An event, Year 12 Excursion Melbourne Cable Park Fern , has been planned for Thursday, 1 February 2024.

Year 12 students will be attending an excursion to support the start of the year, getting to know each other and their teachers,
challenging themselves and having fun! This will include water and climbing activities.
All who attend and participate in the Aqua Fun Park activities MUST be able to swim.

Aqua Fun Park and Climb High Ropes Courses are strenuous physical activities that require strength, stamina and fitness. By
signing this form, you acknowledge this and understand that injuries can occur and accept the risk of injury to your child.

By signing this form, you agree that students participating in the Aqua Fun Park:
- MUST be able to swim
- MUST wear a BUOYANCY vest at all times
- NEVER swim under the obstacles and ALWAYS go over the top of the surface. NEVER DIVE HEAD FIRST into the water - always
feet first.
- Follow all safety instructions as outlined by the Water Park including climbing safely, using the correct sliding position, no
pushing or rough play, use caution and respect the public and lifeguards on duty.
- ALWAYS follow the directions of the lifeguards and teaching staff

By signing this form, you agree that students participating in the Climb Melbourne:
- attend the MANDATORY safety breifing prior to commencing the activity
- MUST wear the safety harness at all time when participating
- ALWAYS follow the directions and instructions of the Melbourne Cable Park staff and teaching staff

Parent/Guardians and students, by signing this form, undertstand and agree to the conditions outlined above.
Parents/Guardians and students, by signing this form, agree to the school signing the Participation Waiver on thier behalf, and
relinquish important legal rights accordingly. Please refer to the full Participation Waiver details that the school will be signing on
your behalf in the attachment to this event.

Dress Code: Casual clothes, appropriate dress for water and climbing activities.
Transport: Bus - the school will arrange transport by busses to and from the venue.

Consent can be provided online through your school's parent portal (https://dandenong-hs-vic.compass.education), or alternatively
by returning the form below to House Leadership Team.

Normal classes will run for periods 1 and 2.

Busses will leave the school at Recess and return by the end of the school day.
Students can wear appropriate, casual clothes for water and climbing activities.
The excursion will be fully paid for by the school.

I give permission for Ajdin SEJFULAH to attend this event. Where the staff member in charge is unable to contact me, or where it is
impracticable to contact me, I authorise the staff member in charge to administer such first-aid as the staff member in charge judges to be
reasonably necessary. I understand that this is an official school event and that Ajdin SEJFULAH will adhere to the dress code, as outlined
above, and behave in alignment with the school's code of conduct. I agree to meet any medical expenses and/or transport costs incurred in
the event of sickness or injury. Further, I agree to meet any transport costs should my child be sent home as a result of misbehaviour or
inappropriate conduct.
