Dear Doctor B,

Greetings!

One of the goals of the K to 12 Basic Education Program is to develop learners’ competencies,
work ethics and values relevant to pursuing further education and joining the world of work. To
achieve greater congruence between basic education and the nation’s development target, work
immersion, a required subject has been incorporated into the curriculum that will provide learners
with opportunities to become familiar with the workplace, apply the competencies in area of
specialization and for employment simulation.

To achieve the objectives, work immersion is thus a requirement for graduation for
secondary education. Learners will be immersed in actual work environment to enhance their skills
in Caregiving.

In this connection, the school would like to request permission that work immersion will be
conducted in different ward/department of your hospital for a total of ______ hours. For any suggested
day and time which are engaged in the field of specialization stated above

Attached herewith are the list of our 8 students, Memorandum of Agreement (MOA) and the
competencies they need to acquire in the duration of their immersion.

I am hoping for your favorable action regarding this request.
