WITNESSETH

WHEREAS, WE, the abovenamed ARBs are the registered owners of agricultural land covered
by Collective Certificate of Land Ownership Award (CCLOA) pursuant to Section 5 of R.A. No. 6657, as
amended by Section 10 of R.A. 9700, embraced by OCT/TCT No. O-CLOA-1176, more particularly
described as follows:

A parcel of land with Lot No.4577, Cadastral No. PLS-24, Survey Plan No. ______________ ,
LRC Record No. ______ located at Brgy. Geguinta, Las Navas, Northern Samar, containing a total area
of ONE HUNDRED TWENTY NINE THOUSAND SEVEN HUNDRED ELEVEN (129,711) square
meters, more or less.

Technical Description
Lot 4577, Pls-24
A parcel of land, Lot 4577, Pls-24, situated at Barangay Geguinta, Municipality of Las Navas, Province of
Northern Samar, Island of Samar. Bounded on the NE, along line 1-2 by lot 3366, Pls-24; on the E, along
line 2-3 by lot 3361, Pls-24; on the S and SW, along line 3-4 by lot 4576, Pls-24; on the NW, along line 4-1
by lot 3368, Pls-24. Beginning at a point marked “l” on plan being N 68-30 W., 4858.44 m. from BLLM
No. 3, Pls-24, Las Navas, Northern Samar.

WHERE AS, WE, the above-named ARBs and legal heirs of the deceased ARB, have agreed to
terminate the collective ownership and for this purpose, have cause the CCLOA be parcelized and the
subsequent issuance of Computerized Titles (c-Titles) per validation and approval of DAR.

WHERE AS, WE, the above-named ARBs and legal heirs of the deceased ARB, have agreed for
a share in the parcelized agricultural land covered by CCLOA of an area not exceeding three (3) hectares
award limit to each ARB based on the following:

Actual area of occupancy

Organized group of ARBs who opted to subdivide into sub-group

WHEREAS, WE have agreed to allocate the lots through raflle draw undertaken by the
MARPO/Designated DAR Personnel _______________________________, or by among the ARBs in
the presence of _______________________________ BARC or representative of any civic organization
in the area held on ___________________________ at _______________________.

WHEREFORE, WE have agreed to the subdivision survey and commit ourselves as ARBs of our
respective lots and area allocated and on the corresponding c-Titles to be generated, registered and issued
therefrom by the Registry of Deeds (ROD).
