Preventive maintenance refers to all sorts of maintenance performed on medical equipment for
the aim of prolonging the livelihood of that medical equipment and ensure proximal and
uninterrupted service of that medical equipment whereas for curative maintenance aims to
restore the basic function of fault equipment.
The PPM performed covers major departments including laboratory, radiology equipment, labor
ward and general OPD equipment as detailed below.

RECOMMENDATION;
PPM procedures should be performed strictly regarding technical guidance at least not less
than twice a year in general medical equipment with exceptional to laboratory equipment which
requires service after every three months for proper operation of respective equipment.
With this report I humbly request payment of the whole work performed in a total of seven days
as negotiated , amounting to five hundred twenty five thousand only/ 525,000/= (7days half
per diem basis) with the following accounting information;
