1.0 GENERAL

1.1. During the period of construction process despite all safety measures has been put in
place, unexpected incident may happen due to certain unforeseen circumstances. The
incident may vary from minor to major incident depending on the situation and the
degree of hazard involved in the work process.

1.2. This procedure is to provide guide to the ERT in responding to any major emergency
occurring at bridge construction.

1.3 This procedure will be reviewed from time to time or as and when necessary and
updated to ensure its relevancy and considering any comments or recommendations
made by the Supervising Consultant, and other relevant bodies.

1.4 The formation of the ERT will comprise of all related parties that are involved in the
construction and completion of the Muara Lassa Bridge.

1.5 As the main contractor, Pekerjaan Piasau Konkerit Sdn Bhd will provide the immediate
manpower, equipment, plants, machineries, etc. when needed in case of any in crisis.
The team will be disbanded on completion of the contract’s expiry period.

1.6 Emergency plan shall be flexible for the team to response to any emergency that varies
from place to place and the construction work process involved.

1.7 The common problem and obstacle in managing and handling any emergency situation
is panic, not knowing what to do and making wrong decision. Therefore, the team must
be prepared to handle the worst-case scenario and be trained accordingly to meet the
requirement.

2.0 OBJECTIVE OF THE ERP

2.1 The main objective of the ERP is to respond to any major incident within the shortest
possible time in order to prevent situation from deteriorating and to save life, prevent
property damage and minimise public inconvenience.

2.2 The ERT shall ensure and maintain control in case of any emergency occurs at site and to
normalise and neutralize the situation by taking appropriate action deemed necessary.

3.0 POTENTIAL EMERGENCY SITUATION

3.1 Based on the scope of works and work process involved in the construction of new road
construction of MUARA LASSA BRIDGE PROJECT, the following situations may trigger an
emergency situation:

a. Site Office:

 Outbreak of fire/explosion.
 Structural collapse

b. Project Site:

 Fire and explosion.


 Topple or overturn of bore piling machine/crane.
 Collapse of structure or temporary structure.
 Accident of machinery with other construction vehicle or worker.
 Accident of construction machinery/vehicle with the public vehicle or
civilian.
 Electrical accident.
 Flash flood/land slide.
 Chemical spillage or leakage
 Workplace violence
 Fatality/injury or ill health
 Environmental damage (collapse of slope)

4.0 ERT ORGANIZATION

4.1 The project ERT organization structure is as at Attachment 1.

4.2 The roles and responsibilities of various team members are at Attachment 2.

5.0 FUNCTION OF ERT

5.1 Purpose of ERT is to set up an effective and efficient organization structure in order for
the organization to response effectively and swiftly to any major emergency situation.

5.2 In case of any emergency situation, the ERT will liaise with all the relevant authority or
parties in order for the team to effectively response to the situation.

5.3 The ERT is to respond quickly to any emergency situation so that human life can save
and to minimise injury or damage to property.

5.4 ERT is to control the situation at site and to bring the situation under control as soon as
possible so as to minimise inconvenience to public.

5.5 The team should have an effective communication system so that the team members
are able to communicate between one another in case of any emergency situation and
correct information can be communicated to the relevant authority or parties.

6.0 RESPONSIBILITIES OF ERT MEMBERS

6.1 In case of any emergency situation, ERT members must know to identify the crisis and its
nature, identify emergency location and degree of the incident. This makes ERT
members to have correct decision and able to handle the emergency situation.

6.2 To get appropriate required assistance from the relevant parties such as Pasukan
Pertahanan Awam, BOMBA or any other parties in order to save lives by providing

necessary first aid to the injured victim until the arrival of the medical assistance before
the victim is being sent to the nearest hospital.

6.3 To quickly bring down the situation under control in order minimise damage to property
and reduce public disturbance by deploying ERT while awaiting the arrival of outside
assistance if necessary.

6.4 To inform the relevant parties of the emergency situation including Supervising
Consultant/ CLIENT and Pekerjaan Piasau Konkerit Sdn Bhd.

6.5 To secure and cordon off the affected area from the public until the arrival of
relevant authorities.

6.6 To identify the required resources–manpower, equipment, plant and machineries to


assist in the situation.

6.7 To manage and control the traffic at the incident area to ensure a smooth traffic
flow at the incident site if public road is affected.

