1. S&H MANUAL DESCRIPTION

1.1 Scope

This document defines the Safety & Health(S&H) requirements associated with the conduct of the
Contractor’s scope of work for “PROPOSED CONSTRUCTION AND COMPLETION OF THE SARAWAK
COASTAL ROAD, PACKAGE 04: PROPOSED CONSTRUCTION AND COMPLETION OF MUARA LASSA
BRIDGE,MUKAH DIVISION, SARAWAK” and applies to all site personnel and visitors.

1.2 Purpose

This document defines principal requirements for Health, Safety and Environment (S&H) matters
during the conduct of the works.

The objective is to ensure that adequate precautions are taken to avoid accidents, occupational
illness and harmful effects on the environment during construction. Also to assure that satisfactory
emergency planning is undertaken and adequate means are taken to secure contract objects.

This document:

-Details the processes by which PPK & SUBCONTRACTORS shall manage S&H while carrying out the
work under the Contract.

-Describes by reference the practices and procedures for S&H Performance.

4.2 Project Scope

The Works to be executed under this Contract include the construction, completion, testing and
commissioning of all requirements and structure as stated in the scope of works and specifications
including but not limited to all the necessary temporary works, site clearing, earthworks,
foundations, civil works and other relevant activities.

The scope of work comprises of but not limited to all the works below:

-Provision of bank guarantees, insurances, workmen compensation and SOCSO, survey and setting
out, site facilities arrangement, mobilization and demobilization of plant, testing, safety & health
care and all other necessary preliminaries and general;

-Demolition of existing obstruction and removal and disposal of the same;

-Excavation, filling, trimming, compaction and all necessary earthwork;

-Construction of concrete drain and all other drainage structures;

-Construction of structure element (beam, slab column);

-Construction of road pavement and road shoulders;

-Provision of pavement markings, traffic signs, guard rails and all other road furniture;

-Turfing, retaining walls and all other slope protection structures;

-Maintenance of existing road and provision of access for others where required;

-Piling works;

-All mechanical & electrical works;

-All architectural works;

-Locating temporary diversion, relocation, replacement of existing utility services, liaison with
appropriate authority or owner, obtaining approval, interphasing and all incidental works including
addition of necessary new services and testing required by the appropriate authority;

-Environmental Management and Mitigation Measures; and

-Any works incidental to and necessary for the completion of the specified works.

5.1 Manager level

 Ensure compliance to the Corporate S&H and site specific requirements.

 Facilitate communication of S&H issues from the workforce to management and vice
versa.

 Ensure company safety and environmental targets are being met.

 Ensure site personnel are aware of their duties and responsibilities in regard to safety
and the environment.

 Ensure training, resources, services, facilities and initiatives provided by the company are
being utilized as intended.

 Ensure hazards identified in the work area are immediately rectified and/or reported
immediately to the Client.

 Demonstrate leadership commitment through personal participation.

 Investigate all identified hazards, incidents, accidents and near misses, with guidance
from the Director.

 Perform S&H inspections of the work area at regular and agreed frequencies.

 Order operations to cease if he/she believes that S&H performance of Work is being
compromised and that ‘time out’ for S&H is warranted.

 Responsible for any tower crane safe work procedure at worksite

 Become the Chairman or Employer’s Representative in S&H Committee

 Participate in Emergency Response Team

5.2 Engineer level

 Ensure the work is complied to agreed Work Method Statement & S&H requirement

 Facilitate communication of S&H issues from the workforce to management and vice
versa.

 Ensure company safety and environmental targets are being met.

 Ensure site personnel are aware of their duties and responsibilities in regard to safety
and the environment.

5.3 Supervisory level

 Ensure the work is complied to agreed Work Method Statement & S&H requirement

 Facilitate communication of S&H issues from the workforce to management and vice
versa.

 Ensure site personnel are aware of their duties and responsibilities in regard to safety
and the environment.

 Lead daily toolbox meeting or pre work site inspection before performing work.

 Ensure hazards identified in the work area are immediately rectified and/or reported
immediately to the engineer level.

 Demonstrate leadership commitment through personal participation..

 Order operations to cease if he/she believes that S&H performance of Work is being
compromised and that ‘time out’ for S&H is warranted.

 Become the Employee’s Representative in S&H Committee

 Participate in Emergency Response Team

5.4 Site workers/Subcontractors level

 Ensure the work is done according Work Method Statement & S&H requirement

 Follow the S&H standards set by PPK at all time while working at PPK project site

 To wear PPE at all time while working at PPK project site .

5.5 S&H Personnel

 Ensure PPK & SUBCONTRACTORS compliance to legal requirement

 Ensure hazards identified in the work during inspection and audit are immediately
rectified and/or reported immediately to the manager level.

 Demonstrate commitment through leading.

 Order operations to cease if he/she believes that S&H performance of Work is being
compromised and that ‘time out’ for S&H is warranted.

 Become the Secretariat in S&H Committee

 Participate in Emergency Response Procedure

 Liaise with authorities whenever required

 Ensure that S&H Programs are being done according to S&H Plan.

6. LEGAL SITE S&H REQUIREMENT

6.1 Project Governing Law

The construction works shall be undertaken in accordance with all applicable legislation and
mandatory statutory requirements; but not limited to:

-Occupational Safety and Health Act and Regulation 1994 (Act 514) Regulation and
Orders;

-Factories and Machinery Act 1967 (Act 139) Regulations and Rules;

-Employment Act 1955, Regulation and Orders;

-Uniform Building By Law 1984 (Act 133);

-Construction Industry Development Board 1994 (Act 520);

-Electricity Regulation 1994 and Rules;

-Safety & Health Guideline and Code of Practice issued by relevant Authorities such as
DOSH, SOCSO and CIDB;

-Environmental Quality Act 1974; and all subsidiary legislations and any other authorities
with jurisdiction such as DOE, ILO, etc;

-Natural Resources and Environment Ordinance;

-Traffic Management During Construction Guidelines by JKR;

-Road Transport Act 1987; and

-Fire Services Act 1988.

