1. INTRODUCTION

1.1 This HSE Plan outlines the safety rules and regulations, which must be followed by all
personnel within . work site at construction of “CRF/NANO & Life Sciences Building
Diamond Jubilee Complex at IIT Kharagpur (West Bengal)”. .

1.2 It is produced primarily for the use of the Kunal Structure India Pvt Ltd. and its Sub-
contractor’s Management and Supervisory staff that is required to ensure that the
rules and procedures are brought to the notice of Subcontractors’ employees and that
such rules and procedures are strictly followed for the safe execution of the
construction of the project.

1.3 Any person uncertain of the contents of the HSE Plan should consult the Kunal
Structure India Pvt Ltd.. Corporate HSE Department for clarification. Any person doing
work requiring special precautions or more detailed guidance on safety procedures he
should discuss with Kunal Structure India Pvt Ltd.. HSE Department prior to
commencement of his work.

3. SCOPE

The HSE Plan has been prepared for the safe execution of “Construction of CRF/NANO &
Life Sciences Building Diamond Jubilee Complex at IIT Kharagpur (West Bengal)”.
being executed by KUNAL STRUCTURE INDIA PVT LTD.., and this is a guideline for all
employees of KUNAL STRUCTURE INDIA PVT LTD.., as well as its sub-contractors for
ensuring minimum risk to environment, property and human beings working on site or using
the internal roads where construction activity is in progress. This HSE Plan enables the
control of Environment, Health & Safety risks and help in the continual improvement of
its HSE performance.

Sub-Contractor Any individual or company which is engaged by
KUNAL STRUCTURE INDIA PVT LTD.. to perform work at
the site or to provide labor, equipment, facilities or material, to
be used at the site.
Act / Regulation Any act and regulation of India together with the
Relevant amendments or revision thereof, that the KUNAL
STRUCTURE INDIA PVT LTD. and its Subcontractors are
required to comply with during the contract period.

3.2.1 Some HSE Definitions & Terminologies:

 ENVIRONMENT - Surrounding in which an organization operates is called Environment.


It includes air, water, land, natural resources, flora, fauna, human beings, and their
interaction.

 ENVIRONMENT ASPECT - Element of an organization’s activities or products or


services that can interact with the environment

 ENVIRONMENT IMPACT - any change to the environment, whether adverse or


beneficial, wholly or partially resulting from an organization’s environmental aspects

 HAZARD - Something that has the potential to cause harm or damage.

 ACCIDENT - An unexpected, unplanned event that can cause harm to people, damage to
Equipment / Environment or loss to Property / Production.

 LOSS TIME INJURY(LTI) - An injury / accident sustained during work time that results in
any person, including subcontractor personnel, being absent from the place of work
and/or he./he is unable to carry out beyond their normal range of duties for more than
three consecutive full days excluding the day of incident(more than 48 hrs).

 SERIOUS INJURY ACCIDENT - Fatality, fracture of skull / spine / pelvis, amputation of a


hand or foot, loss of sight of eye, loss of consciousness due to electric shock or lack of
oxygen, hospitalization due to burns, decompression sickness, loss of consciousness
resulting from absorption of any substance by inhalation, ingestion or through the skin,
acute illness resulting from exposure to a pathogen or infected material.

 SERIOUS NEAR MISS - An ACCIDENT that has minor or no loss effect but which under
slightly different circumstances could have resulted in a REPORTABLE ACCIDENT.

 REPORTABLE ACCIDENT - ALL LOSS TIME INJURY accidents, all SERIOUS INJURY
ACCIDENTS, all injury accidents involving members of the public, all property damage
accidents and / or case of theft involving loss exceeding Rs. 5000/-, all utility damage
accidents, all company vehicle accidents, all fires and any accident or incident likely to
have a substantial loss effect for the company.

 OCCUPATIONAL ILLNESS - Any illness is being temporary or permanent directly arisen


from the type of work or environment under which the personnel worked. The disease
which encountered during carry out the constant/production activity.

 AUDIT - Systematic examination to determine whether activities and related results


conform to planned arrangements are implemented effectively and are suitable for
achieving the organization policy and objectives.

 INTERESTED PARTIES - Individual or group concerned with or affected by the HSE


performance of an organization.

 NON-CONFORMITY - Any deviation from work standards, practices, procedures,


regulations, management system performance, etc. that could either directly or indirectly
lead to injury or illness, property damage, damage to workplace environment, or a
combination of these.

 OBJECTIVES - Goals in terms of HSE performance that an organization sets itself to


achieve.

 OCCUPATIONAL HEALTH & SAFETY – Conditions and factors that affect the well-
being of employees, temporary workers, contractor personnel, visitors and any other
person in the workplace.

 HSE MANAGEMENT SYSTEM - Parts of overall management system that facilitates the
management of the HSE risk associated with the business of the organization. This
includes the organizational structure, planning activities, responsibilities, practices,
procedures, processes and resources for developing, implementing, achieving, reviewing
and maintaining the organization’s HSE Policy.

 ORGANIZATION - Company, operation, firm enterprise, institution or association, or part


thereof, whether incorporated or not, public, that has its own functions and administration.

 PERFORMANCE - Measurable results of the HSE management system, related to the


organizations control of environment, health & safety risk, based on its HSE policy and
objectives.

 RISK - Combination of the likelihood and consequences of a specialized hazardous event


occurring.

 RISK ASSESSMENT - Overall process of establishing the magnitude of risk and deciding
whether or not the risk is tolerable.

 SAFETY - Freedom from unacceptable risk of harm (Freedom from Danger).

 TOLERABLE RISK – Risk that has been reduced to a level that can be endured by the
organization having regard to its legal obligations and its own HSE Policy.

 DEVIATION OR NON-CONFORMITY – It is something not in compliance with quality


standard, specification or measuring requirements, or as deviations from specified
procedures or way of working within production, environment, working environment
(safety) or security.

 CORRECTIVE ACTION - Action taken to eliminate the causes of an existing non-


conformity, defect or other undesirable situation.

 PREVENTIVE ACTION - Action taken to eliminate the causes of a potential non-


conformity, defect or other undesirable situation in order to prevent occurrence or
recurrence.

3.3.1 GENERAL REQUIREMENTS:

This document contains the HSE Management System to eliminate or minimize the risk and
implement, maintain and continually improve a HSE Management System that includes the
Environment, Health & Safety requirements to be adopted in accordance with ISO
14001:2004 and OHSAS 18001:2007, the national regulations and Client's Safety Guidelines.

Since Drug Abuse and HIV are the inherent hazards in the Construction Projects an ongoing
awareness is necessary to highlight the serious concerns to the concerned, the Trainings and
Awareness sessions are also initiated to safeguard personnel from such Risks. In addition to
HSE policy Drug and HIV Policy is also adopted as a part of HSE Commitment set forth in
relevant Policies.

These Policies are communicated and implemented through the same process explained as
above.

4. ROLES AND RESPONSIBILITIES

4.1 PROCEDURE :

Those persons with specific safety responsibilities are listed below. Everyone on site
is expected to comply with all project safety requirements and to ensure that his/her
subordinates comply also.

4.2 PURPOSE :

To define the relationship and responsibilities of Kunal Structure India Pvt Ltd.
personnel so as to ensure proper execution of responsibilities, full coverage of all
activities which are understood by all, systematic application of functions and good
relations with Company and Subcontractors.

4.3 PROCEDURE :

This procedure defines the Project Organization and the responsibilities and duties of
personnel at Kunal Structure India Pvt Ltd.Site Office during the construction.

4.4 ROLES & RESPONSIBILITIES:

4.4.1 Project Director

POSITION : Project Director


REPORTS TO: Managing Director
SUPERVISE : Overall HSE Function

RESPONSIBILITIES AND DUTIES:

1. The Project Director Responsible for Safety shall initiate HSE policy and to ensure that
each project in the KSIPL business uses the service of the HSE department and operates
within the HSE Department Policy.

2. Promote an interest and enthusiasm for Occupational Health, Safety and Environment
in the organization and within the business.

3. Ensure that arrangements and resources for the operation of the HSE organization are
adequate (reviewed as necessary) and the HSE Management System is effectively
enforced and implemented.
4. Ensure an effective safety service is provided, staffed by competent Safety Personnel
and ensure that each project operates within the corporate Policy and guidelines. The
Project Director will report to the Managing Director and Board of Directors.

5. The Project Director will guide HSE Department Department and will monitor overall HSE
function through HSE Department Head.

6. Ensure Health, Safety, Environment is the first agenda action point of all Board Meetings
and the board properly discharges the above responsibilities.

4.4.2 Project Manager

POSITION : Project Manager


REPORTS TO: MANAGEMENT REPRESENTATIVE ( SR.VP/ AVP) / VERTICAL
DIRECTOR
SUPERVISE : DEPUTY Project Managers AND DEPARTMENT HEADS

RESPONSIBILITIES AND DUTIES :

The Project Manager is responsible and accountable for project safety and for working with
various departments, employer’s representative, and assigned staff to ensure that from

project inception to contract completion adequate resources and top management support
are provided to ensure a safe work site in accordance with HSE Plan. This includes proper
staffing, financial support, safe design and management support for necessary actions taken.
The responsibilities and duties shall include the following:

1. Delegate specific responsibilities and duties to all field management staff and ensure
such functions are carried out.
2. Establish a realistic Safety Policy for the site.
3. Make clear and stress at every opportunity that Safety and Health are a line
responsibility. Lead by example.
4. Direct field management in carrying out their duties and responsibilities.
5. Preside over Safety Committee Meeting and delegate this responsibility to Deputy
Project Manager when unable to attend.
6. Make final decision on matters affecting field construction work, after consultation with
various groups within the field organization.
7. Advise and / or instruct subordinate regarding any actions necessary to correct any
Hazardous activities or work conditions.
8. Review and finalize Kunal Structure India Pvt Ltd.. Safety Management System and
Safe Work Practices and other safety Documents.
9. Authorize Safety Incentive Scheme.
10. Patrol the site as frequently as possible to ascertain the work and safety status.
11. Establish and direct an Accident Investigation Team when an accident occurs.
12. Suspend work or prohibit the use of facilities, if emergency measures are required, to
correct the hazardous activities or work conditions, until their rectification is confirm.
13. Remove any person from the site who seriously or repeatedly fails to comply with the
safety and security requirements and rules.
14. Take the lead in promoting housekeeping at the highest standards.
15. Review safety performance, safety topics, and safety activity status with the Deputy
Project Managers, Safety Officer (HSE) and the Construction Managers on a regular
basis.
16. Report on safety performance, safety activities, and any accidents or near misses, to
the employer’s representative in a regular and timely manner.
17. Review and assign the responsibilities and duties of each position.
18. To monitor and measure, with the field Management Staff and HSE Department, the
safety performance of each employee. Recognition shall be awarded to those who

have performed consistently well. Corrective measures/ disciplinary actions shall be


applied to those who fail to comply with their designated duties and responsibilities.

4.4.3 CONSTRUCTION MANAGER

POSITION : CONSTRUCTION MANAGER


REPORTS TO: Project Manager
SUPERVISE : ENGINEERS

RESPONSIBITIES AND DUTIES :


The Construction Manager shall take the initiative to ensure that the total construction work
progresses smoothly and safely.

The responsibilities and duties shall include the following :

1. Coordinate, plan the work, schedule, and liaise with other Managers to ensure that
conflicts of interest between Sub-contractors do not occur and are performed in a safe
and efficient manner.
2. Direct Engineers on their safety duties and responsibilities.
3. Preside over the Construction/Progress Coordination Meeting and take the lead to
address safety issues in the meeting.
4. Analyze all works, identify any serious hazards and be proactive in implementing
procedures that encourage safety construction..
5. Review Method Statements and Risk Assessments himself or assign competent
personnel to do so.
6. Ensure that Method Statements which incorporate adequate safety provisions for
safety have been provided for all critical work.
7. Direct Engineers to check that the work at the site complies fully with the Method
Statement and the precautions given.
8. Patrol the site daily and ascertain the work and safety status.
9. Advice and / or instruct Engineers regarding necessary corrective actions for unsafe
activities and hazardous or unhealthy working conditions.
10. Review the Tool Box Meeting activities carried out by subcontractors. Dictate the
frequency of Tool Box Meeting of each work group with the advise of the Engineers
and Safety Personnel.

11. Confirm that Subcontractors are fulfilling their responsibilities in regards to safety
requirements.
12. When critical measures are required to correct unsafe activities or work conditions,
suspend work or prohibit the use of facilities until rectification is confirmed.

Procedures at the site.

4.4.4 CONSTRUCTION SUPERVISOR

RESPONSIBITIES AND DUTIES :


The Construction Supervisor is responsible for the safety of all workers under their control.
This means that all safety regulations are complied with and enforced within the group or
groups of workers that they supervise. HSE Plan is included in the scope of the work under
their control.
The responsibilities and duties shall include the following :
1. Instruct the subcontractor’s personnel under their control on the measures necessary
to perform the work safely and without health risks.
2. Ensure that no work is carried out without an appropriate Method Statement and Risk
Assessment, that subcontractor’s person in charge receives a copy of the
appropriated Method Statement and Risk Assessment, and that work is carried out in
accordance with the Method Statement and Risk Assessment.
3. Stop work if critical measures are required to counter unsafe actions or working
conditions, until the conditions are rectified.

