- Review JSAs, excercise Stop Work Responsibility and perform Hazard HSE officer
hunt prior to start the job.
- Review if the tasks are related to the critical task lists.
- Any tasks diviated from the plan , SWA has to be conducted.
- PPE must be worn at all time particuarly the impact gloves.
- Prepare certified and functioning equipment prior execution.
- Confirm that there is no hydrocarbon / Mecury and ensure that gas
detection system is in place and functioning.
- If the hazardous gas / Mecury is shown, the safeguards such as the
crew's competency for hazmat traning needs to be in place.
- Ensure that the crane has been inspected prior to start of any shift.
- Check the weather condition prior to perform any task.

- Perform JSAs, V&V, crane checklist, lifting plan (including drop zone Task supervisor
diagram) and SWC for lifting & rigging and drop inspection prior to work.
- Barricade working, lifting and laydown area.
- Inspect all handtools and lifting equipment before use espectially the
elevator.
- Use certifiled and correct lifting device with identification of SWL.
- Inspect and eliminate any loosen parts on lifting equipment/ loads.
- Beware of overhead loads, don’t stand under loads.
- Hands off load use adequate No's of taglines and push-pull sticks.
- Use a safety harness and tie with an appropriate securing point.
- Secure handtools with landyard.
- Make sure limit switch / interlocking system working properly by using
daily checklist before tripping the pipe
- Keep walkway and area along lifting path clear. Task supervisor
- Remove any objects causing trip hazar and highlight any trip hazards that
cannot be removed.
- No hand on loads.
- Use taglines and push/pull tool to control the direction.
- Watch your step.
- Look for potential pinch points and highlight them.
- Discuss and identify line of fire.
- Stay on proper position and ensure proper technique of body positioning.
- Only authorised personnel is allowed in the working area.
- Test the door interlock system before use.
- Verify tong operator have tight fitting clothing to prevent entanglement in
rotating component.
- Apply low torque and gradually increase up to make up torque.
- If the tong dies or slip dies loose bite, change the dies as required.
- Clean the pipe if the surface pipe are dirty or covers with oil.
- Clean the tong dies constantly with wire brush.
- Use correct size of dies.
- Do not rotate the tool without the torque lock.
- Avoid rotating the tool while the slip bowl in place to eliminate the chance
of drillpipe damage.
- Isolate Energy Source before changing out slips / troubleshooting CTJ
and slips

- Use radio with three ways communication. Task supervisor


- Check radios communication.
- Specify tasks in order.
- Confirm everybody understands the tasks.
- A banksman stays at the visible location for crane operator.
- A banksman gives orders in clear and correct signals to crane operator.
- Only banksman gives the signals except for any emergency cases.
- Clarify lifting direction to the crane oprator and captain.
- Prepare back up radios and batteries.
- Ensure a good commucation especially when the crane operaor cannot
clarly see the banksman's signal. If the communication lost, the crane
operatior has to stop the lifting operation until the communication resumes.
- Ensure the crew aware of the function mode of CTJ which is using.

- Verify that all PCEs is certified and valid. Task supervisor


- Stay away from line of fire.
- Barricade the area when performing function test the cutter.
- Verify that the safety pins, whipchecks and restraints are installed
properly.
- Verify all hose conditions is good and all connections is correct and tight
prior use.
- Release trapped pressure in pressure lines and bleed off the pressure
after tested.
- Ensure that TIW valve is in good condition.
- If having problem with well control, ensure to install TIW valve, close VBR
and open choke HCR valve to observe pressure.
- Verify well history related to any harzardous material (Mecury, H2S, etc). Task supervisor
- Conduct Mercury survey and gas testing.
- Spill kit needs to be onboard at all time.
- Clearly identify eyewatch station, first aid kit and spill kit's location.
- Verify crew's competency such as Haz Mat traning, H2S awareness
training.
- Absorbent matting needs to be placed under hose connection to absorb
any accidental spills.
- Spill containment kits to be used effectively to contain and clean up any
leaks and spills.
- Connections to be encapsulated with absorbent material and secured
with tie wraps.
- Inspect the lines for leaks on a regular basis.
- Supervisor to be informed immediately of any leakages/ spills.
- Barricade the area.
- A bucket needs to be in placed while bleeding off pressure or
disconnecting the hydraulic hoses.