6.2 Provision of Competent Person

PPK & SUBCONTRACTORS are aware that they are required to provide competent person as stated
by law but not limited to; such as:

Safety & Health Officer For project value > 20 millions PPK & SUBCONTRACTORS
Site Safety Supervisor For every project PPK & SUBCONTRACTORS
Scaffold Inspector For scaffolding installation PPK / SUBCONTRACTORS
Scaffold Installer For scaffolding installation PPK / SUBCONTRACTORS
Crane Operator For crane activities PPK / SUBCONTRACTORS
Lifting Supervisor For tower crane activities PPK / SUBCONTRACTORS
Rigger For tower crane activities PPK / SUBCONTRACTORS
Signalman For tower crane activities PPK / SUBCONTRACTORS

PPK & SUBCONTRACTORS are also required to provide their own workers with valid CIDB green
cards under CIDB requirement and valid working permit for non Malaysian workers under
Immigration requirement. The working permit location shall be at Matu/Daro , any foreign workers
who have their work location differs from their current work location must have proper
endorsement letter from Immigration Department where their work permit was issued at the first
place.

6.3 DOSH Worksite Registration & Hoarding

PPK are aware that the site shall have to be notified to DOSH not later than seven days from the
commencement of construction activities but if the project can be finished within 6 weeks, therefore
it shall be exempted from the requirement.

The worksite shall be fully barricaded by protective hoarding so that public can be protected from
work in progress. The minimum height of a hoarding shall be 1.8m and built continuously until the
ground. The entrance to and exit from worksite should be located in such a manner as to prevent
danger and inconveniences to the public. Proper security should be maintained so as to prevent
entry of unauthorized personnel into the worksite. All building material shall be put inside hoarded
area. Gate shall be constructed with suitable design and strength for traffic movement.

6.4 S&H Reporting

6.4.1 Notification of Accident, Dangerous Occurrence, Occupational Poisoning & Occupational


Disease (NADOOPOD)

PPK & SUBCONTRACTORS shall ensure that once incident take place in the project site, all staffs are
responsible to report the Incident to Emergency Response Team, Project S&H Team and Project
Manager immediately.

The Project Manager shall convene an emergency meeting to discuss the accident/incident with
assistance of S&H Committee members. The S&H committee shall conduct an investigation and
report the accident / incident within 24 hours. Upon identified root cause. the immediate corrective
measures shall be taken to prevent a similar accident from further recurrence.

For any major accidents eg. fatality, permanent disability, search and rescue (SAR) and others shall
be reported to nearest related agencies eg. Police Station, Bomba, DOSH and SOSCO, within 24
hours.

All the occupational accident, disease and poisoning shall be reported to DOSH in accordance to
requirement stipulate in Notification Of Accident, Dangerous Occurrence, Occupational Poisoning
And Occupational Disease Regulation by using Form JKKP 6 and JKKP 7. Annual return for the Health
and Safety record will be using Form JKKP8, JKKP9 and JKKP10.

6.4.2 High Risk Activity Reporting & SHO Achievement Indicator

DOSH has imposed to every construction SHO to report high risk activity and their site achievement
using online form.

6.4.3 Client & HQ S&H Reporting

SHO shall furnish monthly S&H report to be submitted to the client and headquarters.

6.5 Emergency Response Preparedness

In relation with “provision of safe work arrangement” & “provision of safe access & egress”, PPK has
already prepared for emergency response at site. Refer to Emergency Response Plan.

6.6 Provision of Welfare

PPK & SUBCONTRACTORS shall provide such welfare as far as practicable to their workers but not
restricted to:

-Workers’ camp

-Sanitary facilities

-Canteen/Cooking facilities

-Seating facilities

-Washing facilities

-Lighting

-Safe arrangement for access and egress from workers’ camp to worksite

6.7 Mosquito Breeding Prevention

PPK & SUBCONTRACTORS are aware that every construction site has the risk for mosquito breeding
which shall increase the probability of dengue fever case at the worksite or the community area.
Therefore they are committed in preventing such cases by:

-Housekeeping

-Larvaciding

-Fogging

6.8 CIDB Green Card & Site Supervisors/Skill Workers Accreditation

PPK & SUBCONTRACTORS are aware that every construction site is governed by CIDB Act 520 while
commencing their tasks. Thus, they shall commit to the requirement of CIDB Green Card and
Accreditation. No personnel without CIDB green card shall be allowed to work at the worksite. Both
PPK & SUBCONTRACTORS shall ensure the maintenance of CIDB Green Cards for their each
respective employee. MAIN SUBCONTRACTORS of PPK shall ensure that OTHER SUBCONTRACTORS
hired, shall apply the same responsibility. PPK’s site personnel shall have the power to remove any
workers without valid CIDB Green Card from working at the site as per instruction of PPK’s Project
Manager.

7. HAZARD MANAGEMENT

PPK is committed and shall ensure SUBCONTRATORS commitment to reduce the number of accident
at project site by using hazard management technique, Hazard Identification, Risk Assessment &
Risk Control (HIRARC), as per DOSH’s guideline. Common hazards involved at worksite are such as
below but not limited to:

- Suspended load
- Drowning
- Slip & Trip
- Cutting and Pinch point
- Electrical
- Hazardous Chemical or Substances
- Noise
- Ergonomics
- Working at night or dark condition
- Working in adverse weather condition
- Working in trenches & confined space
- Working at height
- Crane Lifting

7.1 Suspended load

PPK & SUBCONTRACTORS objects shall be used above the ground floor or any structure unless such
floor or structure has been so designed and constructed as to support the load imposed by the
object.