4. Ensure that all tools, machinery and equipment used by the subcontractor are
adequate for the job and meet satisfactory standard. Ensure that these machinery and
equipment are maintained and inspected according to the procedure of the
Maintenance Regime for machinery and equipment.
5. Ensure that worker wear and properly use Personnel Protective Equipment according
to the requirements.
6. Ensure that only competent personnel work on the site.
7. Ensure that housekeeping standards are maintained and make arrangements for
waste disposal.
8. Ensure that the subcontractors’ workers receive the necessary Tool Box briefings.
9. Attend Construction / Progress Coordination Meeting and discuss any safety matters
which is within their work control.
10. Complete an Accident Investigation Report ensuring that each accident and / or near
miss is investigated and such corrective action taken to prevent re-occurrence in the
future.
11. One copy of this report is given to the Construction /Engineer and one is sent to the
Safety Supervisor.
12. Carry out daily safety patrol and submit the Site Safety Inspection Checklist to HSE
Department upon completion of the safety tour.
13. Ensure attendance in any Safety Training and Courses.
14. Ensure that all workers familiarize with the Emergency and Evacuation
Procedure and ensure compliance with the requirement of the above.

4.4.5 PROJECT HSE MANAGER

As a full time HSE Manager on construction site, his responsibility shall be as follows;

1. He shall monitor that client and Company's procedures are followed during

through audio/visual means, Safety meetings and training.
5. He will carry out Safety Inspection on Scaffolding, Equipment, Ladders, Lifting
Equipment and Tools on regular basis and certify their fitness.
6. He will prepare safety statistics and report and submit on monthly basis to Project
Manager and Head Corporate - HSE.
7. He will advise Project Manager on HSE related matters including hygiene of labor
camp and work site.
8. He shall monitor and assist improvements in weekly tool box meetings and Safety
committee meetings at this project.
9. He shall ensure that the accident investigation and reporting procedure is informed
to all employees and meet the necessary HSE standards.
10. Discuss and initiate housekeeping, solid waste disposal, air/water quality
management, noise/ventilation and fire prevention activities.
11. He shall ensure awareness of emergency procedure among all employees and
conduct emergency evacuation drills every quarter or as per the requirement. He shall
organize required training sessions at site.

4.4.6 SITE ENGINEER

RESPONSIBILITIES AND DUTIES :


Construction Engineer shall directly supervise the work force and the selection of the actual
sequence and method of construction.
The responsibilities and duties shall include:

1. Familiarize with the Kunal Structure India Pvt Ltd.. Safety Policy, HSE Plan,
Emergency and Evacuation Procedures and any
other relevant Safety Programmes. Lead by example.

4. That the method and sequence of construction operation are in accordance with
approved Method Statements and safety controls stipulated in the Risk
Assessment, Safe Work Practices and other Safety Guidelines. Ensure that
respective supervisor understand the work hazards and that a copy of the
particular Risk Assessment is made available to the supervisor for the applicable
work.
5. That housekeeping is maintained and the job orderly and clean at all the times.
6. That supervisors and subcontractors are trained in the safety requirements for the
work and held accountable for the safety performance of workers and / or areas
they supervise.
7. Ensure that Tool Box Meeting are held accordance with requirement and attends
randomly to participate and show support.
8. That a daily safety tour is made with the Safety Supervisor to inspect for unsafe
conditions or behaviors and to show support for the program. Items that require
correction will be noted and acted upon immediately. Site Safety Inspection
Checklist will be completed and submitted to HSE Department.
9. Participate in the Construction / Progress Coordination Meeting and discuss any
safety issues require addressing.
10 Conduct accident investigations and file reports following all serious injuries or near
miss accidents within the area. Participate in the Accident Investigation Team when
assigned by the Project Manager.
11. In the event of any Emergency, ensure compliance by all subordinates with the
Emergency Procedure.

liaison and coordination channels are maintained among the Kunal Structure India Pvt Ltd..
management and the Subcontractors on Safety and Health Control.
The responsibilities and duties shall include the following:

1. Provide safety information and guidance to supervisors on proper safety procedures,


current and potential hazards and employee training requirements.
2. Conducts project safety inspection and reports infractions to the applicable supervisor
and the Construction Manager and subsequently inspect to see that infractions are
corrected.
3. Review the Method Statements and Risk Assessment and to suggest improvements
or any safety requirements which need to be incorporated into the above. Ensure that
the respective supervisor applies the approved method statements and safety control
stipulated in the Risk Assessments during the execution of the work.
4. Maintain register of any defects or violations observed, and of remedial actions taken
5. Conduct investigation of all fatal, lost-time accidents, significant first aid cases, near
miss accident and damage to property or equipment. Complete and analyses each
Accident Investigation Report.
6. Conducts the Safety Induction Programme for new employees and subcontractors.
7. Assists in Safety Training, Courses, and Seminars.
8. Assist Construction Supervisors and Foreman during the Tool Box Meeting to be held
every morning or weekly depending on the working status.
9. Inspect for fire hazards and check that fire protection equipment is adequate,
accessible, and in proper working order.
10. Supervise and implement Security Procedures.
11. Assist in supervising medical facilities and maintaining medical records.

1. Familiarization with the Kunal Structure India Pvt Ltd.. Safety Policy, HSE Plan,
Emergency
and Evacuation Procedures and any other relevant Safety Programme.
2. Enforces established safety regulations and work methods. Assures that proper
tools and safe equipment are provided to perform the work properly. Takes
disciplinary action necessary to ensure compliance with the rules.
3. Hold a minimum of one Tool Box Meeting a week with workers and records the
meeting.
4. Informs Construction Supervisors and Safety Supervisor of topics discussed at the
Tool Box Meeting and provides a record of the meeting to the Safety Supervisor
5. Inspect for unsafe practices and conditions as a part of the regular tour of work
areas and ensures prompt corrective action to eliminate causes of accidents.
6. Instill by action, example, and training, a sincere safety attitude in fellow supervisory
personnel and employees.

1. Sub-contractor’s Site Manager has full responsibility for the Safety Management of
his part of the work and of the area concerned.
2. Subcontractor’s Site Manager shall submit their Safety Organization, and HSE Plan
to Kunal Structure India Pvt Ltd..
3. Subcontractor’s Site Manager shall assign a full-time safety supervisor as required
by the Factories Act.
4. Subcontractor’s Site Manager and Safety Supervisor shall attend the Safety
Committee Meeting and Site Inspection or any other safety meeting where
attendance is required. Important matters that have been deliberated on at the
meeting shall be disseminated to all subordinates.
5. Subcontractor’s Site Manager shall attend and ensure that his subordinates
participate in various Safety Training held by various institutions or by Kunal Structure
India Pvt Ltd..

6. Subcontractors shall provide to their workers either new or well maintained tools and
equipment to be used for the work.

Equipment. Issuance of Personnel Protective Equipment are to be recorded.
Personnel Protective Equipment shall be promptly replaced if damaged or
deteriorated.
8. Assign sufficient competent workers to the work. Where Kunal Structure India Pvt
Ltd.. requires acknowledgment of the ‘competent’ person, the subcontractor shall
furnish detail and experiences of the said person.
9. Subcontractors shall provide and ensure that where License person is required to
be appointed for inspection or carrying out the work, the subcontractor shall
complies with and furnish the copy of the license to Kunal Structure India Pvt Ltd..
10. Method Statement shall be prepared and submitted to Kunal Structure India Pvt Ltd..
11. Report all accidents, property damage and near misses to Kunal Structure India Pvt
Ltd..
12. Participate and support / contribute to any Safety Promotional Activities with the
objective of increasing the Safety Awareness of the work.
13. Strictly maintain discipline of the workers.
14. Lead in housekeeping activities and expect a high standard is maintained throughout
the work.
15. Work closely with Construction and HSE Department of Kunal Structure India Pvt Ltd..
16. Ensure that all personnel understand the Emergency and Evacuation Procedure.

5. SITE HSE COMMITTEE

The Kunal Structure India Pvt Ltd.. Shall set up a Safety Committee. The size and
composition of this committee will depend on the work site, but will include:

 The HSE Officer or a person who is responsible for safety in the work site.

 Sufficient representatives to ensure that all aspects of the operation of the work site
are properly represented.

The Kunal Structure India Pvt Ltd.. will ensure that Safety Committee members are:

 Competent for their tasks.

 Committed to safety in the work place and having an attitude towards safety
Promotion.

 Given the necessary support so that they can effectively perform their duties.

5.1 SAFETY COMMITTEE MEETING :

5.1.1 Objective
Objective of the Safety Meetings is to assemble people with particular responsibilities for
safety so that they can formally address issues and take appropriate actions in relation to the
achievement of the work site safety management objectives. Kunal Structure India Pvt Ltd..
will organize a Safety Committee with the following main aims:

 Confirm if the management of safety, environment and health is being properly carried
out by all the parties concerned.
 Ensure that the construction work is being performed safely and smoothly, complying
with safety rules and regulations.

5.1.2. Frequency
Kunal Structure India Pvt Ltd.. Safety Committee will be held once a month.
5.1.3 Members
The Kunal Structure India Pvt Ltd.. Safety Committee shall consist of Employer’s
representative, Kunal Structure India Pvt Ltd.., and Subcontractor’s Management
Staff, Safety Personnel and appointed workers. The Safety Committee will be
presided over by the Project Manager or his Deputy as the Chairman of the Safety
Committee. The secretary of the Safety Committee will be the HSE Officer / Manager.
The Employer’s representative shall be invited as an observer for the Safety
Committee Meeting.

5.1.4 Duties and Responsibilities


a) Chairman
To summon and preside over the Kunal Structure India Pvt Ltd.. Safety Committee to
attain the purpose planned herein. He has absolute authority over members for
improvement of safety control.

b) Secretary
 To organize the Safety Committee and prepare minutes of meeting.
 To present to the Committee the status of inspection reports and incident
investigations.
 To prepare / coordinate the Monthly Safety Report
 In the absence of the Chairman, preside over the meeting.

5.1.6. Enforcement of Resolution

The Chairman shall inform members of any outstanding issues in the meeting. Member(s)
concerned shall ensure the items identified as needing improvement shall be carried out
within a stipulated period. Failing which shall rendered disciplinary measure determined by
the Chairman.

PROCEDURE OF SAFETY COMMITTEE METING

The Safety Committee Meeting will be held once in month and committee members will do
site inspection after the welcome and observation sheet will be provided to the committee
members to write safety deficiencies on working site.

The objective of the Safety Committee is to assemble persons in particular responsibility for
safety so that they can formally address issues and take appropriate actions in relation to the
achievement of the work site safety management objectives.

The Safety Committee should have a clearly defined functions and responsibilities. Minutes
of decisions taken and follow up actions proposed should be clearly recorded and retained.

6. HSE TRAININGS

6.1 Objective

The objective of HSE Trainings are to equip personnel with the knowledge, skill and
attitude which will enable them to perform their duties in a safe manner.

6.2 Procedure

6.2.1 All workers and supervisors are required to be trained in the potential hazards that
may exist on a construction job site, elimination of hazards and the procedure to be
followed to perform all work safely.

The HSE Officer shall liaise with the Construction execution Department and maintain
a program for the training of workers to meet HSE Plan and statutory regulations in
advance of work.

The HSE Officer shall keep an updated record of the training courses completed by
workers.

6.2.2 Safety Induction Program

All new personnel shall receive an Kunal Structure India Pvt Ltd.. Safety Induction
Course lesson on basic safety requirements of the project and significant features of
the construction work relating to the safety when he or she arrives at site.
The lesson shall be given by Kunal Structure India Pvt Ltd.. HSE Officer or his
subordinates. Documents relating to safety such as safety booklets will be supplied to
the newcomer.
The primary objectives of the induction course are to:
 Stress the importance of HSE
 Teach how to avoid accidents
 Contribute to developing Project Safety
 Guide employee in exercising safe working conditions

It is a mandatory that all employees attend the safety induction course prior to
starting their work. No employee will be permitted to work on the construction
site without having attended the safety induction course. The security pass will
be issued only to the workers who have undergone the First Day safety
Induction course.
The First Day at Work Safety Induction Form will be completed by the participant and
submitted to HSE Dept. for recording purposes. The safety induction course will also
serve to inform all personnel that failure to work safely and follow safe practice will
result in disciplinary action (penalty action) which may include expulsion from the site.
Employees which have attended the safety induction course will be issued with an
Kunal Structure India Pvt Ltd.. Security Pass within 24 hours.

