Sir, Please refer to above cited subject matter vide which Project Highway was inspected by Authority Engineer, Dr. Mahesh Kumar i.e. me along with Team Leader, Resident Engineer and Contractor's Representative Mr. Udham Singh (Project Manager) on dated 19.04.2024 and following points are observed during site vist: L Site visit was started from at Ch, 355+000 (Flyover), where excavation for RE wall levelling pad (ind side) is found in progress. The contractor has informed that erection of RE wall will be started within 03 days. Contractor has mobilized 01 no’s of excavator, grader, roller and water tanker at site. Contractor has mobilized following staff at site: I. Ol nos PM (Mr. Udham Singh) I, 02nos DPM Il. 08 nos Site Engineer ‘The contractor has setup site office at Ch. 354+000 RHS of the project highway. Soil dumping is in progress at Ch. 352+000 (median portion). Road marking work already started, but it is stopped since last 02 days. Pot holes and cracks on newly laid BC Surfaces are observed at several locations such as Km. 3114320 RHS, between Km,323+840 to Km, 323+870 LHS (major cracks & pot holes) and Km. 341+980 LHS, which are required to be remedied immediately considering the road user’s safety. Ltd. tite DBM surfaces have been found damaged at chainages between Km 345+940 to Km 346+350 RHS, Km. 346+520 to Km.347+080 RHS, Km, 351+630 to Km, 351+650 LHS, Km. 3514900 to Km, 351+940 RHS. Jerks on the structures are observed on almost all major structures especially at Km. 308.860 RHS, Km, 313.725 RHS, 316+126 LHS, 3184430 LHS, 3244325 Both sides, 3244840 both sides, 326+920 LHS, 328+800 LHS, 331+465 RHS, 333+100 RHS and 340+800 both sides. Immediate remedial measures are required in view of the prevailing unsafe condition ‘Miscellaneous works including road furniture work Viz, Junctions, Bus Stand, Kilometre stone, Hecto-meter, Fifth Kilometre, MBCB, Signage, Signals, Safety appurtenance are not yet started. During the site inspection, it is found that thick wild vegetation and weed have grown up in the median as well as on the avenue side throughout the project length, These are unsafe for road users and construction workforce are required to be cleaned immediately. Rigid Pavement at Toll Plaza have developed cracks. Joint sealing is also damaged at some locations. Cracks on PQC are found at Km, 3414400 RHS (Near Drain), There is no progress in fixing of road signages. Only few signages have been fixed from Km. 307+600 to 333+000. Lack of safety measures are observed at site. Electrical works at underpasses, grade separators, etc. are not yet started. Power connection formalities and activities are not initiated by the Contractor till now. After site visit, meeting was held with PM at site and discussed about planning of the work. PM. informed that: a) Flyover at Ch, 355+055: - + Erection of RE wall (Jind side) will be start within 03 days. + For girder launching (04 nos) crane will arrive at site within 10 days. * Shuttering plate, reinforcement and labours will be arranged within a week for preparation of slab casting. b) MNB+VUP at Ch. 350 490: - © PM informed that he has finalized the labour team who has already worked at this structure, Labour will arrive at site within 07 days, © Work will be resume within 10 days, c) Hot mix plant: - * PM informed that they have finalized the HMP of camp no. 3. ‘+ HMP service engineer will visit within 15 days and after servicing HMP will start. ‘+ For material (aggregate) they have finalized material supplier and supplying of material within 10 days. - Page 2 of 3Fibre | tc Intercontinental Consultants Clb and Technocrats Pvt. Lid. anise d) Toll plaza at Ch. 3414375: - * PM informed that they have discussed with the previous agency engaged in this, work and work will be started within 10 days. ¢) Safety sign board and road marking: - © Road marking work is in progress. © For sign boards, PM inform that they have finalized agency. Fixing of sign boards will be started within 15 days. £) Kerb painting and crash barrier painting: - © PM informed that it will be started within 10 days. 16, Similarly, other balance work in Jind Rohtak section will be started within 15 days and complete by 30" June 2024. ‘You are instructed to make the compliance of the above observations and submit a compliance report to this office within 7 days. Any incident / accident happens due to non-maintenance and lack of the safety measures at the project highway, then EPC contractor will be solely responsible for the same and action will be initiated as per provision mentioned in the Contract Agreement.