No objects shall be left suspended above any personnel or property, at any circumstances. Every
activity that includes such hazard shall be barricaded to avoid any unauthorized admittance.

The installation and usage of cantilever form traveller shall also be considered as hazard related to
suspended load. Therefore, the installation & dismantling of cantilever form traveller shall only be
done by trained and experienced work or subcontractor to avoid falling or collapse of the form
traveller. The installation of form traveller shall be supported by using scaffold system or chain block
or crane lifting wherever and whenever required.

7.2 Drowning

Where employees or visitors are exposed to such hazard there shall be provided by PPK &
SUBCONTRACTORS at all times during the exposure, adequate personal floating device (PFD).

No personnel shall be allowed to work near or above water unless they attended S&H Induction
Briefing, being informed with ERP and confirmed to PPK regarding their ability to swim.

While working near or above water, all personnel shall wear their PFD at all time. Where working
platform is provided for such work, adequate railings shall be installed to prevent falling into the
water.

7.3 Slip &trip

PPK & SUBCONTRACTORS shall not allow any employee to use any elevated structure which is in
slippery or oily condition unless the surface is cleaned, dried, sanded or covered.

All passageways shall be kept free from accumulations of any form of obstructions that could cause
tripping.

7.4 Cutting & Pinch point

PPK & SUBCONTRACTORS shall ensure any vertical or horizontal projections; edges or tools which
could cause cut shall be removed or otherwise made safe.

Every pinch point shall be identified and informed to employees. Where machine guarding is
applicable to prevent both cutting and pinch point hazards, such guarding shall be installed and shall
not be removed by anyone. Where machine guarding is not applicable, proper information and
supervision should be provided to employees.

For most applications machine guarding is an engineering control method that is the best of several
of available options for protecting personnel working around machinery and equipment. The
installation of machinery and machine guards is a governing factor in controlling and preventing
accidents and injuries. The selection of a guarding method to be used if the machine does not have a
manufacturer's installed guard may depend on a number of things such as space limitations; size of
stock, and frequency of use. The following general guidelines are provided to assist in that selection.

Design and construction characteristics of machine guards include:

-The guard must be considered a permanent part of the machine or equipment.

-The guard must afford positive protection. Personnel should not be able to reach a
hazard by reaching into, over, under, or through a properly designed and installed guard.

-The guard must prevent access to the danger zone during operation of the equipment.

-The guard must be as convenient as possible and must not interfere with normal
operation of the machine or maintenance functions. This may include hinging guards to
allow for access, using drift pins, latches, or minimizing the number of cumbersome
attachments.

-The guard should be designed for the specific job and specific machine, with provisions
for lubricating, inspecting, adjusting, and repairing the machine.

-The guard must be durable and constructed strongly enough to resist normal wear.

-The guard must not present a hazard in itself.

-The guard should not be easily bypassed or defeated. The use of "dead man" controls is
the preferred method because if the safety device fails or is bypassed, the machine will
not present a hazard to personnel.

7.5 Electrical

PPK & SUBCONTRACTORS shall ensure any electrical equipment, source or cables are safe to be used
and not exposed to wet condition either from weather, personnel, machines or the electrical
equipment itself. In accordance, no looping of electrical cables over nails or brackets or scaffolding

shall be allowed. Any electrical materials, wiring, temporary or permanent, accompanied with ELCB
& proper earthing, shall be installed and check by authorized personnel according to Sarawak Energy
Sdn. Bhd. guidelines.

PPK & SUBCONTRACTORS shall post and maintain proper warning signs where such hazards exist at
their own respected worksite.

PPK & SUBCONTRACTORS shall ensure that their employees or visitors are well protected from such
hazard by providing necessary PPE & arrangements. All elevated power lines shall have sufficient
vertical clearance for large and heavy vehicles and shall not be lowered than 6 metres from ground
surface.

PPK & SUBCONTRACTORS shall liaise with CLIENT, CONSULTANTS or LOCAL AUTHORITIES whenever
their works interrupt the power supplies for local community or having doubt on risk of
electrocution.

All electrically powered portable tools shall be inspected prior to use. Tools with frayed cords,
broken ground prongs, etc are not to be used. All portable electrical powered tools shall have
insulation and ground tests done on a regular basis. Conductivity tests shall be performed on all
electrical protective equipment at prescribed intervals. This protective shall include rubber insulating
gloves, line hoses and covers, and nonconductive matting

Working on exposed circuit/parts that are energized will not be permitted unless the workers are
qualified and trained to do so. Safety related work practices shall be used to prevent electric shock or
other electrically induced injuries. Qualified workers are those who have been trained to work safely
on energized circuits and, when appropriate, to use the proper personal protective equipment,
insulating and shielding materials, and insulated tools. Two workers, one of whom must be trained in
CPR, shall work together any time work must be performed on high voltage energized circuitry or
equipment.

7.6 Hazardous Chemical & Substances

PPK & SUBCONTRACTORS shall ensure that dust control is in place to prevent such hazard to injure
health or obstruct vision or causing dirty roads or to exceed safe exposure levels. In order to do so,
provision of dust mask, vehicle tyre cleaning drive through or ground wetting shall be done.

PPK & SUBCONTRACTORS shall ensure that such chemicals or substances are stored properly in a
way that shall not impose any safety or health hazards to employee or visitors or environment. Any
chemical stored shall be recorded and related safety data sheet for every chemicals or substances
shall be provided. This also includes provision of relevant PPE and storage in order to safe handle
such chemicals.