Tool Box Talk

A Tool Box Meeting (TBM) is one of the most effective ways to eliminate accidents. A
TBM is a short meeting of the individual work groups, and is held in the close vicinity
of the work location, prior to start of work. Construction staff and subcontractor’s
supervisors, foremen, and workers shall attend tool box meetings held by Kunal
Structure India Pvt Ltd.. daily each morning.

The TBM is to be recorded in the TBM Record Form, and to be submitted to the HSE
Officer who will maintain the records. The HSE Officer, his deputy or his authorized
safety person will monitor that the meetings are held as required depending on the
work status of the Subcontractor.
Mainly, the working plan and procedures for the day or the week are explained and
confirmed including the following:

Kunal Structure India Pvt Ltd.. HSE Department will issue any particular TBM topics
from time to time which may include matters raised by the employer’s representative
or KUNAL STRUCTURE INDIA PVT LTD.. management, to be addressed to all
personnel on site. KUNAL STRUCTURE INDIA PVT LTD.. HSE Department shall
monitor the conduct of the TBM and report to the Project Manager and Construction
Managers where any particular groups were lacking or failed to conduct the TBM.
Corrective action, which shall be determined by the Project Manager, will be taken on
the group concern to improve the performance.

6.2.3 Safety Training for Managers


Management training is intended to provide the knowledge, motivation, and skill
necessary to manage the safety and health /loss control programme. Safety I/C in
consultation with Project Manager / Dy. Project Manager will identify who should
attend a particular course relevant to his/her work.

6.2.4 Trade and Skill Training


Safety I/C, in consultation with the Project Manager / Dy. Project Manager, will
determine that a job holder is competent to do his job safely and ensure that
appropriate training is given. Trade and skill training that is required in the
construction industries are:
 Lifting Supervisor
 Crane operator
 Scaffold Supervisors
 Excavation and Blasting Operations (if any)
 Electrical Workers
 First Aid Training
 Emergency Preparedness Training.

LTD.. or in conjunction with the relevant body or organization.

6.3 SAFETY PROMOTION

6.3.1 Objective

The objective of safety promotion is to develop and maintain awareness among all
personnel, of the work sites commitment to safety and of the individual’s responsibility
to support that commitment.

6.3.2 Procedure

6.3.2.1 Safety Notice Boards

KUNAL STRUCTURE INDIA PVT LTD.. will provide Safety Notice Boards that display
information such as Safety Inspection Result, Safety memorandum, Accident Report,
Safety posters.

 Posters and Signs

Posters and signs shall be adopted as visual aids for accident and fire prevention.
Posters shall be written in English and Hindi and will be conspicuously displayed.

 Safety handbooks and brochures

To increase safety awareness and as part of personnel safety training, safety


handbooks or brochures will be issued.

 Accident Statistic Board

KUNAL STRUCTURE INDIA PVT LTD.. will provide and maintain an Accident
Statistic Board 3m x 2m in size written in English and in Hindi. The content of which
shall include:

a) Date;
b) Total Man-hours Worked;
c) Total Lost Time Accident;
d) Total Fatalities;
e) Total Damaged Incident;
f) Frequency Rate;
g) Severity Rate;
The board shall be erected at the main entrance to the Site and/or at other location
determined by the employer’s representative and KSIPL's Management. KSIPL's HSE
Department will be responsible in ensuring that the statistic board is updated to reflect
the safety performance of the site.

 Safety Talks or Screening of Video on Safety

KUNAL STRUCTURE INDIA PVT LTD.. HSE Department will conduct a Safety Talk
(minimum weekly) to the workforce or screening of video on safety so as to promote
the safety and health of workers in the site. The topic of the Safety Talk or screening
of video will be decided by the KUNAL STRUCTURE INDIA PVT LTD.. HSE
Department. KUNAL STRUCTURE INDIA PVT LTD.. will decide the category and
type of worker which require to attend for the talk. Record of attendance will be kept
by the HSE Department. Respective KUNAL STRUCTURE INDIA PVT LTD..
Department Head and Subcontractors Management shall ensure the attendance of
these personnel.

 Safety Incentive Program

KUNAL STRUCTURE INDIA PVT LTD.. will develop a procedure to recognize and
acknowledge good safety performance by individuals, teams or the subcontractors.
The development of the Safety Incentive Program which includes how it should run

and types of awards will be developed with the consensus of the KUNAL
STRUCTURE INDIA PVT LTD.. Safety Committee. The Safety Incentive Program will
be implemented within six months after the inauguration of the KUNAL STRUCTURE
INDIA PVT LTD.. Safety Committee.

 Penalties and Fines

KUNAL STRUCTURE INDIA PVT LTD.. has implemented a policy whereby persons
found violating safety rules are subject to fines. Persons are warned in advance and
given the opportunity to correct their infractions. Persons with poor records of overall
safety performance found to be in violation of safety rules and who fail to correct their
offenses when verbally instructed to do so, will be fined.

KUNAL STRUCTURE INDIA PVT LTD..’s safety staff can provide additional
information on which violations involve fines. Note that all funds collected through
safety violation fines are channeled back into and used for safety promotion activities.
a) Fines will be assessed for safety offenses and infractions.
b) The fines will be deducted from the progress payments for the subcontractor’s
work.
c) The HSE Officer shall inform workers of safety offenses and infractions that
are subject to fines.

7.SUBCONTRACTOR EVALUATION, SELECTION AND


CONTROL

7.1 INTRODUCTION
The purpose of this Guide is to provide standards for the evaluation of all the
subcontractors to be appointed for Project with respect to their ability to carry out work
in conformance with Kunal Structure India Pvt Ltd.. HSE Plan.

7.2 SCOPE
This procedure is intended for use in the selection and installation of subcontractors.
The Project Manager or any other persons appointed by him will be responsible for
undertaking this evaluation and selection.

7.3 PROCEDURE
7.3.1 Safety Requirement
KUNAL STRUCTURE INDIA PVT LTD..’s HSE Plan shall be made available to
subcontractors bidding work to KUNAL STRUCTURE INDIA PVT LTD.., and bidding
instructions shall include requirements that subcontractors comply with the HSE Plan.
In addition, KUNAL STRUCTURE INDIA PVT LTD.. may add any special requirement
for the Project to the above requirements.

In the tendering process the subcontractor shall include in its cost estimating all the
items necessary for it to comply with both the general and specific HSE requirements.

7.3.2 Collection of Past Information


Prior to selecting subcontractors, the project shall collect information on
subcontractors, which have worked for K u n a l S t r u c t u r e I n d i a P v t L t d . . in the
past. The Project Manager or Construction Manager will collect and assess the
Subcontractors safety performance at the previous projects.

7.3.3 Evaluation on Safety Management


KUNAL STRUCTURE INDIA PVT LTD.. shall evaluate the safety management
capability of subcontractors using the information and the Inquiry mentioned above.

7.3.4 Selection of Sub-Contractor

The project shall select a subcontract after evaluating their proposal taking into
consideration its cost, quality, schedule and safety.
It is recognize that it is not simple to apply quantitative weighting to these
considerations. However the project shall avoid awarding of work to a lowest bidder
without regard to their HSE Management Plan and performance record.
At the time of selecting a subcontractor, the subcontractor will be informed that in the
event that they elects to subcontract out part of its work to others, he shall ensure that
its own subcontractors:

a) Are aware of and understand their safety obligations described in Kunal


Structure India Pvt Ltd..’s HSE Plan.
b) Have the capacity and ability to perform the work in conformance with KUNAL
STRUCTURE INDIA PVT LTD..’s HSE Plan.
c) Perform their work in compliance with KUNAL STRUCTURE INDIA PVT
LTD..’s HSE Plan. The subcontractor shall be fully responsible for the actions
of its selected subcontractors.
d) “All HSE Specifications shall full fill in “Sub-Contractor HSE
Requirements” FORM by the Sub-contractors before
Commencement of work.

8. HSE INSPECTION

8.1. Purpose

To ensure that all HSE requirements are observed and any unsafe acts/conditions are
immediately corrected.

8.2 Objective

The objectives of the site audit/patrol are:

1. To ensure that workers carry out safe work practices during the execution of their
tasks in accordance with safety requirements;
2. To check and correct immediately any unsafe acts/conditions observed;
3. To maintain a register of daily site safety inspection checklist for audit purposes;
4. To complement preventative maintenance progress;
5. To demonstrate management commitment to safety.

8.3 Reference

Site Safety Inspection Checklists are to be provided after as part of HSE Plan.

8.4. Procedure

8.4.1 Safety Audit


In compliance with the requirements, KUNAL STRUCTURE INDIA PVT LTD.. will
carry out audits on the safety management system of the site. The audit will be
carried out once every three months. KUNAL STRUCTURE INDIA PVT LTD.. will
prepare an audit report and take action on the corrective measures as recommended
by the auditors and will submit the action plan to the employer’s representative within
2 weeks time.

8.4.2. KUNAL STRUCTURE INDIA PVT LTD.. Safety Committee Site Safety Inspection

The Safety Committee Chairman will lead the Committee to inspect the site at least
one week before the monthly safety meeting. The Safety Committee Chairman will
decide when the inspection to be carried out and notify to all Safety Committee
Members. The route of the safety inspection will be decided on after all members
have congregated. The employer’s representative shall be invited as an observer in
the site safety inspection.

Following the inspection, the Safety Officer shall furnish a status report to the
Committee at the monthly safety committee meeting to discuss the observation made
by members during the inspection.

8.4.3. HSE Officer’s Weekly Safety Inspection


The HSE Officer shall lead the Weekly Safety Inspection. The Weekly Safety
Inspection team shall consist of KUNAL STRUCTURE INDIA PVT LTD.. and
Subcontractor’s Supervisors.

Attention will be paid to identifying and resolving imminent and potential hazards and
to confirm compliance with safety rules and regulations.

All deviations shall be rectified within the agreed period. Reports will be submitted to
the Project Manager, Construction Managers and Site Supervisor in charge of the
applicable work, and all respective Subcontractors. These reports shall be used in the

Construction /Progress Coordination Meetings to discuss and confirmed that


deviations are rectified within the agreed period.

8.4.4 Daily Safety Patrol

 Site Engineers/supervisors/foremen in-charge of specific work areas shall conduct


daily safety patrols to check for any unsafe acts/conditions and, if observed, to
order the relevant subcontractor/workers to take immediate corrective actions
(Site Safety Inspection Checklist)
 The HSE Officer shall monitor the daily safety patrol activities and ensure that
they are carried out responsibly and correctly.
 Supervisors/foremen are responsible for preparing and submitting observation
and daily safety reports to the HSE Officer for his review. The latter shall
comment and make recommendations to correct any reported unsafe
acts/conditions.
 The HSE Officer shall summarize all the supervisors/foreman's daily safety reports
into an overall safety report and submit it to the Project Manager, Dy. Project
Manager, Construction Manager who will ensure that corrective actions for unsafe
acts/conditions are carried out.

9. INCIDENT INVESTIGATION AND ANALYSIS

9.1 Purpose

To ensure that every accident and incident is

 Handled expeditiously, diligently, and with the utmost care;


 Investigated thoroughly to determine its cause;
 Corrective and preventive measures are implemented to prevent recurrence of
similar accident.

9.2. Objective

The objectives of this procedure are:

i) To establish a clear and efficient system for the reporting and handling of
accident, incident and near misses;
ii) To ensure that all accident, incident and near misses are thoroughly
investigated, their causes determined and the appropriate accident reports
generated for statutory submission and record purposes; and
iii) To ensure that corrective and preventive actions are taken to avoid recurrence
of similar accidents.

9.3 Definitions
Incident:
Any undesired condition that results or could have resulted in harm to people,
damage to property, the environment, or loss of production. This includes “near miss”,
“accidents’ and “dangerous occurrences” as defined under the Factories Act.
Accident:
An incident involving actual loss through injury, damage to assets, or environment
harm.

Major Incident
The criteria for major incidents are as follows:
1. Any fatality associated with the works. Serious injuries requiring hospitalization
in excess of 24 hours, unless detained solely for observation.
2. Major stoppage of work for more than 24 hours.
3. Property damage in excess of Rs. 100,000.
4. Accident that result or likely to result in negative media coverage.
5. Near misses with a high potential to result in any of the above losses.

Reportable Injury (or) Lost Time Injury

1. Fracture of any bones


2. The loss of sight or an eye
3. Any other injury, excluding occupational disease, which results in the person being
hospitalized for more than 48 hours, unless detained solely for observation.

Minor Injury (or) First Aid Case


An injury which does not cause the injured person to lose a normal job time beyond
the day or shift upon which the injury occurs.

9.4 Procedure
9.4.1. Upon witnessing/discovering of an incident, the person(s) involved shall report it to the
Duty Supervisor/Foreman who shall in turn inform the HSE Officer or his appointed
representative. If the Duty Supervisor/Foreman is injured they should report directly to
the HSE Officer/ his appointed representative. If the HSE Officer/ his appointed
representative is injured the Foreman should report to the Construction Manager/ Dy.
Project Manager/ Project Manager.