6.7 To carry out investigation on the incident with a view to identify the root cause of the
incident so that appropriate corrective and preventive measures can be taken to
prevent such incident from re-occurring.

7.0 INITIATION OF EMERGENCY RESPONSE

7.1 Upon notification by the first person at the scene or the any other person at the incident
site, Emergency Response Commander (ERC) will decide whether to active the ERP.
Should the ERP to be activated, all the ERP members will be notified immediately either
through mobile phone.

7.2 In case of any incident the ERC is to immediately inform CLIENT Project Manager/
Supervising Consultant and Pekerjaan Piasau Konkerit HQ and will follow with an initial
report using the standard reporting format within 24 hours.

7.3 To ensure that other relevant parties are being informed in due time, emergency
contact numbers will be displayed at the control room and at the notice board of project
site at various work zones. The list of emergency contact numbers is at Attachment 3.

7.4 Should there be involve any insurance claim, the procedures will follow the flow chart as
required by the client outlined in the Emergency Communication Flowchart.

7.5 When emergency situation is activated, the ERC will active the ERT and the Emergency
Control Room at Pekerjaan Piasau Konkerit Site Office Meeting Room will be
operational. The ERT will assemble at the designated location and ERC will brief all the
ERT members on the back ground of the emergency situation and current condition at
site.

7.6 ERT to proceed to the incident area with the necessary equipment and manpower.

7.7 ERC to control the ERT by providing directions and the necessary actions to be taken
from the control centre at the Pekerjaan Piasau Konkerit Site Office meeting room.

7.8 All business partners/ visitors which may be present at the time are accompanied by the
hosting person.

7.9 Do not go to any others part of work area for any reason.

7.10 Report to ERC ensuring that all occupants are safe and search by head count.

7.11 Do not leave the assembly point until instructed to do so by the ERC

7.12 All occupants, business partners/visitor must remain in the group until further advised.

7.13 Traffic Management Team to be deployed at incident site to control traffic when
public road is involved.

8.0 EMERGENCY RESPONSE PROCEDURES

8.1 Emergency Procedures for Accident of Fire/Explosion.

a. The potential fire /explosion incident may happen at any highly flammable storage
area, flammable chemical storage area, and electrical supply sub-station or
distribution box. In case of any fire incident due to any electrical short circuit, may
trigger explosion if the situation is not quickly brought under control.

b. In case of any fire /explosion incident, the action process will follow the action
flow chart at Attachment 4.

8.2 Emergency Procedures for Topple or Overturn of Bore Piling Machine/ Crane/Heavy
Machinery.

a. The incident of topple or overturn of bore piling machine or crane may occur due
to any unforeseen circumstances or due to sudden adverse weather condition at
work site.

b. In case of any incident of topple or overturn of bore piling machine/crane, the


action process will follow the action flow chart at Attachment 5.

8.3 Emergency Procedures for Collapse of Structure or Temporary Structure.

a. The potential collapse of structure or temporary structure is from the construction


of steel structure work or scaffolding erected as the temporary working platform.

b. In case of any incident of collapse of structure or temporary structure, the action


process will follow the action flow chart at Attachment 6.

8.4 Emergency Procedures for Accident of Machinery with Other Construction Vehicle or
Worker.

a. The accident of project site machinery with another site vehicle or worker may
occur at site especially when the working space is very limited a large number of

workers and machinery particularly when conducting construction work at the


road median or at the construction area.

b. In case of any incident of collapse of structure or temporary structure, the action


process will follow the action flow chart at Attachment 7.

8.5 Emergency Procedures for Accident of Machinery/Vehicle with Public Vehicle or


Civilian.

a. The accident of project site machinery with public vehicle or civilian may occur at
site as most of the construction work involved public especially when conducting
work along the existing BJT road side or at the median of the road. The project
vehicle may accidently hit the public vehicle or motorcycle or civilian passing
through at the junction to project site.

b. In case of any accident of machinery/ vehicle with another public vehicle or


civilian, the action process will follow the action flow chart at Attachment 8.

8.6 Emergency Procedures for Electrical Accident.

a. The accident may occur when conducting work at area where the overhead
temporary electrical supply cables at the WPC10 project. The unintentional hitting
of overhead cable may cause electrocution to workers or other parties who may
be affected at the particular area at that point of time.

b. In case of any electrical accident at site, the action process will follow the action
flow chart at Attachment 9.

