1.0 Description of the Incident:

At approximately 07:30hrs. CSSI civil supervisor instructed CSSI excavator operator, and
CSSI flagman, to commence mechanical excavations adjacent to the paperback inside of
the perimeter fence.at approximately 08:30hrs. CSSI civil supervisor instructed excavator
operator and flagman to move location and commence mechanical excavation for a
manhole installation adjacent to where a GRE pipe had been unearthed previously.at
approximately 09:15hrs. JGC HSE supervisor and JGC civil supervisor observed the
mechanical excavation being undertaken and immediately stopped the activity. They
informed CSSI Civil supv. That mechanical excavation cannot commence in this location
due to the GRE pipe being present and that only manual excavation is allowed.
At approximately 09:20 hrs. CSSI civil supervisor informed operator and flagman to
continue mechanical excavation. After a while, the bucket contacted and damaged the
underground GRE pipe.The incident then informed to JGC and CA personnel at
approximately 09:25hrs.

2.0 SUMMARY OF FINDINGS & PICTURES

3.0 Corrective action taken:
1. Disciplinary action must be taken against the person in charge of violation of clear and
concise procedures and instructions.
2. CSSI excavator operator, and CSSI flagman to attend the following training and
retraining:
3. • Site Induction
4. • Excavation Training
5. • Brownfield Induction
6. • STARRT Training
7. STARRT to be reinforced improved and evidence of such reinforcement.

4.0 Preventive action:


- PTW (identify the area to be excavated, tools & equipment, potential underground or
overhead lines, responsible person…etc.).
- STARRT (to be conducted before excavation works beginning with the involved crew, and
specific details of the excavation work must be discussed).
- Scan and rescan of the area to be excavated.
- Manual excavation to be handled carefully and scan every 60cm with a metal tester.
- Mechanical excavation must be carried out by a competent operator, flagman and under
JGC supervision.
- Mechanical excavation will suspend when JGC supervisor not being present.
- Sloping, Safe Access, Gas Monitoring (deeper than 01.2 meters) should carry out in every
excavation.
-Hard barricade to be installed around any excavation