As long as the chemicals are still used at site, they shall be treated as hazardous chemicals under
USECHH and shall be handled according to CPL.

7.7 Noise

PPK & SUBCONTRACTORS are aware that their operations shall exposed their employees and non
employees to loud noise. Therefore such operations shall proceed in accordance to FMA 1967, Noise
Exposure Regulation 1989 to ensure the noise are kept at minimum level.

7.8 Ergonomics

Ergonomic hazard is an unseen hazard where it’s effect is chronic, which can only be seen in future.
This includes repetitive work movement, vibration, lighting, work space, manual lifting and etc.

PPK & SUBCONTRACTORS shall as far as practicable to reduce the long term effect due to ergonomic
hazard by creating awareness to their employees and reducing their time exposure to such hazard.

7.9 Working at Night or During Dark Condition

PPK & SUBCONTRACTORS shall provide sufficient light for the work areas. The bottom line for
lighting is simple, the contractor must provide lighting for all operations, and no exceptions are to be
made. Any contractor personnel working outside the lights will be directed to return to a lighted area
or the operation shall be stopped.

The equipment or vehicle must have signage or additional safety lighting (such as a rotating beacon).
All contractor work vehicles, this includes heavy equipment, backhoes, trenching machines, etc. are
required to have two working headlights and taillights, respectively. Vehicles without appropriate
lighting should be kept from working at night.

Good illumination shall be provided in all walking, working, and service areas and for all difficult
seeing tasks to ensure the safety of all personnel in or at the work environment. There shall be good
direction and uniform distribution of illumination. Night working session must be provided with
adequate lighting system to ensure the safety and visibility of the workers.

7.10 Working in Adverse Weather Condition

The construction of bridge and roads are often exposed to weather changes, whereby such changes
shall affect the work operation.

PPK & SUBCONTRACTORS shall not allow any high risk works such as lifting and working at height
during storm conditions due to low visibility, swaying loads and slippery hazards which can arise
from such conditions. “STOP WORK” shall be initiated for such situations.

PPK & SUBCONTRACTORS shall also ensure that their workers are not exposed to extreme hot
weather as it can lead to dehydration, heat cramps and faint among workers. Proper roofing facilities
shall be provided to employees working in office and moveable tent for site workers. The workers
are also allowed to build their own tent using the site materials.

7.11 Working in Trenches & Confined Space

The similarity of working in trenches & confined space is the lack of access and egress which will lead
to delay of emergency response. PPK & SUBCONTRACTORS shall ensure such provision of proper
ladder at each worksite.

For trenches, soil movements are unpredictable and usually cause personnel or property to be
buried or topple. PPK & SUBCONTRACTORS shall ensure necessary wall retaining structure is build to
restrain the soil movements.

For confined space, hazardous gases are the most dangerous part which usually lead to chain of
death. Basic practice such as removing the hazardous gaseous using exhaust ventilator shall be used
widely at site. No personnel shall work inside confined space unless they are trained and fit to work.

7.12 Working at height

Any objects that could fall shall be secured with any kind of straps or being provided with proper
storage. For works that involved risk of falling, PPK & SUBCONTRACTORS shall provide safety
harness, safe working platforms and ladders, railings and any related ways or arrangements to
prevent such fall.

All personnel involved in working at height shall be trained before performing their tasks. For work
more than 3 metres, it is compulsory for those working at height to wear their safety harnesses.
Safety helmet shall be secured properly by tightening headband and chinstrap to avoid falling
objects. No objects shall be thrown upwards or downwards, as for material transferring, only crane
lifting or pulley system.

No personnel who work at height shall be allowed to work alone and those who work at height shall
be fit physically and mentally to work.

7.13 Crane lifting

PPK & SUBCONTRACTORS shall ensure that all crane lifting operations are done by competent
operators by using cranes with valid Permit Mesin Angkat (PMA). Any tower crane installation shall
be done by DOSH competent firm.

Lifting plan is needed for non routine lifting. A lifting plan is planned by a Lifting Supervisor and shall
consist of risk assessment, load information, crane information, lifting equipment information &
onsite sketches. Basic lifting safety is that the load must not exceed the crane safe working load
(SWL). Other contributing factors are such as weather factor, load centre of gravity, fitness of lifting
equipments, rigging and slinging styles and ground conditions. For lifting above water, the stability of
working barge or floating working platform should be taken into consideration.

A basic lifting team shall include Lifting Supervisors, Crane Operators, Riggers and Signalman. All
lifting area shall be barricaded and post with proper warning signage. No personnel is allowed to be
under suspended loads. Crane and lifting equipments should be checked at least once per month by
crane operator and S&H personnel. PMA shall be renewed every 15 months and join inspection with
DOSH Officer is compulsory. During inspection, details of crane and operator together with

inspection and maintenance record shall be provided to DOSH Officer. Maintenance of crane should
be done at least twice per PMA validity period.

PPK & SUBCONTRACTORS shall ensure that provisions of such arrangements are made applicable on
site. Project Manager are responsible for the implementation of safe tower crane lifting procedure
at site according to DOSH Chief Inspector Special Order on June 2017 which consist of requirements
regarding:

a) Appointment of DOSH Competent Tower Crane Operator


b) Appointment of Lifting Supervisor, Signalman and Rigger.
c) Execution of Permit to Work
d) Maintenance of lifting gears
e) Maintenance of tower crane’s safety devices
f) Provision of records regarding usage, inspection, maintenance and PTW
g) Appointment of DOSH Competent Tower Crane Installer Firm

7.14 Concreting

PPK & SUBCONTRACTORS shall ensure that formworks and reshores shall be certified structurally by
PE and shall be properly braced or tied together so as to maintain position and shape. Where the
formwork structure is two or more tiers it shall have sufficient cat walks and other secure access for
inspection purposes.