8.7 Emergency Procedures for Heavy Rainstorm and Flood.

a. Once there is a heavy down pour for long hours, the Environmental Officer/ Safety
Officer must monitor the worksite to check the water level and water flow into
the stream and drainage within the site and the impact of nearby slope.

c. In case of any heavy rainstorm and flood incident, the action process will follow
the action flow chart at Attachment 10.

8.8 Emergency Procedures for Chemical Spillage.


a. The potential Incident of chemical spillage when transferring of diesel into the
skid tank, when refilling of diesel into the machinery at the work site or when
loading and unloading of other chemical material at storage yard or work site.

d. In the event of any chemical spillage, the action process will follow the action flow
chart at Attachment 11.

8.9 Emergency Procedures for Fatality Incidents

a. In the event of fatal accident, the notifier shall immediately inform ERC who shall
order of stop work at the location of incident.

b. ERT shall implement an immediate systematic safety check of the affected work
area. The inspection shall be monitored by Safety & Health Officer.

c. For normal injury, work shall not resume until ERC has signed off that the work
area is safe.

d. For fatality, work can only resume when the Police and Department of
Occupational Safety and Health (DOSH) officer have inspected and given their
authority for the resumption work.

e. In case fatal incidents, the action process will follow the action flow chart at
Attachment 12.

9.0 ASSEMBLY POINTS

9.1 Responsibilities of all personnel

All workers and staffs must understand the correct emergency response and general
evacuation procedures for their location. During an evacuation, all personnel must
assist the visitors, and contractors to exit the site office or construction area.
9.2 Use of evacuation procedure

 Upon notification that an evacuation is in progress, all workers, staffs and visitors
will immediately use the nearest emergency exit and proceed to their designated
assembly location.
 The Roll Caller will take a roll call or perform an accountability count.
 The Roll Caller will then be reported to the ERC or ERTL. The lead staff or site
supervisor shall designate the names of the persons from his or her
department/team work who may missing.
 Visitors will remain with the staff member(s) they are visiting and their names will
be reported to the ERC.
 All personnel will stay assembled by department or team until further instructions
are received from ERC.

However, it is important to note that in some emergencies, employees must deviate from these
instructions. Use common sense. For example, if smoke is present, employees need to begin
evacuating even if the alarm has not been sounded.

Assembly point for all occupants should be used at all-time unless it is not safe to do so. The
locations for the assembly points are near the proposed contractor site office as shown detail in
the Attachment 14.

10.0 LINES OF COMMUNICATION

10.1 In the event of any major incident or accident CLIENT Project Manager, Supervising
Consultant and Pekerjaan Piasau Konkerit HQ is to be notified immediately through the
phone or any quickest mean and to follow with initial report within 24 hours.

10.2 All other relevant parties which are affected by the incident/accident are to be
notified accordingly.

10.4 Pekerjaan Piasau Konkerit shall establish a positive means of communication either by
hand phone or other effective means.

11.0 PRESS RELEASE/PRESS CONFERENCE

11.1 Any press release or press conference will only be done by CLIENT Manager corporate
communication. No other party is allowed to give out any information regarding any
incident on behalf of CLIENT.

12.0 TRAINING AND TABLE TOP EXERCISE

12.1 In order for the ERT members to function effectively in performing their duties in case of
any emergency situation, the team will have to undergo various training and emergency
drill exercise. Amongst the training required are:

a. First Aider course/training.

b. Traffic management and control.

c. Fire drill and use of fire extinguisher.

12.2 The ERT shall as deem necessary to conduct Table Top Exercise from time to time
together with the Emergency Services, so as to enhance the ability and efficiency of
team members in handling actual situation in case of an emergency.

12.3 The ERP training is designed in accordance to emergency response duties, usage of
emergency equipment, facilities, initiation of emergency process.

12.4 Fire evacuation drill will be conducted annually in which event all relevant parties shall
participate.

13.0 INCIDENT/ACCIDENT INVESTIGATION

13.1 Any major incident or accident happened at site, an incident investigation should be
conducted immediately after the incident and the report to be submitted to the
Supervising Consultant/ CLIENT and Pekerjaan Piasau Konkerit HQ within 7 days after
the incident using the format at Attachment 16.

