2024 at the MNCHS ICT room, Annex Campus, City of Masbate.

I have considered the benefits that my son/daughter will derive in this activity with the
understanding that every precautionary measure is taken care of to secure his/her safety. I shall not
hold MNCHS and the Management Committee responsible for any untoward incident that may
happen beyond their control.