Inspection and supervision of formwork shall be done by designated person or concreting work
supervisors to ensure that concreting, formwork and falsework are done properly.

Stripping of formworks can only be done until the concrete is fully set according to specifications. All
protruding nails, wire ties and other form of accessories not necessary to subsequent of work shall
be pulled, cut or otherwise made safe.

7.15 Scaffolding

PPK & SUBCONTRACTORS shall ensure that every wooden and metal parts of scaffolding installed at
site is strong and free from corrosion. Metal tube & metal frame scaffolding shall comply with BS
1139.

PPK & SUBCONTRACTORS shall ensure that scaffold can only be installed after the designed is
approved accordingly. For metal tube scaffolds structure exceeding 40m shall be approved by

Professional Engineer and for other scaffold exceeding 15m shall be approved by DOSH.
Nevertheless, all approved designs shall be made copy and submitted to DOSH.

PPK & SUBCONTRACTORS shall appoint a Competent Scaffold Inspector to inspect and supervise the
scaffold work. Scaffold installation shall be done by Competent Scaffolder . No incompetent shall be
allowed to install or remove or changing of any parts of the scaffold structure. Inspection of scaffold
shall be done every week or every after bad weather or every changes made to previous structure.
Scaffold tagging can only be done by Scaffold Inspector and shall not be tempered by anybody. There
are certain design of scaffolding that can only be installed by certain level of competent scaffolder,

Basic scaffold safety is that it shall be installed according to PE approved design, by competent
person, inspected by Scaffold Inspector during installation, maintenance & dissembles. The structure
should rest on firm flat base, where its tube or frame is properly interconnected with proper joint or
clamp & strengthen with bracings or wall tie. All walkways shall be made using strong material and
every open edges shall be closed using railings. Proper climb ladder or step ladder shall be provided
as access and egress, where it shall be kept clear from any kind of obstructions. No heavy object shall
be stored on the scaffold structure unless it is designed to withstand double or triple weight of the
load as a safety factor.

7.16 Excavation

PPK & SUBCONTRACTORS shall ensure that any public shall be kept away from the site by
constructing barricade or hoardings. If the activity must proceed near the public area, proper traffic
control shall be provided including the personnel and signage. Where there is doubt for buried
electrical cables and piping, the team shall consult with clients or consultants or local authorities
before any excavation can be started.

The excavation site shall be monitored by designated person after every rainstorm or other hazard
increasing occurrence, to prevent landslides. Temporary sheet piling shall be installed until a proper
retaining wall has been built and reach its full strength.

Any loads shall be kept away for at least 1 metre from any open excavation and trenches to avoid
additional load impose to the ground. For excavation sites that may involve with falling from height
risk, proper barricade or railings and signage shall be installed along the trenches. For excavation
more than 1 metre depth, shall be provided with access and egress.

No employee shall be allowed to work alone in deep trenches and all excavation shall stop during
rainstorm and until the designated person confirmed the work site is safe.

7.17 Compressed Air

PPK & SUBCONTRACTORS shall ensure that all compressed air equipment must be fit for work and
registered with DOSH for PMT or DOE for Safe Air Effluence before used on site.

Compressed air must not be used for cleaning purposes when vacuum cleaners or something safer
will do the job. If not practicable or possible to use other means, compressed air may be used for
cleaning when reduced to less than 30 psi and then only with eye protection and safe guarding of
other personnel in the area. The use of compressed air for cleaning clothing or any part of the body
at any pressure is prohibited.

Every connection of hose shall be connected firmly and to be cable tied or safety pinned, not to be
left on the ground without warnings where it can become tripping hazard. No damaged air pressure
hose shall be use at site. Fire extinguisher shall be put 1 meter away in case of fire emergency. A drip
pan shall be put beneath the machine to prevent soil contamination whenever there is oil leakage.

For unfired pressure vessel, there are criteria which DOSH exempted from having design approved or
having PMT which are:

- UPV which is a component of a rotating or reciprocating mechanical device which function is
to support the operation

- UPV which is a component of a chiller package for a building.

7.18 Ladder

PPK & SUBCONTRACTORS shall ensure that ladders will be inspected by the authorized person prior
to each use. Ladders with broken or missing rungs, broken or split side rails, or with other faulty
parts shall not be used. Do not place a ladder in front of a door which opens toward the ladder
unless the door is locked or otherwise blocked, barricaded, or guarded. No one shall go up or down a
ladder without the free use of both hands. If handling material, a rope shall be used to raise or lower
the material.

Portable ladders placed against a wall or other fixed object shall be securely fastened or held by a co-
worker to prevent slippage. The ladder's base should be placed at a distance from the vertical wall
equal to one-fourth the working length of the ladder in order to assure the proper angle. No ladder
shall be used to gain access to a roof unless the top of the ladder extends at least three feet above
the point of support, at eave, gutter, or roof line. Ladders shall not be placed on boxes, barrels, or
similar unstable bases to obtain additional height. When using portable ladders on smooth floors or
sloping surfaces they shall be equipped with nonslip bases. No wooden ladders are allowed to be use
at site especially outdoor area.

7.19 Welding

PPK & SUBCONTRACTORS shall ensure that all welding activities shall be done by trained and/or
experienced personnel.

Compressed gas cylinders used for the task shall be stored in an upright position at all times and
secured to prevent toppling. The protective caps shall be installed on all cylinders not in use.
Cylinders shall not be handled, shipped, or stored without valve protection caps. Compressed gas
cylinders must be kept away from excessive heat (125 oF, 51.5oC) and shall not be placed where they
can come in contact with an electrical circuit. Cylinders shall not be stored within twenty feet of
highly combustible material. Oxygen and oxidizing gases shall be separated from flammable gases by
a minimum of twenty feet or by a noncombustible barrier at least five feet high.