13.2 Any major incident involving related parties such as Sarawak Energy Berhad, JBALB,
Telekom Malaysia and Sacofa the investigation team should include one of them as
member so that appropriate expertise advice can be acquired.

13.3 Objectives of an investigation are:

a. To identify and determine the root causes of the incident/accident and the
contributing factors so that preventive measure can be taken to prevent a
recurrence of such incident.

b. To determine the events that leads to the incident to happen.

c. To determine losses so that appropriate compensation/ claims to relevant party


can be justified.

d. To establish costs for insurance claim with appropriate justification.

e. To determine any short coming or weakness in the current work system or


procedures so that work process/system can be improved.

14.0 CONCLUSION

14.1 Equipment /Facility List

The emergency equipment and facility may include but not limited to the following:

 Luminous Safety Vest – for all ERT members.


 Haller (loud speaker) with siren
 First-Aid Box
 Stretcher
 First-Aid Facilities Post/Room.
 Torch Light – for all ERT members and other staffs as deemed necessary.
 Fire Extinguisher – at least 3 units for each CLIENT Site Office, Supervising
Consultant Site Office, Pekerjaan Piasau Konkerit Site Office and subcontractor

site office. Dry powder for solid fire and CO2 for the oil and grease fire such as at
the workshop and skid tank area.

 Others equipment’s as deemed necessary

In ensuring the emergency equipment is in good condition and serviceable, inspection


shall be conducted every month.

ATTACHMENT 2: RESPONSIBILITIES OF ERT MEMBER

1.0 Emergency Response Commander (ERC)

1.1 Direct report to CLIENT Project Manager pertaining to any matters in managing new road
construction of MUARA LASSA BRIDGE PROJECT.

1.2 To ensure that emergency equipment is appropriately provided and where necessary be
tested and regularly maintained.

1.3 To conduct emergency drill from time to time to ensure competency of the ERT team
members and other person at work.

1.4 To take charge of directing, coordinating all emergency operations and to coordinate with
client and consultant any emergency information, providing advisory on evacuation activity
at emergency event.

1.5 To decide whether to declare an ‘EMERGENCY’ situation in case of any incident involving
accident to any person at workplace, fire/explosion, structure failure, flash flood, land slide
or vehicle/machinery accident.

1.6 To give instruction by using any megaphone, telephone or other suitable means during an
emergency and to close-out after the event of the emergency.

1.7 To coordinate with External Assistance from Emergency Services for any operations
shutdown of Major Incidents.

1.8 To facilitate client with the latest information of the incident’s occurrence for press
release/conference by client.

1.9 To prepare and submit Emergency Report to the management on emergency occurrence.

2.0 Emergency Response Team Leaders (ERTL)

2.1 Direct report to Emergency Response Commander when emergency response is established
at work place.

2.2 To assist ERC in activating Emergency Response System when emergency occurred at work.

2.3 To be person in-charge of Emergency Response Team in coordinating emergency

2.4 To ensure that ERT members are familiar with ERP and able to co-relate the functions and
importance to TRC staff members, workers and subcontractors.

2.5 To be an emergency event’s moderator in between client, consultant and relevant


authorities.

2.6 To coordinate with client, consultant and relevant authorities (Jabatan Bomba dan
Penyelamat, PDRM, Hospital, Ambulance Service, DOSH, NREB, Local Authority Council, and
etc. as applicable).

2.7 To transmit/convey any emergency instruction from ERC to ERT members and to compile all
feedback from ERT members during the event of the emergency for ERC’s updated.

2.8 To facilitate ERC with the latest information of the incident’s occurrence for press release
by client.

2.9 To communicate with ERC at all times in managing emergency response at work place.

3.0 Emergency Coordinator (EC)


3.1 Assistant to Emergency Response Team Leaders (ERTL) when emergency response team has
activated.

3.2 To ensure that ERP is understood and complied by all ERT members, TRC project staffs, and
down lines (workers and sub-contractors).

3.3 To liaise with client, consultant and relevant authorities (i.e. Jabatan Bomba dan
Penyelamat, Police Department, Hospital, Ambulance Service, DOSH, NREB, Local Authority
Council, and etc., as applicable).

3.4 To assist ERTL in organizing any emergency drill or evacuation drill.

3.5 To transmit/convey any emergency instruction from ERTL to ERT members and to compile
all feedback from ERT members during the event of the emergency for update.

