Madam:

This is to signify our intention to offer Senior High School (SHS) in Clemente Cristobal
National High School (CCNHS), Warey, Malasiqui, Pangasinan.

With this, a consultative meeting with the parents, Barangay Officials and students has
been conducted for the possible and particular track/strand to be offered. Majority of those who
were consulted agreed on General Academic Strand with Electives taken from the TVL. Early
Registrants start with ________ students.

Moreover, the school will undergo demolition for the its existing DepEd,Acuna and PTA
buildings for the Eight Classroom Four storey building that will come from the Central Office. A
major factor why the school did not offer SHS early on.

Your favorable action to this request will greatly impact development in the lives of the
townspeople especially the youth in this far-flung barangay of the Municipality of Malasiqui.