During task, all connection shall be properly secured and tightened and shall not be left on the
ground where it can induce tripping to anyone. Flashback arrestor and gauge shall be installed and
maintained properly as long as the task proceeds. No flammable or explosive substance shall be
stored near the welding area. Specific face shield and hand gloves should be worn while welding.

7.20 Driving

PPK & SUBCONTRACTORS shall ensure that all vehicles and driver shall comply with Jabatan
Pengangkutan Jalan requirement, basically the driver license, road tax, vehicle fitness. Those heavy
machinery that can moves by its own, the operator shall have certain competency from related
agency such as CIDB.

All drivers shall obey the speed limit at every location

Although the speed limit is there, some factors should be taken into account to lower down the

- Drivers’ awareness ; No personnel shall continue to drive if fatigue or drunk
- Vehicle fitness such as wheel condition, brake condition etc.

PPK & SUBCONTRACTORS shall practice good driving habits such as no phone while driving, 2 points
contacts on steering wheel, wearing safety belts for both driver and passengers, keep a car distance
from front vehicle, signalling before changing lane or overtaking, safe and correct overtaking, side
and rear mirror observations and switching on car lights. No unsecured or moving items shall be put
inside the driver-passenger compartment to avoid flying objects during emergency break or accident.

7.21 Permit To Work

PPK & SUBCONTRACTORS shall practice the Permit to Work for high risk jobs that include working at
height, scaffolding, concreting, excavation, lifting & hot work. Each PTW shall be given a specific time
frame and shall be valid until it is handover. Each PTW shall be attached with Job Hazard Analysis of
the high risk work being done. PTW shall be displayed at worksite during working at site.

8. Personal protective equipment

Whenever possible, processes, equipment, and environments should be designed and arranged so as
to prevent or minimize hazardous exposure to personnel. When this is not feasible, approved
personal protective equipment (PPE) will be provided, and individuals will be instructed in its proper
use and limitations. Once personal protective equipment is provided, its use must be enforced by
supervisory personnel. Only approved, properly fitted personal protective equipment will be used.
The S&H Department will provide assistance in determining the type of personal equipment required
in any working area. Personal protective equipment will not be used in hazardous areas or where
hazardous conditions exist until it has been determined that the individual can wear the equipment
without endangering his/her health.

The supervisor of each work center shall be responsible for determining the quantity of personal
protective equipment required and for control of such equipment issued to its personnel.
Supervisory personnel are responsible for training their personnel in the selection, use, inspection,
and care of personal protective equipment required for their unique work situations, and for
maintaining records of such training. Proper equipment storage must be provided to protect against
environmental conditions which might degrade the effectiveness of the equipment or result in
contamination during storage.

The Personnel Protection Program consists of the following areas: Respiratory Protection, Protective
Headgear, Sight Conservation, Hearing Conservation, Hand Protection, Foot Protection and Floating
Devices as outlined in the following sections. In depth information on each program can be obtained
from the S&H Department. The S&H Officer need to obtain the updated list of approved PPE from

8.1 Respiratory Protection

The Respiratory Protection Program is a system whereby respirators will be properly selected, used,
and maintained in a manner which will protect employees from hazardous and oxygen deficient
atmospheres in the workplace.

For employees who will be required to work in respirators in conjunction with work and/or research
activities, the S&H Department will provide additional instruction and qualitative fit testing as
prescribed by current OSHA regulations.

8.2 Protective Headwear

Approved protective headwear (hard hats) shall be worn by all personnel working where there is
danger of head injury from impact, falling or flying objects, or electrical shock and burns. Protective
headwear shall also be worn by operators of materials handling equipment, personnel engaged in
construction work, warehousing, personnel working in excavations, and all personnel using pickaxes,
sledge hammers, and chain

All approved hard hats must meet the specifications of the SIRIM, Malaysia. Metal or light plastic
"bump caps" do not fit in the category of approved hard hats and therefore shall not take the place
of such equipment saws. Head protection worn by personnel exposed to electrical shock and burn
hazards shall be specifically approved for electrical work hazards. Work center heads/supervisors
must ensure that all personnel who work in "Hard Hat Areas" or in occupations which require
protective headwear are issued a properly fitting, nonmetallic safety hard hat in accordance with the
provisions of this section.

It is the user's responsibility to ensure that the suspension straps are properly installed and that the
hat fits properly and is free of cracks, holes, or other imperfections which will negate its protective
qualities. Since the effectiveness of hard hats depends upon their structural strength, no ventilation
holes, openings, cutting of brims, or other alterations are to be made to any hard hat.

8.3 Sight Conservation / Eyes Protection

Personnel engaged in eye hazardous work or research shall be provided with the appropriate type of
eye protective equipment in consonance with the assigned task. Eye hazardous operations are
defined as any task, assignment, or operation that presents danger, however remote, to the eyes of

the worker or those individuals within ten feet of impact, loose foreign bodies, glare, chemical
infiltration, injurious radiation, or any combination of the above.

Eye hazardous areas are defined as those in which there are sufficient overlapping eye hazardous
operations to present a general risk of injury to personnel entering or working therein. Anyone
entering an eye hazardous area must wear approved eye protection at all times.

The type of eye protection for each work center will be determined by the work center supervisor
based on the type of protection required. In general, the following types can be considered for use:

8.3.1 General Use - Spectacles with side shields for operations in which foreign bodies may be
expected to come from the side of the worker or where personnel engaged in eye hazardous work
are working closely together.

8.3.2 Corrective Eyewear - Most work centers would find it cost prohibitive to order safety
spectacles with optically corrective lenses. Cover goggles which can be worn over the workers
regular spectacles should be considered.