Reporting of Incidents

Persons witnessing an incident, accident, or near miss occurrence shall report same
to the HSE Officer/ his appointed representative. In the event of an incident, accident,
or near miss, persons on site shall assist safety investigators by providing accurate
information in a timely manner. Persons found to have intentionally withheld
requested information during a safety investigation will be subject to disciplinary
action that may include dismissal from the Project”

9.4.2 The HSE Officer/ his appointed representative and Duty Supervisor/ Foreman/ First
Aider shall administer first aid treatment to the injured person(s). If the injuries are
minor, the HSE Officer/ his appointed representative shall arrange for the injured to
see a doctor. For major injuries, the HSE Officer/ his appointed representative shall
call an ambulance and inform the next-of-kin, statutory bodies and, if necessary, the
Police. The Duty Supervisor/Foreman shall accompany the injured person(s) to the
hospital.

9.4.3. If the information is given to the HSE Officers appointed representative, he will inform
the HSE Officer immediately. The HSE Officer shall inform the Project Manager of the
accident as soon as possible. If the accident resulted in a major incident or serious
injury then the Project Manager shall in turn report to the employer’s representative.

9.4.4. For a major incident or serious injury, the Safety Committee Members shall assemble
to investigate the causes of the accident. The Duty Supervisor/Foreman shall write an
accident report within 24 hours of the accident occurrence and submit it to the Project
Manager and Safety Officer. (Accident / Incident Report Form).

9.4.5. The Project Manager shall organize an extra-ordinary meeting of the Safety
Committee to discuss the findings of their investigation and propose corrective and
preventive measures to prevent recurrence of similar accident. All
supervisors/foremen shall inform all workers of these measures and implement them.

9.4.6. The HSE Officer shall complete the Accident/Incident Report and submit to the
authorities within 24 hours and compile the monthly accident statistics. For major
accident, he shall submit a copy of the report to the statutory bodies and the
Employer’s representative as instructed by the Project Manager.

9.4.7. The HSE Officer shall investigate all the Dangerous Occurrences/ Accidents/
Incidents as per the statutory requirement.

10. OCCUPATIONAL HEALTH MEASURES

KUNAL STRUCTURE INDIA PVT LTD. will ensure at a construction site of a building or other
construction work that an arrangement is made with a nearby hospital for providing such an
ambulance van for transportation of serous case or accident or sickness of workers to
hospital promptly.

A) FIRST AID BOXES

1. Portable first aid boxes will be maintained fully equipped at each local offices and
work location where 100 or more persons work together.
2. In each site office and location one employee, suitable trained in first aid, will be
available at all working hours for the purpose of attending to emergencies.

3. Every First-Aid box is distinctly marked “First-Aid”

B) HIV/AIDS PREVENTATION AND CONTROL

1. KUNAL STRUCTURE INDIA PVT LTD..- will adopt the Employee Policy on HIV/AIDS
2. Prevention and Control for Workman engaged in site
3. The peer educators on completion of the training will serve as a focal point for any
information, education and awareness campaign among the workmen throughout the
contract period.

C) PREVANTION OF MOSQUITO BREEDING

1. Empty cans, oil drums, packing and other receptacles, which may retain water will
be deposited at a central collection point and will be removed from the site
regularly.
2. Still water will be treated once every week with oil in order to prevent mosquito
breeding.
3. Posters in Hindi and English, which draw attention to the dangerous breeding by
fumigating/spraying of AMO (anti mosquito oil)
4. KUNAL STRUCTURE INDIA PVT LTD.. will ensure at all times that no employee
is working under the influence of alcohol/drugs, which are punishable under Govt.
regulations.
5 Smoking is strictly prohibited at work sites.

D) OCCUPATIONAL HEALTH HAZARD

1. Occupational Health Hazard” is the one which becomes chronic in


nature, over a period of time, due to continuous exposure of personnel
to operations / situations such as handling chemicals, working in noisy
areas, ergonomic reasons, etc.
2. For effective management of occupational health risks at work, it is
essential to identify inventories - the potential physical, chemical,
biological, ergonomic and psychological health hazards.

3. Essential data shall be collected and evaluated for the risks associated
with occupational health and there after preventive / corrective measures
are decided which shall be incorporated to avoid the risks.

4. Necessary Personal Protective Equipment (PPE) shall be provided to the


supervisors and crafts that are associated with jobs involving occupational
health risks.

11. LABOUR WELFARE MEASURES

 KUNAL STRUCTURE INDIA PVT LTD.. will ensure that a number of toilets are made
available at the work site with the ratio being no less than one toilet for 50 workers or
part thereof. The toilet will be located so that person does not have to walk more than
five hundred meters to use them.
 The toilets will have adequate water supply and be kept in a clean and tidy condition
at all times.
 Drinking water, washing facilities, latrines, first aid and medical facilities will be
provided at labour camps.
 Security and Portable Fire- fighting equipment will be arranged.
 Electrification, proper illumination will be arranged.
 Camps will be kept clean and tidy.
 Frequent health checkups and awareness programs will be arranged.
 Frequent inspections will be carried.

12. HAZARD ANALYSIS AND MANAGEMENT

The objective of hazards analysis and management is to provide a means whereby hazards
or potential hazards are identified and managed in a way that eliminates or reduces to an
acceptable level, the risk of an incident occurring.

Should establish and maintain a programme for the identification and assessment of hazards
(Risk Assessment). This programme should be aimed at:

a) Recording of known hazards.

b) Identification of new hazards.


c) Analysis of the effects or the potential effects resulting from these hazards.
d) Development and implementation of means to eliminate the hazard or to manage
the hazard in a way that represents on acceptably low risk or incident.

Should ensure that persons responsible for the analysis of hazards and for determining the
means of eliminating or reducing any safety risks are:
a) Technically competent.
b) Given the necessary support so that they can effectively perform their duties.

12.1 CONTROL OF MOVEMENT AND USE OF HAZARDOUS SUBSTANCES

The objective is to provide a means whereby the receipt, movement, storage and use of
hazardous material is managed in a way that eliminates, or reduces to an unacceptably low
level, the risk of an incident occurring

Every work site should maintain a register of hazardous material complied from material
safety data Sheets (MSDS) or from other analysis. The register should show:

A) The composition, physical and chemical characteristics of the material.


B) Safeguards to be applied in the transportation, storage, handling and use of occurring
in the handling or use of the material.

Should ensure that there is controlled access of hazardous material to the work site premises
sufficient to ensure:
i. that the movement of hazardous material is known and recorded.
ii. that a technically competent person can receive the material and ensure its safe
storage or distribution.
iii. that the material is correctly labeled

Designated storage areas for hazardous material should be appropriate for the material and
should be secure against unauthorized access.
Should ensure that personnel involved with the storage, handling and use of hazardous
materials are competent and are fully aware of the safeguards and measures.

13. SAFE WORK PRACTICES AND PROCEDURES

13.1 EXCAVATION :
1. No excavation works shall carryout first without notifying to the relevant authority and
getting permission.
2. Sides of all excavations must be slopped to a safe angle not steeper than the angle
of repose of the particular soil. If it is not possible to give a proper slope at the sides
of excavation, where there is a danger of fall or dislodgment of earth or any material
shall be securely supported by timber or any other type of shoring. Where the exca-
vation is being carried out with ‘Poclain’, step down procedure should be followed.
3. No excavation or earthwork below the foundation level of any adjoining building in an
existing plant / factory shall be taken up unless adequate steps are taken to prevent
damage to the existing structure.
4. Every accessible part of an excavation, pit or opening in the ground into which there is
a danger of person falling, shall be suitably fenced with a barrier upto a height of three
feet as close to the edge of the excavation as practicable. Suitable warning signs are
also to be displayed.
5. No material or load shall be placed or stacked near the edge of the excavation or
opening in the ground. The excavated material shall not be placed within 1.5 mtrs of
the trench or half the depth of the trench whichever is more.
6. Cutting shall be done from top to bottom. No undercutting of side of excavation shall
be allowed.

7. All narrow trenches 4 ft or more deep shall at all times be supplied with at least one
ladder for each 100 ft in length or fraction thereof. Ladder should be extended from
bottom of the trench to at least three ft., above the surface of the ground.
8. The side of trenches which are 5 ft. or more in depth shall be stepped back to give
suitable slope or securely held by planking, strutting and bracing, so as to avoid the
danger of side collapse.
9. Before starting any excavation of any description in the existing plant/factory area
premises, permission in writing (WORK PERMIT) must be obtained from Electrical Di-
vision, Civil Engineering and Design Department of the clients to avoid any damage to
the underground electric cables or pipelines.
10. Any excavation or ditch more than 5 feet deep must be properly shored if angle of re-
pose is not given, before any worker is permitted to work in it. A competent person
shall inspect all timber and planks used there in. Undercutting of banks shall not be
permitted. Erosion of soil over excavated pits, trenches must be prevented from run-
ning water by de-watering pumps, etc.
11. Excavation inside any part of the existing plant / factory must be properly fenced and
marked with suitable warning boards or lights at all times. This also applies to any
trench or drain which has its cover removed.
12. No loose material or load shall be placed or stacked near the edge of any excavation
so as to endanger the lives of person working below.
13. When it is necessary to block off any road inside the existing plant or factory
premises, proper road-barrier must always be used and existing rules of clients to be
followed.
14. Pick and shovel people working in excavations should be kept far enough apart to
prevent injury to one another.
15. All workmen working inside the pit shall wear the necessary Personal Protective
Equipments (PPEs).

13.2 WORKING AT HEIGHT - Introduction


Fall from height are the most common cause of fatal injury. The common cause of
falls from height includes :
(a) Failure to recognize the risk/problem
(b) Failure to ensure that safe system of work are put in place.
(c) Failure to provide adequate information, instruction, training or supervision.

(d) Failure to use appropriate equipment.


(e) Failure to provide safe plant/equipment

13.3 Guidelines for Scaffolds

13.3.1 General Scaffolds :

1. In the erection, maintenance and subsequent dismantling of scaffolding, all the


regulations and acts shall be adhered to. In general, it is required that the scaffold
shall be firstly of adequate strength stability, good construction and material and
secondly that the related permanent structures shall be at all stages of construction
be safely supported against collapse. All scaffold materials shall be free from patent
defects.
2. The works supervisor shall assign a competent approved person (CAP) to the site to
supervise all aspects related to the erection, maintenance and dismantling of scaffold.
3. The scaffolding erectors should use safety lines to anchor their body harness in the
process of erection and dismantling. These safety lines should be securely anchored
onto an expansion bolt anchored on to the roof of the building or to any other
adequately strong anchorage.
4. Scaffold erectors should not be allowed to work on scaffold without the use of body
harness, safety lines and helmets.
5. Handrails and intermediate guard rails shall be provided along each ledger level
approximately 1.1m and 600 mm above each ledge respectively. Where safe access
to the scaffold from the building or structure is not available, ladders should be
provided. This is to avoid workers having to scale a scaffold. Toe boards should also
be added onto platforms on scaffolds to prevent materials falling over it.
6. Materials should not be allowed to be discharged from the side of the building and
hitting the scaffold structure as serious damage such as broken ledgers and dislodged
tie-backs could result.
7. Scaffolds should not be used as supports for hoisting materials unless it is designed
for and approved.
8. During scaffold erection, the site supervisor (CAP) shall carefully keep watch, provide
"NO ENTRY" and "DANGER OVERHEAD" signs, and if possible rope off the related
work area for safety and provide warning lights for night works.

9. All scaffolding that are in the process of erection, alteration and dismantling shall be
properly indicated and warned to make sure that no workers will use such scaffold.
Access to the scaffolds can only be allowed, after it is fully completed and checked.
All scaffolding structure built must display either:
 A tag indicating that the scaffolds is "NOT SAFE FOR USE" if it is incomplete, has
been damaged or weakened.
Or
 A tag indicating that the scaffolds are "SAFE FOR USE" if the works are
completed checked for safety and certified.
10. Excess materials shall not be placed on the work floor or runway, avoid storage or
deposit materials on the working platform.
11. Materials, tools and equipment which may be displaced or fall from the scaffold shall
be secured adequately.
12. Barricade all open sides of scaffold with proper guardrails and replace all missing or
damaged guardrails.
13. All working platform shall be fully and properly boarded.
14. Provide proper ladders or steps for workers to gain access from one level or another.
15. All other general safety requirement for site work shall be observed.

13.3.2 Suspended Scaffolds :


1. Erection of any suspended scaffold shall comply with standard scaffold practice.
2. Workers should be fully instructed as to the correct operations of the suspended
scaffold. They should be adequately supervised whenever practicable.
3. All workers working on a suspended scaffold shall be provided with body harness.
4. All workers working on a suspended scaffold shall be instructed to wear safety
helmets.
5. No work shall be carried out in a suspended scaffold during inclement weather.
6. The weight of men, work materials and components mounted on the scaffold must not
exceed the manufacturer's rated loads.
7. A systematic and routine maintenance program to be maintained.