3.6 To get latest report from Traffic Management Officer for evacuation drill activity at
Assembly Point Station (if any).

3.7 To coordinate with Historian for any event’s update and to advise ERTL, Jabatan Kerja Raya
Sarawak and Supervising Officer’s on the latest situation and information of any incident at
work.

3.8 To provide clear passage for emergency vehicles to enter the incident site.

3.9 To interact with Liaison Officer or the supervisor on site to cordon the incident area with
proper and suitable barricade in order to avoid public and media intervention.

3.10 To provide details of existing Fire Fighting System/equipment in a building/premise/site


to Jabatan Bomba dan Penyelamat in case of any outbreak of fire.

3.11 To facilitate ERTL any information for Emergency Report or for press conference which is to
be made up by the JKR regarding to the incident occurrence’s update.

4.0 Historian
4.1 Direct report to ERTL/EC when emergency response team has activated.

4.2 To maintain appropriate record/log book and to record all events of any incident or
accident happened at the project site.

4.2 Should any incident or accident happened at project site, to record all the related activities
right from the beginning up to the end until the situation is clear and back to normal (event
chronology).

4.3 To archive relevant documents of all the incident/accident activities at the end of the year
for records and authorities audit purposes (if any).

4.4 To liaise with ERT members in order to acquire correct information on the events of any
incident or accident.

4.5 To update ERTL/EC and Liaison Officer with any latest information needed.

5.0 Liaison Officer

5.1 Direct report to ERTL/EC when emergency response team has activated.

5.2 To circulate all relevant information (i.e. from Historian) in due time to Project
Management staff in case of any incidents, events or activities happened at project site.

5.3 To communicate with all departments and subcontractor’s SHE Representative pertaining
to matters related to project safety and health in case of any emergency at work.

5.4 To coordinate with outside agencies (third party) as and when necessary in case of any
emergency situation occurred at the project site.

5.5 To ensure that no staffs to give any information to the media or press in case of any major
incident/accident. Press release will only be done by the client, Jabatan Kerja Raya Sarawak.

5.6 To update ERTL/EC for any latest information needed.

6.0 Traffic Management Officer

6.1 Direct report to ERTL/EC when ERT System activated at work.

6.2 To assemble all site personnel and workers according to departments/sections/


subcontractors for easy identification at the Assembly Point Station.

6.3 To carry out assembler’s head count and maintain crowd control at the Assembly Point
Station.

6.4 To counter check with the personnel or workers if anyone is injured during the evacuation.

6.5 To check with down lines (the personnel or project workers) if any of their colleagues came
to work but are not present at the Assembly Point Station.

6.6 To direct any injured person to First Aid Team for first aid treatment.

6.7 To update ERTL/EC on latest personnel and workers listing, records of any missing
personnel or workers in evacuation activity at Assembly Point Station.

7.0 Traffic Controller

7.1 This team is in charge of helping manage the traffic flow, movement and direction of
emergency vehicles. By doing so, he assists and guides the drivers in their duties of
reversing and deploys emergency equipment’s.

7.2 As there is a lot of movement during emergency situation, this team is an essential part of
the smooth flow of operations.

7.2 The team direct the flow of traffic and he needs to ascertain that the deploy area is clear of
any obstructions. This team need to constantly monitor the surrounding loading areas for
any underground services or obstruction.

7.3 Traffic Controller will be assisting by Flagman during the emergency to give direction to
emergency services.

8.0 First Aider Team


8.1 Direct report to ERTL/EC when ERT System activated at work.

8.2 To set up equipped first aid base station for medical emergency preparedness at work
place.

7.3 To ensure that all medicines in the first aid box are sufficiently kept and maintained.

8.4 To periodically check the first aid box to ensure that necessary medicines are in sufficient
quantity and with valid expiry date.

8.5 To ensure emergency transport equipment i.e. portable stretcher, wheel chair and etc. are
well kept and sufficient at all times.

7.6 To provide first aid treatment to the victim before transferring to hospital or clinic.

8.7 To assist Ambulance Medical Services in any emergency occurrence at work place (if any).

8.8 To account and register victims’ particulars in First Aider Log Book and to update ERTL/EC
for any latest records of injured person and treatment history.
