Qualified
Applicants (RQA) S.Y. 2023-2024 with a final rating of 76.62 and with one (1) year
teaching experience in Lyceum Northern Luzon, Urdaneta City.

I believe that with the above qualification of Mr. Tugade, he will be best suited for the
substitute position.