13.3.3 Scaffolding Personnel :

Scaffolds must only be erected, altered or dismantled under close supervision by a


competent person. The competent person (Station Manager/Safety Engineer) shall
approve and fix the tags.

13.3.4 Materials Storage and Handling :


The following points should be noted for handling and storing materials, it is by no
means exhaustive, and every possible precaution shall be taken to upkeep the quality
and reliability of the materials.

1. All scaffolding members and gears should be carefully hoisted up or lowered;


this will prevent them from being damaged.
2. Pallets should be of adequate strength and the platforms rose above 150mm
above ground. This will prevent the scaffolding members from coming into
contact with the ground thus preventing damp and rust.
3. Pallets provide with mesh guards or proper boxes should be used for storing
scaffolding fittings.
4. Timber scaffolding boards should be bounded with galvanized steel straps at
both ends to prevent splitting.
5. Timber scaffolding boards should be stacked on dun age and maybe stacked
on top of each other up to a height of about 1.2m. The top most boards
should be weighted down to prevent warp. It should be stacked in a way that
they cannot fall unexpectedly or shaken loose.
6. All materials should be placed on pre-assigned space and not left on the aisle.

13.3.5 Inspection and Record :


All scaffolds shall not be used unless it has been registered and subsequently
inspected by a competent person within the previous seven (7) days and a
report made out on the specified report form attached. For suspended scaffold daily
pre-operation check is required and a thorough check at least once in every twelve
months by a competent person.

Scaffolds on most sites are used fairly constantly so that inspection is required to be
carried out every seven days and the records should be kept at site for ready
inspection by various interested parties such as employer’s representative, safety

committee members, etc. Inspection of all scaffolding is advisable, immediately after


a major storm or strong wind occurrence at the site.

13.3.6 Work at Elevated Places :


1. Scaffolds, ramps, ladders and platforms shall be provided for all work to be
done 2m or more above the ground.
2. Erecting, replacing and dismantling of scaffolds, ramps and platforms shall be
directed by the main contractor or the sub-contractor's qualified construction
supervisors and shall be erected and supervised by competent personnel.
3. Unauthorized person(s) shall be prohibited from entering the construction area
of scaffoldings, ramps and platforms. Safety ropes and caution signs like "NO
ENTRY" and "DANGER OVERHEAD" shall be displayed during construction.
4. Scaffolds, ramps and platforms shall be kept free from grease, mud or any
other material or equipment.
5. Erected scaffoldings shall be inspected by competent supervisors once
in every 7 days and the result of inspection shall be recorded on the safety
tags. A register of all scaffolds shall be listed and updated regularly by
Contractor/ sub-contractors competent persons.
6. A record defining safety status of scaffolds and name of person responsible for
scaffolds, ramps and platforms shall be displayed.
7. Wooden materials used for construction of ramps, platforms and runways shall
be of good quality. Synthetic or natural fiber ropes or other similar flexible
materials shall not be used as guardrails.
8. Safety nets shall be provided where scaffolds are not feasible or unavailable,
such as when assembling pipe rack and structure, etc. In such cases, the
workers shall always use body harness and anchor at a secure point.
9. All opening and ends of platforms located 2m or more above the ground shall
be barricaded or covered.

10. All planking/scaffolds, ramps and platforms shall be inspected regularly every
week and any damaged or unsafe portions shall be repaired or replaced
immediately.
11. There shall be a landing platform at every 4 mtrs. Interval.
12. Loose materials such as unused bolts, nuts, rivets, debris etc. shall not be
allowed to accumulate on the work platforms.

13.3.7 Erection of Scaffold :


1. All scaffoldings erected at the job site shall comply with the applicable Indian
standards.
2. No light duty scaffold parts/fittings to be used for this project unless otherwise
agreed.

13.3.8 Platform :

Safe working platform shall be provided for persons to work at height. Proper
means of access (such as stairs, ladder, and ramps) must be provided.

The working platform shall be tagged with the safe load (in terms of number of
persons) that can be carried out by the platform.

13.3.9 Safety Net :


Safety nets used at the construction site shall comply with acceptable standards i.e.,
IS 11057.

13.3.10 Safety harness/fall protection :


1. Where it is not possible to provide a safe working platform then the use of safety
harness will be considered:

3. Proper anchorage that the harness is attached to be sufficient strength to
support the falling body weight of the worker
13.3.11 Ladders :
Ladders are commonly used at site. Frequent accident occurs from workers falling
from ladders. When using ladder the following should be followed:
1. Use clear varnish to protect ladders; paint may cover a defect.

2. Be sure the ladders are set on a firm level base.


3. Have a man at the spot or lash the top.
4. Make certain the ladder reaches at least 1 m above landing platform.
5. The correct pitch of a ladder is 0.3 m out at the base for every 1.2 m vertical
height.
6. Use the right length ladder for the job. Never lash two short ladders to make it
longer.
7. Do not carry loads on ladders - use a hoist line.
8. Do not lean sideways from a ladder - it’s safer to move the ladder.
9. Face the ladder when climbing or descending.
10. Beware of wet or greasy rungs.
11. Inspect ladders before use and regularly when stored.
12. It should be of sound construction and free from patent defect.
13. Metal ladders should not be used near or adjacent to overhead power lines.
14. Not to be used if any rung is missing.
15. Not to be used for any other purpose than to provide access.
16. Applicable colour codes shall be painted on the side rail for every quarterly
after thorough inspection (green, blue, yellow & red).

13.4 SHUTTERING WORKS (Form Works) :

1. Refer Method of Statement before commencement of works.


2. Deploy competent person for form-work and for scaffold.
3. Use sound planks and should be tied during scaffold erection and decking.
4. Provide proper access to reach the scaffold platforms.
5. Provide lifeline at the edge; minimum thickness of wire rope should be 8 mm or
PP Rope of 16 mm worker while working at the edges should wear full body har-
ness and should hook his lanyard to the lifeline (wire rope tied).
6. Clean water / oil spillages, Saw dust immediately after completion of work.
7. Keep the edge clear.
1. Ensure condition of the PPEs (Shoes, helmet) is good before starting the job.
2. Provide planks on formwork for walking.
3. Plywoods and other materials should be unloaded on the safe accessible place.
4. Workers supplying the plywoods should not go to the edge other wise they should

use safety harness.


5. Train all the concern person about the hazards related to formwork.
6. Ensure proper edge protection - immediately after placing the plywood.(Hand rail,
mid rail and toe guards)
7. Avoid using loose clothes.
8. No work to be carried out below the area & area must be cordoned off.
9. Avoid manual handling during wall and column shuttering.
10. Follow kinetic method of material handling.
11. Conduct training on material handling.
12. Use PPE’s such as (Hand gloves, helmet safety shoe, goggles)
13. Ensure thorough housekeeping and proper material stacking throughout the
workplace.
14. Provide adequate manpower
15. Ensure adequate lighting arrangements (Minimum 50 Lux).
16. Ensure proper communication while inserting tie rods between the workmen
17. Engage workmen on top who can observe both sides so that injury due piercing
should be avoided.
18. Stack on the leveled and firm surface.
19. Avoid over stacking (max-3 levels)
20. Ensure stacking doesn’t obstruct the access.
21. Ensure safe distance is maintained for lifting by proper means.
22. Stack shuttering material considering the sizes i.e. do not stack big items on
small items.
23. During lifting from stacks, provide adequate manpower and tag line to control
the movement of shuttering material being lifted.
13.5 CONCRETE POURING WORKS :

13.5.1 Form-work for concreting:


1. Shuttering and supporting members viz. props, tie rods, etc. shall be of
adequate strength to support the load / pressure of concrete and the form-work
designs shall be approved and checked in advance.
2. The area shall be suitably cordoned / barricaded and unauthorized entry shall be
restricted by displaying signboards, etc.

13.5.2 Concrete Pouring:

1. Manual handling of concrete shall be restricted as far as possible. PPE like


hand gloves, gumboots, goggles etc. shall be provided.

2. The out riggers / wheels of concrete pump / concrete mixer / Boom placer shall be
placed on firm ground / platform. Pump accessories shall be checked for its safe
working pressure considering maximum pipe line height.
3. The pipeline for transporting the concrete shall have the shortest route with
minimum bends and shall be installed on firm supports at suitable intervals.
Pipeline shall be properly joined with clamps and securely tied to nearby support
and checked in advance before starting the concreting. Pipe segments shall be
cleaned in advance to avoid choking of concrete during casting.
4. Ball catcher / Trap to arrest the ball must be provided at the end of the pipe line
after the concreting is over and flexible pipe is removed. The supervisor, who is
authorized to give clearance for ball passing, shall check that whether the ball
catcher / trap is fixed properly before passing the ball. The supervisor shall
instruct all workers to keep safe distance during ball passing
5. Proper signaling system shall be followed.

6. All mechanical equipment / tools used in concreting activity like batching


plant / concrete mixer, concrete pumps, vibrators, etc. shall be operated by
trained person only.
7. Ready Mix Concreting - Loaded transit mixers shall move / park on firm ground
as far as possible. The reversal of transit mixers shall be guided by helpers and
reverse horn shall be used for reversal. Cleaning/washing of transit mixers shall
be done at designated area only.
8. Traffic management plan shall be followed for transit mixer vehicles ply.

13.6 SAFE WORK PRACTICES - HOISTING, LIFTING, ERECTION

13.6.1 Lifting Equipment


When using lifting equipment:
1. Know the correct weight of the load before it is lifted.
2. Do not operate if you are not trained.
3. Never overload when lifting materials. Keep to its safe working load (SWL)

4. Examine lifting equipment before use. Don’t lift if the load is not secured.
5. Do not stand or walk under a suspended load.
6. Crane operators found tempering with any safety devices shall be dismissed from
site immediately.

13.6.2 Erection Work :


1. Erection plan and procedure shall be prepared and the same shall be
checked thoroughly by construction supervisor.
2. Warning signs of “NO ENTRY” and safety ropes shall be provided by work
supervisors.
3. The strength of the road on the route of the crane shall be checked by the
Engineer. The supervisor shall check for any obstacles on the route of the
crane.
4. All cranes, lifting gears and winches shall be tested and inspected half yearly,
The valid certificate shall be displayed on the lifting machines. Cranes or
winches shall be locked or braked when not in operation.
5. Subcontractors performing work requiring daily/regular lifting operations—such
as piling, decking, erection of major plant-- shall provide a person competent
in lifting operations who will be appointed by KUNAL STRUCTURE INDIA PVT
LTD.. using Appointment Letter as the lifting supervisor for the subcontractor’s
work .

13.6.3 Erection by Crane :

1. Lifting work shall begin only after confirming by preliminary check that the
crane is set up satisfactorily. The crane access and its position must be
checked for stability to prevent crane form toppling. Site Engineer must check
and ensure that the load bearing of the ground is adequate for the lifting work.
If the crane will be positioned a long period of time, the appointed Site
Engineer must carry out daily check to ensure no deterioration of the ground
condition.
2. The crane shall be secured horizontally and steel plates or square timbers
shall be placed under the outriggers firmly, and then secured with knock pins
after being set in position.
3. The operator shall not leave the crane or winch during lifting work.

4. The inclined angle of the boom during operation shall not exceed the range of
30 - 80 degrees unless otherwise specified for the machinery. When using the
jib its length must be minimized. An angle indicator shall be provided for the
crane operator to visually check the boom angle. Crane booms shall be
lowered to the ground level, and the hook shall be secured to the specified
position when the crane is not in use.
5. When moving cranes, the boom must be lowered and a boom walker must be
provided in addition to the crane operator. Crane boom must not be operated
closer than 2m to any overhead line or electric transmission wire. When it is
absolutely necessary to operate closer than 2m, special permission must be
secured from the Employer's representative and arrangements made to cover
or de-energize the circuit line.
6. Only authorized persons with a license/training shall be permitted to operate
the crane or do slinging for lifting equipment.
7. The strength of the ground where the crane is to be placed shall be examined.
If necessary, reinforcements, such as installation of steel plate shall be
arranged.
8. The foreman and signalman shall be assigned for each erection work under an
operating system established for the work. They shall stand where they can
observe the loading operation and be clearly visible to the crane operator
during the crane work. A uniform signal system shall be used for flags,
transceivers or hands for signaling.
9. The capacity of the crane to be used for erection shall be determined after
careful consideration.
10. Total lifting weights and the center of gravity of equipment to be erected shall
be carefully re-checked before commencing erection work.
11. Load indicators shall be installed on the cranes.
12. Before starting the work, the condition and functioning of the brakes, limit
switches, over hoist prevention devices, wire ropes and lifting devices shall be
checked and inspected and the crane shall be operated on a trial basis without
a load. As a preliminary check, the equipment shall be lifted and held 10cm
from the ground. In this state all elements shall be checked and inspected to
see if they are functioning properly. If risky conditions are detected, the lifting
work shall stop immediately.

