RELEASE WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

That I, __________, Filipino, of legal age, single/married, a resident of __________, acknowledge

that I have voluntarily severed my engagement with ___________ (the “COMPANY”) on ___________.
And that I hereby acknowledge my receipt from the COMPANY my last pay in the amount of
___________. (Php_______). Thus, by these presents I also hereby acknowledge having received from
the COMPANY, to my complete and full satisfaction, all compensation and benefits due to me or which
may be due to me from the COMPANY as a consequence of or arising from my engagement with the
COMPANY, or those provided by under the law or under any existing or previous agreement with respect
to my aforesaid engagement with the COMPANY.

In consideration of the foregoing, I hereby release and forever discharge the COMPANY, its
proprietors, officers, employees, representatives, agents, successors, assigns and heirs from any action,
sum of money, damages, claims and demands whatsoever, which in law or in equity I ever had, now have
or which I, my heirs, successors and assigns hereafter may have upon or by reason of any matter, cause or
thing whatsoever, up to the time of these presents the intention hereof being to completely and absolutely
release the COMPANY, its proprietors, officers, employees, representatives, agents, successors, assigns
and heirs from all the liabilities arising wholly or partially directly from my engagement with the
COMPANY.

I will not any time and in any manner whatsoever, directly or indirectly, disclose to any person or
entity any information of any kind relating to the business of the COMPANY, its manner of operations,
plans, or data of any kind.

Hence, I hereby warrant that I will institute no action, whether civil, criminal, labor or
administrative against and the COMPANY, its proprietors, officers, employees, representatives, agents,
successors, assigns and heirs. And that any and all actions which I may have commenced either solely in
my name or jointly with others before any office, board, bureau, court, or tribunal against the
COMPANY, its proprietors, officers, employees, representatives, agents, successors, assigns and heirs are
hereby deemed and considered voluntary withdrawn by me and I will no longer testify or continue to
prosecute said action(s).

Moreover, it is hereby understood and I fully agree that this Release, Waiver and Quitclaim may be
pleaded in bar to any suit or proceeding to which either I, or my heirs and assigns, may have against the
COMPANY, its proprietors, officers, employees, representatives, agents, successors, assigns and heirs in
connection with my engagement with the COMPANY. And that the payment which I have received as
provided herein should not in any way be construed as an admission of liability on the part of the
COMPANY and is voluntarily accepted by me. For this purpose, I hereby acknowledge that I have
received all amounts that are now due from the COMPANY. I also acknowledge that during the entire
period of my engagement with the COMPANY, I received and was paid all compensations, benefits and
privileges to which I was entitled under all laws by reason of such engagement.

Furthermore, I also acknowledge that my work with the COMPANY had not been characterized by
physical exertion and that I am not suffering from any ailment that may be attributable, in whole and in
part, to the nature of my work with the COMPANY as the cause or reason from aggravation thereof, and
that the working conditions and environment in my place of work as well as the entire workplace had
been ideal, safe and healthy.Finally, I declare that I have read this Release, Waiver and Quitclaim and
fully understood its contents. I further declare that that I voluntarily and willingly executed this Release,
Waiver and Quitclaim and with full knowledge of my rights under the law.