8.3.3 Welding, Cutting, and Brazing - These operations require special filtered lenses of varying
shades dependent upon the type of work being performed.

8.3.4 Banding and Unbanding - Eyes can be protected by using approved goggles or safety glasses
with full-cup side shields. An approved face shield is also available to provide face protection when
used in conjunction with appropriate eyewear.

8.3.5 Impact Hazard Goggles and Spectacles - Eyecup goggles with glass lenses or plastic enclosed
eye-shields are appropriate for heavy impact and large particles, such as in chipping, caulking,
sledging, etc. Spectacle goggles may have leather, plastic, or wire mesh side shields that can not
easily be removed and are appropriate for moderate impact and small particles such as in scaling,
grinding, woodworking, machine operation, and general work. One piece plastic eye shields, with or
without side shields, are for moderate impact and sparks.

8.3.6 Impact and Chemical Face Shields - Shields, curved to cover the face area, provide general
protection from flying particles, sprays, sparks, etc. Goggles may be worn underneath when the eyes
require more positive protection. Nonconductive types are made without metal for use by electrical
workers.

8.3.7 Harmful Liquids and Fine Dusts - Chemical hazard goggles shall be used for work involving
chemicals and for any operation which produces fine dust or hazardous and irritating liquids. In
conditions where chemicals are concentrated or where splash hazard are continuous, the goggles
should be worn under a face shield for greater protection.

It shall be the responsibility of the individual and the work center supervisor to ensure that personal
protective eyewear is maintained in a clean and fully operational condition, and that it is used while
performing eye hazardous operations.

8.4 Hearing Conservation

Noise is one of the most pervasive occupational health problems. Exposure to high levels of noises
causes temporary or permanent hearing loss and may cause other harmful health effects as well.

In essence, this means that personnel exposed to noise levels above 90 dba for the duration of an 8
hour work day or to impulse or impact peak levels exceeding 140 dba shall be provided with hearing
protection devices. Either circumcural hearing protectors (earmuffs) or aural insert hearing
protectors (ear plugs) may be used. Plain cotton is not an acceptable hearing protection device.

8.5 Hand Protection

Hand protection shall be provided to all personnel working in operations involving heated or frigid
materials, petroleum products, acids, caustics, or other chemicals that are hazardous when in
contact with the skin, live electrical circuits, abrasive work, or when handling materials with sharp
edges. Other hand protection may be supplied at the discretion of the work center supervisor for
work involving the handling of contaminants, materials which are splintered or extremely rough
surfaced, or for other special work needs.

8.6 Foot protection

Foot hazardous operations are those which have a high incidence or potential for foot or toe injuries.
Examples of work generally associated with a high incidence of foot injuries are construction,
material handling, warehousing, maintenance, pier operations, vessel operations, transportation,
and grounds work. Safety shoes with built-in protective toe box are primarily intended to provide
protection from heavy falling objects and must be labeled accordingly.

Thus going "barefoot," or using open-toed shoes, sandals, or "flip flops" is expressly prohibited in
PPK Construction area. All personnel involved in outdoor work must wear footwear which offers
protection from lacerating and penetrating injuries as might be caused by broken glass, nails, or
shells.

8.7 Personal floating device (pfd)

Accidental drowning can usually be linked to one or more of the following factors

- Failure to provide or wear PFD


- Failure to wear PFD correctly
- Disregard or miscalculate of a hazard
- Lack of supervision
- Inability to cope or calm during drowning
- Absence of rescuers and rescue equipments

There are 5 types of PFD but for an amphibious work, it is recommended to choose only three kinds
of PFD which are Life Jacket, Work Vest & Ring Life Buoy.

Life jacket has better time rate buoyancy than work vest due to the size of buoyant material used to
make the life jacket, but, in term of usage while working, work vest is better than life jacket, as it
does not restrict the wearer’s body movement so much rather than life jacket. Ring life buoy is to be
thrown to drowning personnel in order to keep him floating a little bit longer for marine rescue by
boat. Life jacket should be worn before travelling above water and work vest should be worn while
working at edge and above water.

PPK & SUBCONTRACTORS shall provide such PPE to their own workers respectively, regardless
whether it is mentioned or not mentioned in the CLIENT-MAINSUBCON or MAINSUBCON-SUBCON
contractual agreement, as this is a legal requirement from OSHA 1994 under EMPLOYER’S
RESPONSIBILITY and the cost shall not be imposed to the employees.

All site personnel under CLIENT-CONSULTANT-MAINCON-SUBCON or visitors shall comply to PPE


requirement at work site.

9. S&H COMMUNICATION

9.1 S&H Committees

PPK will facilitate the effective operation of a Site S&H Committee conducted in compliance, once
every three months with relevant regulatory requirements. A safety and health committee shall be
establish at work if there are 40 or more persons employed at the place of work. All contractor need
to appoint employer and employee representative for the committee.

9.2 S&H Committee Meetings

The S&H Committee meeting will be conducted on quarterly basis. The meetings shall be minute.
These meetings shall:

 To assist the project to implement health, safety and emergency response programs at
the site.

 To review the effectiveness of S&H programs and to make recommendation, suggestion


to the project management for improvement.

 To make recommendation to the project management on corrective actions to be taken


whenever unsafe practices or conditions are discovered or uncovered after an accident.
Such findings and recommendations are to be recorded in the accident investigation
report.

 Review progress on incidents, near misses and hazards that have been reported.

 Identity and agree implementation of any lessons, equipment or operational changes


which may arise from incident or hazard reports.