13. Lifting work shall not be carried out during bad weather, such as strong winds
or heavy rains.
14. Crane shall never be loaded in excess of the manufacturer's stipulated rating.
Lifting load for each crane shall be controlled within 90% of the maximum
lifting load (to read from load indicator). The lifting load shall include dead load
of lifting, hook, rope, etc.
15. During the lifting work, the operation shall be carefully supervised to prevent
hasty lifting, prolonged suspensions and lifting beyond the limit. Abrupt lifting
and stopping shall be prohibited.
16. Simultaneous rotation and sudden lifting or rotation and boom movement shall
be prohibited.
17. The boom shall be slowly rotated so as not to produce centrifugal forces on
equipment or materials being lifted.
18. Workers shall be forbidden to ride on lifted equipment or material when lifting
or swinging is taking place.
19. The crane's wire rope shall be rolled up entirely after the work is completed.
20. Safe Working Load (SWL) for the crane shall be conspicuously displayed on
the crane body.
21. Crane load chart shall be displayed inside the operator’s cabin in the language
understood by the operator.
22. The crane shall have a valid operating license issued by the competent
authority. The validity of the license shall be of six months.
23. The lifting hooks shall be provided with a safety latch.

13.6.4 Winch Work :

1. The winch shall be installed on firm ground with a good view for the work. The
area where the winch is installed shall have ample space for inspection and
maintenance and be kept clean at all times.
2. The winding drum diameter shall be 20 times the diameter of the wire rope.
The end of the wire shall be wound regularly on the winding drum. Warning
markers shall be attached to the wire rope to prevent over winding.
3. The winch shall be installed securely to prevent floating, slipping and vibrating.
Foot brakes shall be provided with adequate spacing from the ground to
permit the brakes to be operated freely. Gears shall be provided with a cover.

The operating winch shall be located properly for convenient operation. Winch
motors shall be grounded.
4. Winch work shall be performed under the signal of signalman. When winch
work is finished or suspended for a while, the power shall not be switched off.
The winch operator shall not leave his position during winch operations.

13.6.5 Rigging Work :

1. Rigging work shall be performed under the direction of an authorized and


experienced foreman and/or supervisor.
2. The lifting weight shall be the total weight of the equipment or materials, crate
and lifting tackle.
3. Sling wires shall be registered and inspected by competent approved persons.
4. Wire ropes shall be of good quality, free from rust, corrosion and deformation.
Wire rope shall be replaced when either of the following critical condition is
observed: Quantity of broken wires exceeds 10% of the total in one strand.
Reduction in diameter exceeds 7% of the original.
5. Materials shall be lifted by at least 2 ropes and the sling angle shall not exceed
60 degrees.
6. Rigging shall be provided at sharp edges of material to be lifted to prevent the
ropes and slings from being damaged or cut.
7. A guide rope (tag line) shall be provided for extra-long or unstable special
materials to be lifted.
8. Workers riding on lifted materials is strictly prohibited.
9. When lowering materials on the ground, batten shall be placed as required.
10. The hook shall be placed above the center of gravity of the material to be
lifted. When the center of gravity is in the upper half or not in the middle of the
material, special precautions shall be taken to prevent swinging, slipping or
falling.
11. Lifting ropes and slings shall be removed after confirming that the lifted
material is stable.
12. Every sling shall be provided with a dated test certificate giving the, Safe
Working Load (SWL). The SWL shall be displayed.
13. Every sling shall be inspected every three months by a competent person. If
the sling is determined to be acceptable then it will be identified as such by

marking with a color. The color will change for each three month period. If the
sling is determined to be unacceptable for use then it shall be removed from
site or discarded.”
14. Spreader beams shall be used to lift uneven loads. The spreader beams shall
form a part of the lifting gear and should have safe working load clearly
mentioned on it. The spreader shall only be used after testing and certification.

13.6.6 Chain pulley Block :

1. Every chain pulley block shall be tested and thoroughly examined by a competent
person before put to use.
2. Every chain pulley block should be checked for the following before putting in to use:
a) Non - return Mechanism of the ratchet.
b) Hock latches for both the hooks, load hook and hanging hook.
c) Deformation in shape and dimensions of the hooks, and for cracks.
d) Dimensional wear for the chains, and the connectivity of the chains.
e) Fasteners of the body covers.
3. Discard the chains:
a) If the dia of the chain reduces by 5% from its original dia.
b) If the dia reduces by 10% at the end of the chain from its original dia.
4. Lifting shall be made under the supervision of a competent person.
5. No structural alternation or repair shall be made on any part of the chain pulley block
that affects the safety.
6. The maximum safe working load (SWL) shall be marked on the lifting appliance.
7. Operators shall not leave lifting appliances unattended with power on or with a load
suspended.
8. During hoisting operations, effective precautions shall be taken to prevent any person
from standing or passing under the load.
9. No lifting appliance shall be loaded beyond the SWL.

13.6.7 Manual Handling :

1. Whenever practical, certified mechanical lifting devices must be used to assist in


the handling of material in excess of 20 kg.
2. Suitable gloves must be worn when handling materials where contact may be
made with sharp edges, abrasive surfaces, slivers, chemicals, etc.

3. All circular objects must be suitably choked with chokers.


4. All workers required to perform manual lifting tasks shall be instructed on:
5. Back care and correct lifting methods.
6. Hands and feet to be kept clear of pinch points.
7. The position of operators when using hand dogs or pinch bars (i.e. to be such that
slipping tools will not cause falls or injuries).

13.7 BARRICADES :
1. Barriers such as guardrails, hole covers or others must be provided with sign to
protect all workers and public from injury.
2. Excavation and road blockages must be barricaded. Flashing lights should be used
during night time.
3. Excavation deeper than 1.5 meter must have a barrier. In critical area of heavy
vehicular or pedestrian traffic, rigid guardrails, which must not be less than 0.9 m in
height, must be erected around the excavation.
4. All work areas, walkways, platforms etc. elevated 1 meter or more, must be
encompassed by an approved guardrail or rope.
5. When work is carried out at elevated levels, the area below must be free of workers.
Warning signs and barricades in the area below are advised.
6. Barriers must be constructed of metal or wood and be of adequate strength.

13.8 FLOOR OPENING :


All floor opening must be protected by one of the followings:
 Rigid guardrail and mid-rail.
 Rope guardrail and mid rail.

13.8.1 Roof and Elevated Floor Edges :


Roof and elevated floor edges must be protected by one of the followings:
A) Perimeter guardrail
B) Temporary, non-rigid barricade set back from edge at least 2 meters.
13.9 HOT WORKS :

1. All welding and cutting apparatus shall be inspected daily. Defective


apparatus and equipment shall be replaced or repaired. Regular monthly
testing and inspection is required.
2. Keep fire extinguishers available and check area before leaving.
3. Combustible items such as gas cylinders, rubber hoses and debris shall be
removed or shielded from heat, sparks and slags from welding and cutting.
4. Welding or cutting work at elevated places shall be conducted only under safe
conditions (a safety harness shall be worn). Welding or cutting in an area
where flammable vapor may be present shall be carried out carefully under the
supervision confirming with a gas detector that the area is safe and obtain a
permit for such work.
5. When welding or cutting in confined areas is required, the space shall be well
ventilated. During the work, the concentration of oxygen in the air must be
greater than 20%. When working in a confined area, the workers should at
least be paired (2 persons) to monitor hazards in preventing accidents.
6. Where electric arc welding is carried on in such a manner as to involve risk of
persons employed (other than persons employed in the welding process)
being exposed to the electric arc flash, effective provision shall be made by
screening or otherwise to prevent such exposure.
7. Domestic LPG cylinders shall not be used for any hot works.

1. between gas torch and cylinders.

13.10 FIRE PREVENTION

13.10.1Smoking :
Smoking is strictly prohibited in the Site and at material storage.

13.10.2 Fire Extinguishers :


Fire Extinguishers shall be made available at the work site, plants and offices.
Quantity, locations and type of fire extinguishers shall be approved by the Safety I/c.
13.10.3 Fire :
In case of fire, the discoverer shall immediately raise an alarm and notify the KUNAL
STRUCTURE INDIA PVT LTD..'s representative or employer’s representative

personnel or in case of nu-controlled follow the Emergency Response Procedure.


Persons working adjacent to the site of the fire shall immediately try to put out the fire
with fire extinguishers if the fire is controllable.

13.10.4 In Fire Emergency :


 Raise an alarm,
 Get the nearest fire-fighting equipment to fight the fire if it is within your capability,
 Shut off all power to machines and electrical supplies,
 Turn off all gas outlets and cylinders,

13.11 MACHINES AND EQUIPMENT


13.11.1 Machinery :
Machines are one of the leading causes of injuries. Improperly trained operators are
often the victims. So until the operator has checked the machine and is authorized to
run it – hands off! The following must be remembered when operating a machine:
a) Before turning on a machine, safety checks shall be made for:
A) People clearance
B) Guards in place
C) Emergency stops in working condition
b) The machine shall not be left running by the operator.
c) Loose clothing or jewelry shall not be worn while operating machines; also long
hair must be tied.
d) When a machine is out of order, a warning sign “Machine under Repair” must be
displayed.
e) Machines shall be kept clean.

13.11.2 Hand Tools :


Many hand tools are available for specific jobs or for specific materials. The right tool for
each job shall be used correctly and safely. The following shall be noted when using
hand tools:
1. The right size spanner to fit the nut shall be used.
2. All the files shall have a handle.
3. Chisels and punches with mushroomed heads should be ground.
4. Hammer heads shall be tightly wedged on their shafts.

5. Split wooden handles shall be renewed.


6. Edges of cutting tools shall be kept sharp.
7. Hands shall be kept behind the cutting edges when working.
8. Tools shall be kept in boxes or racks when not in use.
9. Sharp edges of tools that are to be stored or carried shall be protected.
10. Tools that are worn or damaged beyond repair shall be scrapped.
11. The correct tool for the job shall be always used.

13.11.3 Maintenance Work :


When carrying out maintenance work:
 Before maintenance work is started, it should be made sure that all machinery
comes to a complete stop.
 The safety guards shall be replaced when work is completed.
 Proper coordination between the maintenance crew and site personnel during
maintenance work shall be ensured.
13.12 ELECTRICAL
13.12.1 General :
1. Temporary electrical work shall comply with applicable Indian standards electrical
safety requirements.
2. All electrical works shall be performed by qualified and trained electrical workers.
Equipment shall be locked or secured to prevent starting by unauthorized persons.
3. Live parts of apparatus and wiring shall be effectively guarded to protect all worker or
objects from coming into contact.
4. All equipment and wiring shall be checked daily by the operator before starting work.
All electricity circuits shall be grounded.
5. Warning signs or posters, such as "DANGER", "NO ENTRY" shall be displayed at
dangerous places, such as substations, switch boxes and overhead or underground
cables.
6. All cables should be run at high level, at a height not obstructing the movement of
people and equipment.

8. Only industrial sockets shall be used.


9. Joints in the cables shall not be allowed between the local switch box and the
equipment.
10. Jointing of cables shall only be done with proper cable connectors.
11. Jointing of cables shall not be done with plastic insulation tapes.

13.12.2 Welding Machine :

1. Before welding machines are used, insulation shall be tested and certified to be in
safe operating condition.
2. Low voltage shock preventer devices shall be provided for all welding machines.
3. Welding machines shall be installed 100cm or more away from the switch box.

13.12.3 Movable Wiring :

1. Wiring shall be cab-type cable having a dielectric strength of not less than the rated
voltage of 600 V. Cable having ample capacity to the load shall be used. Cable with
any external damage shall not be used.
2. All cables shall be installed away from any steel materials such as wire rope, steel
frame, scaffold, etc.
3. Three/ Four core cables shall only be used.

13.12.4 Grounding :

1. To prevent short circuits or electric shocks, special precautions such as grounding


shall be taken for wiring work where metal scaffolds or steel structures are erected.
Grounding shall be secured by connecting the wire to an earth rod buried firmly in the
ground. Brass or steel bolts and nuts shall be used for grounding terminals of all
electrical equipment.
2. Earth Leak Circuit Breakers (ELCB): ELCB’s shall be provided to every temporary
electrical installation.
3. All distribution boards shall have earth fault relay in the incoming.
4. Power cables are to be earthed from the armor of the cables with clamps.
5. All welding generators shall be provided with double earthing. One at the neutral and
the other at the external casing.

13.12.5 Abrasive Wheels / Disc machines :

1. Abrasive wheels, cutting discs and related machines shall have the manufacturer’s
specification plate (stating voltage, maximum speed, etc.) adhered to it.
2. Hand held tools shall be equipped with an operating switch or lever requiring constant
hand or finger pressure to operate (‘Dead-man Switch”).
3. The use of such tools/equipment shall be limited to the safe design capacity of these
tools. They shall not be modified.
4. Safety guards must be in place and maintained at all times.
5. Person using such tools/equipment must be experienced/trained in the safe operation,
and authorized by his supervisor to use and operate them. They shall carry the
necessary certificates with them.
6. They shall be maintained in safe working condition for use at all times.
7. Such tools/equipment shall be inspected at least once every month, and records shall
be maintained.
8. Electrical driven tools/equipment must be checked and inspected by a licensed
electrician.
9. Defective tools/equipment shall not be used.
10. Tools/equipment found defective or unsafe, must be tagged “Unsafe - Do Not Use”.
11. Portable type must be removed from the work area immediately. Non-portable type
must be de-energized or lockout.
12. Appropriate personal protective equipment, such as safety goggles, face shields,
hand gloves etc. shall be worn when grinding or cutting.

13.15.4 Storage of Flammable Liquids :


1. All flammable liquid at project site like Diesel, Lubricating Oil, and Construction chemi-
cals etc. shall be stored separately.
2. Flammable liquid containers shall be stored on concreted floor. Dyke wall around the
storage tank / drum shall be provided to prevent loss of containment.
3. Warning Signage shall be displayed.
4. Foam type fire extinguishers and Sand buckets shall be provided near the storage area.
5. No receptacle containing flammable liquid shall be repaired unless thoroughly cleaned
and freed from the liquid.

6. Decanting of flammable liquid from the drums shall be carried out using hand pumps.

13.16 WORK PERMIT SYSTEM:

Work permit systems must to be followed for all critical activities like Excavation, Work at
height, Electrical Installation, Confined Space, Rigging etc., from respective departments
before commencement of works with prior intimation.

14. PERSONAL PROTECTIVE EQUIPMENTS (PPEs)


14.1 General
Safety helmets approved by KUNAL STRUCTURE INDIA PVT LTD.. shall be worn
with chinstraps in accordance with the following color code:

All workers shall wear suitable working clothes. They shall wear KUNAL
STRUCTURE INDIA PVT LTD.. approved safety shoes and safety helmets (with chin
straps) at the work site. Personal Protective Equipment or devices shall be provided
by respective sub-contractor as required, and their use shall be enforced.

14.2 Eye and Face Protection


Safety goggles shall be used as and when required. Workers engaged in electric arc
welding shall be required to wear helmets fitted with shields with filters of appropriate
shade and gloves where appropriate.

Workers engaged in grinding, chipping, or similar work which can create flying objects
(including using hand-held pneumatic breakers for chipping concrete) shall be
required to wear goggles to protect eyes from flying particles. Where dust is emitted,
a face mask shall be used.
14.3 Safety Harnesses / Fall Arrestors
Workers in hoppers, bins or confined spaces or on steep slopes, swinging scaffolds,
structural steel, or work at an elevation 2m or more above the ground/platform shall
be secured by full body safety harnesses with double lanyard as per IS 3521 and
anchored. Where anchorage is not available, independent lifeline (PP rope not less
than 16 mm) with fall arrestors shall be provided.

15. MAINTENANCE REGIME FOR CONSTRUCTION


EQUIPMENT AND MACHINERY

15.1 Objective
The objective of the maintenance regimes is to ensure that all equipment and
machinery used at the site are in good working order and do not present any safety
hazard due to inadequate repair and maintenance.

15.2 Procedure
1. It is a duty to provide safe, efficient and reliable equipment for the project. A list of
equipment with relevant details, test certificates and legal documents shall be
submitted to KUNAL STRUCTURE INDIA PVT LTD.. and will be kept by the Safety
Officer.
2. All equipment and machinery that is to be used on site shall be inspected by the
subcontractors’ competent person (normally mechanic) to verify that it is in a safe
working condition. The list of the above competent person and report of this
inspection will be submitted to HSE Officer.
3. All statutory equipment must be inspected by competent or approved person as
required by the Factories Act.

4. Schedule Inspection and Maintenance


 All construction equipment is to be inspected and maintained according to the
schedules.
 Records of all inspection and maintenance will be completed and maintained for
review.

16. TRAFFIC MANAGEMENT

Due to the number of vehicles and plant equipment being used on sites, it is very
important that care is taken in the site traffic / transportation procedures. The
following details are to be taken into consideration when setting up site.

1. Adequate traffic direction signs, lighting and barriers are to be erected in order to
direct the safe movement of vehicles and plant.

2. Parking areas are to be designated for commercial and private vehicles.

3. All private vehicles are to be issued a site parking permit, before they are allowed to
park on the project site parking areas.

4. Traffic signs are to be erected at the site entrance, to warn employees and the
general public of the movement of vehicles and plant in the construction site area.

5. All signs are to be inspected on a regular basis to ensure that they are in the correct
position and visible.

6. All vehicles and plant on the site are to be in possession of any relevant test or
inspection certificates.

7. For any road works or any operation near the public roads, traffic diversions shall be
organized, and approved by the relevant authority.

8. Vehicles are not to reverse in or out of the site entrance or within the site area, without
an attendant in particular when near to the public highway or footpaths.

9. Any person found breaking the site rules shall be banned from operating vehicle or
plant within the project site area, and if necessary will be removed from site.

10. Alcohol is strictly forbidden on the project site and anyone found operating plant or
vehicles while under the influence of alcohol shall be permanently removed from site.

11. Riding on the back of any vehicles and on the bucket of the any plant are not allowed.

12. All drivers/operators shall go through the Safety Induction and wear required PPE
before entering site premises.

13. A transportation route plan shall be established and can been seen and reviewed by
all personal located in various locations throughout the site office.
14. The appropriate guard rails, barriers, temporary fences or overhead protection shall
be provided for work areas involving footpath, sidewalk, and entrance to buildings
where appropriate.

15. A speed limit of 15 KMPH signs and instructional safety signs shall be posted at main
entrance of the site.

16. Speed limit within the working area is 15 KMPH.

17. The plant has an organized flow system all drivers who have permission to enter must
make themselves aware of the one way and two way system within the plant areas
and yards.

18. Adequate lighting will be provided for when working nights.

19. All site road surfaces shall be maintained in a safe condition and, whenever possible
be wide enough to allow vehicles to pass safely in opposite directions. Road surfaces
should be maintained to minimize dust nuisance. Sprinkle water regularly on main
road areas to reduce excessive dust.

20. Sharp bends, blind corners and steep grades shall be avoided whenever reasonably
practicable to do so.

21. Parking of vehicles shall be permitted on the Site only in such areas as will not cause
obstruction.

22. Only drivers with a valid driving license will be allowed to enter the office car park and
or plant. Drivers of all types of vehicles including mobile construction plant shall be in
possession of a valid license for the class of vehicle they are driving. Any driver may
be asked to produce his license at any time.

23. A vehicle entry permit system will be put in place all persons requiring entry to either
the car park or Plant must first apply through the appropriate application form, which
will be at the HSE Department.

24. Monitoring of the vehicle entry system will be by the HSE Department and Security.

25. Temporary permits may be issued for special circumstances for instance VIP visitors,
material deliveries etc., these will be obtain through the security guards and approved
by the HSE Manager.

26. Drivers / Operators are to ensure that vehicles are not overloaded and are operated
within their design capacity, and that loads are secured.

27. Wherever possible, disabled vehicles are to be removed to the side of the road and
safety cones arranged around the vehicle to indicate that the vehicle is unserviceable.

28. Learner drivers are not permitted on the site.

29. Pedestrians are to be given RIGHT OF WAY at all times within the Site compound.

All drivers shall promptly report road accidents and significant incidents to the HSE
Department and Local Police.

17. HOUSE KEEPING

Work site shall be kept reasonably clean and free of rubbish and debris, which may create
safety or fire hazards, and all materials, equipment and apparatus, shall be stored neatly in
the designated area. The work area shall be made safe, efficient and pleasant by:
a) Not leaving rubbish lying about.
b) Keeping all gangways, aisles and stairways clear.
c) Wiping up spill oil, grease or liquids.
d) Cleaning up turnings, chips of off-cuts.
e) Using metal containers for oily or greasy rags and waste.
f) Stacking goods and materials clear of gangways.
g) Storing tools safely when not in use.
h) Keeping benches and worktops uncluttered.
i) Never choking a drain line, pit holes, etc
j) Making sure flammable solvents are kept in approved containers and are only
used when needed.

DANGER, WARNING & CAUTION TAGS

 The purpose of providing Danger, Warning & Caution tags/boards/signs is for


protecting personnel, plant and equipment form injuries or damaged by
maintaining safe method of installation and starting up operational circuit’s plants
and equipment.
 All personnel will be trained on the procedure.

With regards to environment the objective is that Kunal Structure India Pvt Ltd.shall put in
use its best to prevent and take all reasonable precautions to avoid pollution or contamination
of the working location or water, arising out of its performance of the work.

It is KSIPL’s policy to manage waste in such a manner as to prevent danger to the health and
safety of its employees, sub-contractor’s employees, others and to minimize the adverse
impact on the environment. For these reasons, KSIPL has committed for controlling and
monitoring safe disposal of the various type of waste usually accumulated at the construction
locations and camps.

18.1 Dust & Noise:

Dust Control: All efforts will be made to restrict dust caused by activities to minimum.
Noise Control: All practicable measures will be taken to minimize noise less than 85 dB
arising due to operations /equipment etc.

18.2 Waste Management

Specific disposal requirement will be identified / provided to assist site management. Sub-
contractors and others will be clearly notified the goals to assist the client in maintaining the
site environment safe.

The waste management plan will observe three categories of waste:

1) Domestic and office wastes - generally non-hazardous can be disposed of in a simple


disposable facility within waste containers.
2) Industrial (Non-hazardous) waste can also be disposed of in a simple disposable facility.
3) Hazardous wastes, special or chemical, require disposal in accordance with the
environmental protection act to prevent any hazard to human health or to the environment.

18.3 Pollution & site hygiene

1. Mechanical plant, equipment, etc. which emits smoke, fumes or other obnoxious
gases should not be allowed on the site.
2. Provide and maintain temporary channels, drains and the like for keeping the site
clear of water.
3. All reasonable precautions are to be taken to ensure the efficient protection of all

streams and waterways against pollution arising out of or by reason of the execution
of the works.
4. No unwanted building debris, chemicals, any noxious or polluting matter should be
dumped on any vacant land plot of land, roadside or drains thereby causing chokage
leading to mosquito breeding or causing contamination of the site soil or the drainage
system.
5. No food is to be consumed or left in work areas.
6. Place of work to be left in a tidy and safe condition at the end of each work period.
7. Smoking is not to be allowed in work areas.
8. Where a work presents a potential hazard, appropriate notices must be supplied / dis-
played, and the area made secure as far as is reasonably practicable.
9. No paint containing lead or lead products shall be used except in the form of paste or
readymade paint.
10. Asbestos shall not be used.
11. All workers should be trained in safe manual handling as incorrect manual handling
results to back injuries. Also special objects require special handling.
12. Where the work process is such that it significantly increases the ambient noise level
in those area preventive measures such as enclosure, PPE shall be used to reduce
the noise level below the threshold limit.
13. The project HSE officer shall be informed prior to the commencement of work, pro-
curement of materials connected to the contract work of a hazardous nature.
14. All efforts are to be made to see that contamination of any product is avoided.
15. Certain areas designated as hazardous (eg. noisy areas) warning signs must be
obeyed.

16. Necessary sanitary convenience for the staff and workmen will be provided
and maintained in a clean orderly condition and clean and deodorize the ground after
removal.

As soon as the site incident controller is aware of the emergency and its location, the
site incident controller will proceed to be the scene and on arrival he will:

1. Assess the scale of emergency and decide if a major emergency situation ex-
ists or is likely. On this decision, he will activate the subsequent major emer-
gency procedure.
2. Direct all operations within the affected area with the following priorities:
 Secure the safety of personnel
 Minimize damage to equipment, property and the environment
 Minimize loss of material
3. Direct rescue and firefighting operations
4. Ensure that the affected area is searched for unattended casualties.
5. Ensure that all non-essential workers in the affected area evacuate to the ap-
propriate assembly point.
6. Direct the outside emergency services if called for emergency.
7. Pending the arrival of the site main controller, assume the duties of the post
and, in particular:
 Direct the shutting down of equipment and evaluation of areas likely
endangered by the emergency.
 Ensure that the outside emergency services have been called in.
 Ensure that key personnel have been called in.
8. Report all significant development to the site main controller. Provide advice
and information, as required, to the senior officer of the Fire Brigade.
9. Ensure the head counting for personnel.
10. Control traffic movement within the works.
11. Ensure that chronological record of the emergency to be maintained.
12. Have regard to the need to preserve evidence that would facilitate any subse-
quent enquire into the cause and circumstances of the emergency.
13. Direct the instructions about the “ALL CLEAR” communication through mobile
phones .

5.2 SITE MAIN CONTROLLER (SMC)

As soon as the site main controller is aware of the emergency, the site main
controller will proceed to the emergency control centre. On arrival, he will:

1. Relieve the site incident controller of responsibility for overall


control.
2. On declaration of a major emergency, ensure that the outside
emergency services are called in.