 To conduct periodical site health and safety inspections, at least once a month, to ensure
that all work sites are complying with laid down safety requirement. If unsafe practices
and conditions are observed, the Committee shall meet as soon as possible to resolve
the issue.

 To investigate any complaint of occupational health, safety, ERP and environment issue
from workers or public that is brought to Committee’s attention.

 To keep in view the accident statistics and to monitor the trend of safety performance.

 To assist the project management to promote safety and health activities at site.

9.3 Toolbox Meeting (TBM)

TBM is run by respective SUBCONTRACTORS work supervisors or task leaders in each task group-
wise which is normally consisted of 5-20 workers. Main topics are risks and precaution concerned
with their specific works on that day. Sub-cons have to submit their weekly tool box attendance to
PPK on weekly basis. TBM Record form is filled then.

Mass TBM is held once per month for PPK & SUBCONTRACTORS. It shall be held at assembly point
on Wednesday, the last week in that month, and shall start at 0700. The mass TBM shall be cancelled
if there is any adverse weather condition or holiday.

9.4 S&H Report

PPK S&H Department shall prepare and submit a monthly report to CLIENT, CONSULTANTS & PPK.
The report shall include but not limited to the following:

i. Accident statistics

ii. Training conducted and attended

iii. S&H & ERP Minutes of Meeting

iv. Safety promotion

v. Environment Issue

SUBCONTRACTORS shall submit to PPK any S&H reports required by PPK’s S&H Officer &
Environmental Officer.

9.5 S&H Site Induction Briefing

All personnel working at the site shall receive an introductory briefing or training prior to entering
site, explaining:

o Overview of the company policies, objectives and commitment to safety and


the environment.

o Overview of Safe Work Procedures, hazard identification, HIRADC and risk


assessment.

o Legal requirement

o Weekly Toolbox Briefing, Use of PPE, Emergency Response Training

o Other project related matters.

9.6 S&H Signage & Labeling

PPK & SUBCONTRACTORS shall ensure that where supervisors or monitoring cannot be done
continuously at certain location or it is a requirement by law, they shall provide proper S&H signage
to create S&H awareness to nearby personnel. This signage can be either:

Mandatory requirement
Warning
Emergency preparedness

Labeling shall be applied to chemicals or schedule waste at site according to CLASS Regulation and
Schedule Waste Regulation. Relabeling shall be done whenever any chemicals are transferred from
its original container to another container. Language use for this purpose shall be understood by
every personnel.

9.7 Bulletin board

PPK shall ensure that S&H bulletin boards shall be place at locations such as site office, site entrance,
canteen, workers’ camp, labs, plants etc. It shall consist of following but not limited to:

- S&H Incident Sharing


- S&H Poster
- S&H Statistics
- S&H Program

9.8 WHATSAPP S&H Group , Electronic Mail & Site Memo

PPK & SUBCONTRACTORS shall also communicate S&H matters using media social apps and email to
ensure quick information sharing among PPK & SUBCONTRACTORS. The group shall only discuss
regarding S&H matter especially regarding unsafe act and condition found at site.

Electronic mail & site memo shall be used for formal communication among PPK &
SUBCONTRACTORS.

10. S&H MAINTENANCE

PPK & SUBCONTRACTORS shall ensure that every S&H items provided are maintained at work site
until the end of project. Such maintenance shall include:

10.1 S&H Audit And Inspection

The members of the S&H Committees shall conduct site audits minimum quarterly. The finding of
such inspections and audits shall be discussed in the S&H Committee meeting.

The Project S&H team shall conduct an inspection on monthly basis. The inspection will cover S&H
issues associated with the construction activities on the project. All findings of the S&H Audit and
Inspection shall be recorded in the Site Assessment for further action in rectify the finding as to
ensure compliance to S&H requirement and the risk is reduce to the minimum. PPK &
SUBCONTRACTORS management shall ensure that all audit and inspection findings are rectified
accordingly.

10.2 Training And Competency

PPK & SUBCONTRACTORS shall ensure that all personnel are competent to perform the job activities
assigned to them. In the event that PPK & SUBCONTRACTORS are unable to demonstrate the
competency of any person whose activities can directly impact on the Works S&H performance, that
person shall be removed from the site. Appropriate records of competency certificate, education,
training, skills and experience are maintained.

10.3 Unsafe act & Unsafe Condition

PPK & SUBCONTRACTORS shall ensure that any unsafe act and condition shall be reported and
rectified as they are the root cause of any accidents.

10.4 S&H Promotion

PPK & SUBCONTRACTORS shall ensure that S&H practices are well accepted by all personnel at work
site. Therefore, S&H Campaign & S&H Awards approach shall be used at site.

S&H Campaign shall focus on specific hazard or S&H matters for a month but shall not neglect the
other hazards. Activities that can be done during such campaign:

S&H Award shall be given in any form of

But subject to proposal or approval by CLIENT or PPK’s Project Manager.

11.S&H Good Practice

PPK & SUBCONTRACTORS shall ensure that:

- Major housekeeping shall be done on monthly basis


- Site pre inspection shall be done every start of work
- Housekeeping shall be done every end of work
- Mosquito breeding control such as larvaciding & fogging shall be done quarterly
- Pairing/Team system shall be implemented at all time during project
- No form of discrimination to those who practices safety at site
- Reverse parking at worksite
- No open burning at worksite
- No prohibited drugs or alcohol to be taken at worksite

12.PENALTY

PPK & SUBCONTRACTORS shall agree that failure to comply with the S&H requirement shall be
penalized. The penalization shall be in term of:

-Verbal warning

-Warning letter for FIRST & SECOND OFFENCE after verbal warning

-Removal from site for THIRD OFFENCE

-Deduction in term of payment from wages (PPK) / claims (SUBCONTRACTOR) based on S&H
compound schedule.