7. Ensure the casualties and receiving adequate attention. Ar-
range for additional help, if required. Ensure that relatives are
advised.
8. Liaise with Chief Officers of the fore and police services and
with the experts on fire, safety, health etc., provide advice on
possible effects on areas outside works.
9. Where the emergency is prolonged, arrange for the relief of per-
sonnel and the provisions of catering facilities.
10. Ensure the proper consideration is given to the preservation of
evidences for investigations.
11. Ensure with incident controller about instructions of “ALL
CLEAR” communication.
12. Ensure and control rehabilitation of affected areas.

5.3 DUTIES OF THE EMERGENCY TEAM

services and other services for quick control of the situation, he will
also perform emergency control measures as per directions from SIC /
SMC.
6. He will furnish the full details about the incident to the Senior Manage-
ment and Fire & Safety Officer (HSE), SMC / SIC, and other emer-
gency services at site.
7. He will set up an emergency control post on site for team leaders of as-
sistance groups from where the control and direction of the emergency
operation is safe and he will arrange some communication media e.g.
telephone at the emergency control post.
8. He will keep himself in constant touch with the emergency post and
continue to pass on information / receive direction from emergency
post to the concerned personnel.
9. He will ask for spare manpower from the shift in-charges of the unaf-
fected areas for the help in emergency operations, if required.
10. He will announce through paging system or any other way, in the con-
cerned affected area for evacuation of the personnel to their desig-
nated assembly point, if required.
11. He will ensure the counting of working site personnel and contract
labours in the affected area.
12. He will send the filled and written incident report to fire station and time
office if applicable.
13. He will send the filled and written injury report to the medical centre
and fire station if applicable.

5.4 DUTIES OF SHIFT INCHARGE OF UNAFFECTED AREA IN EMERGENCY

1. After getting information from emergency control Post of affected area


he will send the spare persons from his area and report to the emer-
gency post on site.
2. He will render all necessary help to the shift in-charge of the affected
areas in tackling and controlling the emergency situations as directed
from the control post.
3. He will maintain normal operating conditions of his equipments, until
gets the instructions from emergency control post / SMC / SIC.

5.5 DUTIES OF FIRE & SAFETY OFFICER (HSE) / INCHARGE

On receipt of information of emergency in the area:

1. Immediately turn out to the scene of incident with the fire & safety
equipment and fire & safety manpower.
2. Survey the situation and will work in coordination with concerned shift
in-charge.
3. Conduct fire-fighting and will tackle the emergency with fire & safety
equipment / appliances and installations as per the need of emergency
situation.
4. Carryout rescue operations without delay, if required. Render neces-
sary first aid to the injured in case of injury at site.

5. Be in constant touch with the control room / emergency control post


and continue to pass on information / receive direction from control
room / emergency control post concerning the emergency operations.
6. Ensure that all the fire & safety equipment are properly collected, be-
fore leaving the scene of emergency after control. Refills and replace-
ments must be arranged as soon as possible for the fire & safety
equipment.
7. Will conduct onsite mock drill once in every month for all workers in-
cluding sub-contractors personnel.

5.6 DUTIES OF SECTIONAL HEAD (FIRST AID)

On receipt of information regarding emergency:

1. He will immediately proceed to the First Aid centre.


2. He will be in constant touch with emergency post on site and render
medical aid to the injured brought to the first aid centre from incident
site.
3. He will decide to send the injured persons to the nearby hospitals, if
needed. Adequate support of transportation shall be provided by Ad-
ministration section.
4. He will inform about casualties to Administration section.

5.7 DUTIES OF SECURITY SHIFT INCHARGE

On receipt of information regarding emergency:

Shall immediately notify the incident to the following listed services and persons as per
directions given by the shift in-charge of the affected area:

1. Fire Station
2. First-Aid Centre / Medical In-charge
3. Site Main Controller - Project Manager
4. Site Incident Controller - HSE Manager/Safety officer
5. SH (Security)
6. SH of Concerned Unit
7. SH (Administration)
8. He will ensure that the ambulance is sent to emergency site.
9. He will ensure to give emergency information to the Second in com-
mand in case of no availability of the above said authorities.
10. In order to avoid delay in informing concerned persons / services he
will wait only for 3 rings of telephone to each, and proceed further said
order. On completion of round he shall retry to those not contacted to
ensure that none is left uninformed.
11. He will ensure about counting of the site personnel both company em-
ployees and contract labour at Assembly Point, and will inform / consult
with concerned shift in-charge any person is missing / not reported at
Assembly Point.
12. He will keep in constant touch with emergency post and remain
standby at main gate for receiving the instructions from emergency
gate.

13. He will note the timing of occurrence.


14. He will ensure that the incident has been notified to all the listed assis-
tance groups by the telephone operator.
15. He will control the movement of personnel and see that no unautho-
rized persons are allowed to enter the affected area.

5.8 DUTIES OF ADMINISTRATION INCHARGE

On receipt of information regarding emergency:

1. He will arrange the transportation for the emergency work at the site of
incident, if required, as per the requirement of SMC / SIC (Conc. Area).
2. He will tie up with nearby hospitals and fire stations and ensure that an
emergency vehicle is kept on standby at all the times.
3. He will arrange to contact the nearby hospitals, fire stations and other
administrative officers of the city, factory directorate and neighbouring
areas for emergency assistance, as per directions from SMC / SIC /
SH (First Aid).
4. If required he will release the information regarding emergency to the
public & news media.

5.9 DUTIES OF CONCERNED SECTION HEAD

On receipt of information of emergency:

1. He will ensure about the steps to localize the hazards and decide
about the operations of various equipments and units at the site. If nec-
essary, he will ensure the safe shutdown of the plant, so as to stop the
spread of hazard, with the consultation with SM / SIC.
2. He will ensure to give the inventory of hazardous materials and opera-
tions hazards to various assistance groups for emergency handling.
3. He will look after the emergency operations in the concerned area.

5.10 DUTIES OF STAFF IN AFFECTED AREAS

1. They shall act as per instructions of their Shift In-charge. Trained per-
sonnel will be nominated to assistance group B to fight the fire. Other
persons will take care of their areas as usual, unless otherwise di-
rected by the Shift In-charge.
2. They are expected not to be panicky but remain calm. If called for
evacuation they will follow instructions in an orderly disciplined manner.
3. If directed to evacuate, the shift in-charge may direct them to safe shut-
downs of running machines and evacuate in a disciplined manner.
4. After evacuation they will collect at the nearest specified assembly
point and await further instructions from the Shift In-charge.
5. Shall see that the persons assigned with specified duties in an emer-
gency are not disturbed or obstructed.

5.11 DUTIES OF THE OTHER STAFF IN UNAFFECTED AREA

They will follow the emergency signals and instructions given by Shift In-charge /

SMC / SIC.

1. They will not leave their place of work and will continue the routine
work.
2. They are expected not to be panicky but remain calm. If called for

downs of running machines and evacuate in a disciplined manner.
4. After evacuation they will collect at the nearest specified assembly

gency are not disturbed or obstructed.

6.0 PROCEDURES TO BE FOLLOWED DURING NON-WORKING HOURS

The help of most of the personnel listed earlier will not be available and hence a
Separate working procedure has to be followed:

1. All applicable duties for silent hours / enumerated earlier will apply.
2. Shift In-charge of the affected area will handle the situation with the
available manpower and services, till the designated authorities of
emergency organization reach the emergency site.

7.0 EMERGENCY CONTROL POST

Emergency control post will be set up at the time of any major emergency, by the shift
in-charge of the concerned area, for team leaders of assistance groups, from where
the control and directions of the emergency operation is safe. Communication media
e.g. telephone will also be provided at the control post.

8.0 ASSEMBLY POINTS

The assembly points are provided so as to cover the whole site area with the
consideration of wind direction, nearby paging facility and more than one road
approach on each point.

Assembly No. 1: Will be allocated during commencement of works.


Assembly No. 2: Will be allocated during commencement of works.
Assembly No. 3: Will be allocated during commencement of works.

9.0 BASIC STEPS FOR HANDLING THE VARIOUS EMERGENCIES

9.1 EMERGENCY EVACUATION PROCEDURE

1. On receipt of evacuation instructions the personnel in the affected area


shall proceed to their designated assembly point.

2. On receipt of evacuation, personnel shall close all the working gadgets,


taking care that closing or shutting down operations does not lead to
additional hazards.
3. When the emergency is announced, one security guard at the site gate
will be posted by security section and he will secure the vehicle gates
halting all routine traffic and material movement into the site. This
guard shall clear the road from the main gate so that if outside emer-
gency vehicles are needed the road be clear.
4. All personnel of the site should remain at their announced assembly
points until the clear signal is announced.
5. When the emergency announcement has been made, outside contrac-
tors on site are to be instructed to report to the gate. Their supervisors
should make a count of all personnel and report this data to the secu-
rity at the gate.
6. Evacuation of the people will be especially in the opposite direction of
exposure / wind direction.
9.2 IN CASE OF FIRE

In case of emergency, if required, help would be taken from nearby city fire station
and industries under mutual aid arrangement, in addition to our own fire fighting
systems as fixed installations and other fire fighting equipments.

9.3. IN CASE OF COLLAPSE OF BUILDING, SHED OR STRUCTURE

In case of collapse of Building , shed or structure, raise the emergency alarm, inform
the HSE In-charge and evacuate the area. Call Fire Brigade to assist in removal of
debris from the site and immediately arrange any medical help if required.

9.4 IN CASE OF GAS LEAKAGE

1. In case of gas leakage the person who detects leakages, shall immedi-
ately notify the shift in-charge about the severity of leakage and the lo-
cation.
2. During the general shift hours the shift in-charge shall notify the con-
cerned personnel as per the notification list and he will survey the
place carrying canister gas masks and determine with consultation of
senior persons that evacuation is necessary or not.
3. During silent hours, shift in-charge will make evacuation judgment him-
self after surveying the situation.
4. It is the responsibility of shift in-charge that he should inform to Medical
Centre, Control Room and Time Office (during general shift hours) and
Security Office (during silent hours) and Fire Station.
5. To ensure that all persons have got out of the affected area, an imme-
diate head count shall be made. In case someone is found missing an
immediate search should be undertaken by the persons equipped with
necessary personal equipment.

9. 5 LIQUID SPILL CONTROL PROCEDURE

1. Any spillage should be immediately localized and the area cordoned


off.
2. Shift In-charge will announce about the spillage through the public ad-
dress system and inform to the Fire Station, Medical Centre, Security
and Site In-charge.
3. Vehicular traffic will be stopped by the security staff in the affected area
of the plant.
4. It should be kept in mind that chemical spillage will not be drained to
the common drainage system from the site.

9.6 INJURTY PROCEDURE

1. In case of injury to any member, information will be sent to Medical


Centre.
2. Medical Officer will give the first aid at Medical Centre.
3. SH (Medical) will advise medical attention for the injured and decide
whether the injured person be sent to the hospital immediately for
treatment.

9.7 STORM

Shift In-charge will take the following actions in case of storm.

1. All loose, light objects will be removed from the site, which could be-
come hazardous missiles during high winds.
2. Buckets, ladders, maintenance materials and other like things are to be
placed in the safe place.
3. Hydra, trucks, welding machines, etc. are to be placed at safe positions
/ places.
4. All construction materials i.e. beams, pipes, power equipments etc.
which cannot be moved to a safe location, they are to be lashed in
place in the best possible manner known.

9.8 RIOT OR CIVIL DISTURBANCES PROCEDURE OR BOMB THREAT OR


TERRORIST ATTACK

If there are persons on or near company property with the suspected intent of causing
damage to the property, injury to the plant personnel, disruption of normal operations,
etc. The information will be given to SH (P&A) / SH (Security) and the following steps
will be taken:

1. Security shift in-charge will be instructed to give the related details and the po-
lice services will be informed, if required.
2. The staff on duty or other personnel as given under plant personnel emer-
gency notification list will be called for alertness.
3. Security shift in-charge will instruct to the security to close and lock all the site
entry gates.

4. It will be ensured that no unauthorized person is allowed access to the site


without positive identification by the gate security staff & as per instructions
given by SH (P&A) / SH (SEC).
5. Emergency vehicles will be kept ready.
6. Medical staff at the Medical Centre will be alerted.
7. Fire & Safety staff will be alerted.
8. All shift in-charges of the site will be alerted.

9.9 FLOODS, EARTHQUAKE, AND OTHER NATURAL CALAMITIES

In case of a land slide, earthquake or any other natural calamity, the incident
controller will immediately inform the Main Site controller, who will contact the
Client Authorities, and Police for remedial actions. Client Authorities will also
be immediately informed to apprise them of the situation.

1. The staff on duty or other personnel as given under plant personnel emer-
gency notification list will be called for alertness.
2. Emergency vehicles will be kept ready.
3. Medical staff at the Medical Centre will be alerted.
4. Fire & Safety staff will be alerted.
5. All shift in-charges of the site will be alerted.

10.0 REVIEW OF EMERGENCY

A team, under the chairmanship of Project Manager, will review the situation and
decide the actions to be taken to mitigate the effect and restart the operations without
losing any time.
